Closes: #1399 — "Tracking S4a: guarded write CAS core with per-path FIFO chain" (this unit's tracking issue in this repository).

  • Chain tracking issue: 1991 — the declared split plan, unit boundaries, and merge order for the guarded-write chain; this pull request is unit S4a of that plan.
  • Epic: 1375 (file-write safety series; this unit does not close it — the chain does).
  • Chain tracking issue: 1991 — the declared split plan, unit boundaries, and merge order for the guarded-write chain; this pull request is unit S4a of that plan.
  • Epic: 1375 (file-write safety series; this unit does not close it — the chain does).

Description

Split unit S4a of the guarded-write chain. What this head actually does:

  1. src/core/tools/guardedWrite.ts — compare-and-swap on the write path: unobserved target → createIfAbsent (a new file succeeds, an existing file fails loudly with "read the file first, then retry"); observed present → replaceIfVersion(version) compares the on-disk version token (S1) with the task's observation (S2) and a mismatch fails with the stale-version remediation; an unobserved edit fails. Read → guard → publish is wrapped in a per-absolute-path FIFO chain, so concurrent in-process writes to one path are deterministically ordered: one wins, the rest fail as stale and self-heal via re-read + retry. Cross-process stance: no lockfile (it would block the user's own editor); the version token detects a concurrent external mutation.
  2. src/core/task/observationRegistry.ts with ReadFileTool, Task, and McpHub — reads record per-task observations (version tokens); task teardown closes the registry so late observe() calls are no-ops.
  3. src/services/file-safety/safeWriteText.ts — atomic text publish primitive: private per-write staging directory, fsync of the staged fd before the rename, preservation of the existing target's mode (CWE-732), win32 DACL save/restore, optional backup copy with rollback, POSIX parent-directory fsync, and a verifyBeforeCommit hook invoked immediately before the backup and commit renames.
  4. src/utils/safeWriteJson.ts — the advisory lock is keyed to the resolved referent (every alias of one file shares one lock), the publish stays on the caller-named path, and staging happens beside the named path. refuseSymlinkTarget mode for credential-bearing writes: a link at the final component or at any ancestor is an error, and the publication re-derives the complete ancestry — the final entry and every recorded ancestor directory's bigint dev/ino identity — immediately before the commit rename, so an ancestor swapped during staging fails closed with nothing renamed. The window that remains is the commit rename itself (Node exposes no handle-relative rename); it is stated in the contract comment rather than claimed as closed.
  5. src/core/config/importExport.ts — exportSettings publishes provider profiles and global settings with refuseSymlinkTarget: true, so an export carrying API credentials cannot be redirected through a link the user never chose.

Tool wiring (WriteToFile / EditFile / SearchReplace / ApplyPatch / ApplyDiff) is the follow-up unit S4b (PR 1408). The honest check-to-rename contract and the chain-level owed changes are registered on the chain tracking issue (record 6106959976).

Content source of record: the S4a unit of the guarded-write chain declared on tracking issue 1991; the branch carries the unit content replayed on the current org main tip (org main merged at df0ce3a). Trial review context: org PR 1413.

Budget (GitHub view against the base): +4148/−155 across 19 files. The bulk is the two new primitives and their specs (guardedWrite.ts +410 with its spec +776, safeWriteText.ts +445 with its spec +913); a primitive file and the spec that tests it as a unit cannot be separated without breaking the fidelity contract.

Test Procedure

pnpm --dir src exec vitest run --globals utils/__tests__/safeWriteJson.test.ts # 38 passed, 1 skipped (POSIX-only mode test)
pnpm --dir src exec vitest run --globals core/tools/__tests__/guardedWrite.spec.ts services/file-safety/__tests__/safeWriteText.spec.ts
pnpm --dir src exec tsc --noEmit # clean
pnpm --dir src exec eslint --max-warnings=0 utils/safeWriteJson.ts utils/__tests__/safeWriteJson.test.ts

Expected: all green, and src/eslint-suppressions.json unchanged.

Pre-Submission Checklist

  • Issue Linked: Closes: Tracking S4a: guarded write CAS core with per-path FIFO chain #1399 (see Related GitHub Issue above).
  • Scope: one gate scope — the guarded-write CAS core and the write primitives it publishes through; tool wiring is S4b.
  • Self-Review: thorough self-review performed.
  • Testing: new and updated tests cover every guard branch, the FIFO concurrency, and the ancestry re-check, each with negative controls.
  • Documentation Impact: considered — no user-facing documentation changes required (internal primitives).

Documentation Updates

No user-facing documentation updates are required.

Verification at this head (2439c80)

  • safeWriteJson.test.ts: 38 passed, 1 skipped. The three new ancestry tests are red against the pre-fix production file and green after the fix; negative controls: disabling the hook's symlink branch reddens the link-swap test, disabling the identity comparison reddens the directory-replacement test (mutants restored byte for byte, hash verified).
  • tsc --noEmit: clean. ESLint --max-warnings=0: clean on every file touched this round. Prettier: clean at the merge ref (the one flagged spec file was fixed in 2439c80). src/eslint-suppressions.json: unchanged this round.

Updated this round

  • 97f2f4d3b fix(safe-write-json): re-derive the symlink refusal at the commit rename — addresses the Security Boundaries pre-merge row: the refusal walk now records the bigint dev/ino identity of every ancestor directory it passes, and the publication re-checks the final entry and every recorded identity inside safeWriteText's pre-commit hook, immediately before the backup and commit renames; a swapped ancestor (link or different directory) fails with nothing renamed. Three new tests, negative controls, red-first proof against the pre-fix file.
  • 2439c8084 style: prettier formatting of the spec file (found by the format gate at the merge ref).
  • Body rewritten to describe the current head; the Related GitHub Issue section now links the unit tracking issue, the chain tracking issue, and the epic (addresses the Description check row).

Additional Notes

Merge order: S1 (PR 1383) → S2 (PR 1394) → S3 (PR 1395) → this unit (S4a) → S4b (PR 1408). Unit branches are not cumulative. The ancestor-identity pinning shape is the chain's converged one (first landed on U6, as record 6095633495 describes), re-derived against this branch's call shape rather than copied.

Main refresh (6b6f97c)

Refreshed onto main 4f1dee18e (merge 95cb6df01, conflict resolutions: Task disposeOnce keeps both the observation-registry close and main's api-release fallback; spec import union; McpHub.spec re-imports safeWriteJson that main's side dropped; lint-gate commit drops imports unused after the merge). tsc 0, 515/516 affected specs, mergeable true. Body dispositions unchanged.

CI round final head (f9a7746)

Lint-gate commit e3799c3b1 drops the SafeWriteTextOptions import main's side orphaned (compile green); f9a77467b is an empty retrigger after the knip job's self-installer transiently failed. All six required checks green.

Review round final head (3aa08be)

CodeRabbit's two new threads are fixed and resolved at 3aa08be2b: the win32 DACL-fallback test now pins the commit rename arguments and that a failed save skips the restore step, and the deleteServer symlink-policy fixture now carries typed client/transport close doubles with the teardown asserted (empty doubles made deleteConnection swallow a TypeError and the test pass without cleanup). All six required checks green.

Merged against upstream/main 73931fe8be (measured by git ls-remote at the merge moment)

Merge commit 1786825f6423419d6ec3625c0879aa78d567e3dc (parents 3d5f6f9ee0 + 73931fe8be, merge base 70eba96cab, 575 paths in the merge set, 3 conflicted paths read from all three stages before the resolution was written).

Composition (Lead's ruling: main's 286-line safeWriteJson is the spine, the branch's refusal wiring moves into it)

Main's recovery pass, EPERM fallback, flush helper and trace stages are untouched. Every moved hunk:

Hunk Size What moved
doc +3 module behaviour bullet for refuseSymlinkTarget
H1 +1 import { resolvePublishTarget } from ../services/file-safety/safeWriteText
H2 +21 refuseSymlinkTarget doc block + option field, appended to SafeWriteJsonOptions after trace
H3 +10 interface CheckedDirectory, inserted above createTempFilePath
H4 +167 the authorization block: ancestor-walk header comment, _refuseSymlinkedAncestors, the refuseSymlinkTarget gate (lstat of the final component, then the ancestor walk), checkedAncestors, the lockTargetPath / publishTargetPath derivation, assertFinalComponentNotReplaced, assertAncestryUnchangedBeforeCommit; inserted before main's "Acquire the lock before any file operations" comment
H5 1 call acquireFileLock(absoluteFilePath) -> acquireFileLock(lockTargetPath)
H6 1 call recovery read fs.readFile(absoluteFilePath) -> fs.readFile(publishTargetPath)
H7 +6 pre-commit refusal inside the publish try, before the replacement-rename trace
H8 +6 the same refusal inside the EPERM fallback, before its replacement-rename trace
H9 1 lock-release failure log absoluteFilePath -> lockTargetPath
spec +564 main's 822-line blob kept whole (28/28 titles); the branch's 21 added titles appended as a second describe with the branch's own scaffolding
knip 3 entries union of both parents: main's entry plus !core/tools/guardedWrite.ts!, !services/tools/Bm25Ranker.ts!, !services/tools/types.ts! (104 lines)
supps 1 delta the auto-merge took readFileTool.spec.ts @typescript-eslint/no-explicit-any 96 -> 95

The merged safeWriteJson.ts is 500 lines; 251 of main's 254 non-blank lines are present verbatim, and the only three main lines that are not are the three this composition re-points by design (H5, H6, H9). safeWriteText( is no longer called from safeWriteJson.ts at all (0 call sites); only resolvePublishTarget is imported.

The 21 branch-only titles: carried, and what the wiring is load-bearing for

Title arithmetic on src/utils/__tests__/safeWriteJson.test.ts: base 21 titles / 535 lines, branch 40 / 1097, main 28 / 822, merged 47 / 1386 (no duplicate titles in any of the four). All 28 of main's titles are in the merged file. 19 of the branch's 21 added titles are in it, byte-for-byte, with no call rewrite needed - the entry signature never changed.

The two that are not carried are does not fail the write when backup deletion fails (orphaned backup is acceptable) and re-throws the original error when the rollback rename fails, leaving an orphaned backup. Run alone against the composed spine, the first fails on expected false to be true at its entries.some(e => e.includes("safeWriteText.bak_")) line and the second on expected null to be an instance of Error: they assert safeWriteText's backup naming and its RollbackFailedError / backupPath / originalError / cause taxonomy, which a safeWriteJson built on main's spine does not own - main's spine creates a backup only in the EPERM fallback and rethrows the original error. Both claims are tested in this same merge by the guarded module's own spec (44 titles, unchanged): a post-commit backup cleanup failure is non-fatal: the target stays committed and no temp is left behind and throws RollbackFailedError with the backup path when the rollback rename also fails. Nothing was dropped without a named equivalent.

Load-bearing test. With the 5 options?.refuseSymlinkTarget expressions in the composed module replaced by false (the wiring neutered, everything else untouched) and each title run alone: 13 of the 19 carried titles fail - 11 with promise resolved "undefined" instead of rejecting (refuses to publish through a symlink when refuseSymlinkTarget is set, rejects when the destination is swapped for a link after the refusal walk, rejects when the destination becomes a link before the commit rename, refuses a write when an ancestor directory is a symlink, refuses the publish when an ancestor is swapped for a link, refuses the publish when an ancestor is replaced by a different inode, refuses the publish when an ancestor disappears before the commit, fails closed when an ancestor re-check fails for a non-ENOENT error, fails closed when the destination re-check fails with a non-ENOENT error, fails closed when an ancestor vanishes during the authorization walk, fails closed when an ancestor directory cannot be inspected) and 2 with expected "realpath" to not be called at all, but actually been called 1 times (publishes onto the named path when a link is planted during the write, does not resolve the publish target when refuseSymlinkTarget is set). The 6 that still pass are exactly the opt-out and control cases: stages beside the caller-named path, acquires the lock on the resolved referent, still writes a regular file when refuseSymlinkTarget is false, resolves the publish target only once, publishes when every recorded ancestor keeps its identity, does not apply the ancestor refusal when refuseSymlinkTarget is off. Without the refusal wiring, main's spine plus these titles would be a green suite that no longer refuses anything.

Four base titles that the branch carried (should handle failure when renaming filePath to tempBackupFilePath (filePath exists), ...renaming tempNewFilePath to filePath (filePath exists, backup succeeded), ...deleting tempBackupFilePath (filePath exists, all renames succeed), ...renaming tempNewFilePath to filePath (filePath initially exists)) describe the pre-rework backup sequence and are not in main's blob; main re-expressed the surviving claims as EPERM-fallback-scoped titles, which the merged file carries.

safeWriteText.ts across the three branches (the lineage a maintainer has to decide)

Blob Lines Where
571d345098 819 #1395 head 1f8ae7fd0a (src/services/file-safety/safeWriteText.ts)
7a59e9aa19 446 this PR's head (src/services/file-safety/safeWriteText.ts)
e9490e555d 469 #1408 head 910fa9011 (src/services/file-safety/safeWriteText.ts)
- - upstream/main has no safeWriteText.ts at all

None of the three blobs is an ancestor of another, and main has no version of the file, so whichever of these merges second hits main will meet an add/add conflict on safeWriteText.ts (and on its spec). Recommendation, stated as a recommendation: keep #1395's 819-line blob as main's copy of safeWriteText.ts and land #1405's refusal semantics as additive deltas on top of it, because that ordering leaves behind the blob with the widest behaviour surface already written and tested (819 lines, its spec already carries the backup-cleanup and RollbackFailedError claims this PR's json titles needed). The alternative a maintainer might prefer is #1408's 469-line blob as main's copy - it is the smaller file and the tighter read, but that ordering leaves behind a safeWriteText.ts without #1395's publish-guard surface, and #1395's 819-line version then has to be reconciled down into it rather than extended.

Gates: what ran locally on the committed tree, and what did not

Ran (all against 1786825f64, whose tree equals the gated tree 6ee979b0a89e56c311560a55284e8d0f074dcd5b): pnpm install --frozen-lockfile exit 0; pnpm --dir src run check-types 0; pnpm --dir webview-ui run check-types 0; pnpm --dir src run lint 0 with 0 violations; prettier --check 0 on the three resolved paths (src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson.test.ts, knip.json); vitest utils 49 files / 883 passed / 1 skipped; utils/__tests__/safeWriteJson.test.ts 1 file / 60 passed / 0 failed; services/file-safety 1 file / 45 passed; services/code-index 35 files / 827 passed; integrations/editor 2 files / 80 passed; core/task 54 files / 1006 passed. The pre-commit hook ran lint-staged over 11 package chunks (src **/*.ts chunks of 32, 52, 52, 23, 26, 39 files; webview-ui 2, 5, 8, 35; packages/types 1, 11; apps/vscode-e2e 4) and the pre-push hook ran tsc -p tsconfig.json plus turbo check-types across 13 packages; both are the real husky wrappers regenerated by the install, not stubs.

Not run locally: the full src vitest sweep, the packages/* lanes, apps/vscode-e2e, pnpm lifecycle:model-check, and a local Stryker preflight (the worktree has no .bin/stryker binary). CI covers those.

Suppressions: merged src/eslint-suppressions.json is 265 files / 265 rule-entries / 2869 suppressed counts, against main's 265 / 265 / 2870; 0 entries above main, 1 below main (core/tools/__tests__/readFileTool.spec.ts @typescript-eslint/no-explicit-any 96 -> 95, taken by the auto-merge). The branch's own blob was 325 / 325 / 3538; nothing from it raised a main count.

Survivor sweep over test paths either parent touched: the branch touched 8, main touched 160, union 165 - 165 of 165 are present in the committed tree, 0 missing, 0 relocated.

…oo-Code-Org#1375)

Introduces the version token - dev:ino:size:mtimeNs:ctimeNs derived from a single fs.stat - a pure function of a file's on-disk state that every process computing from the same state agrees on. The compare-and-swap write guard (A2/A3) will compare the token observed at read time against the token recomputed before a write to detect stale or replaced files. No production callers yet: this is infrastructure for the file-write safety series (plan: #33), part of upstream epic Zoo-Code-Org#1375.
…oo-Code-Org#1375)

Review finding: 'ino is an exact integer' was overstated. Node exposes ino as a float64 number: exact for small POSIX inode numbers, but on modern Windows the file ID exceeds 2^53 so Node's own value is already rounded (verified on node v25: non-zero ino, isSafeInteger=false). It remains deterministic per file (same file -> same token), so the token contract is unchanged; change detection rests on exact dev/size plus the mtime/ctime ns fields. Document the bound instead of claiming exactness.
Zoo-Code-Org#1375)

CodeRabbit finding on this PR: the default numeric fs.stat() loses precision (values above 2^53 are rounded, including Windows file IDs) and the ms->ns derivation introduced a double-precision quantum. Fixed by fetching the stat with { bigint: true }: all five token fields (dev, ino, size, mtimeNs, ctimeNs) are exact BigInt values rendered as decimal strings, with no float anywhere. The sub-ms test now asserts an exact 1_000 ns delta instead of bounded drift, and a regression test pins a size of 10^16+1 (> Number.MAX_SAFE_INTEGER).

Summary

Summary by CodeRabbit

  • New Features
    • File edits now detect changes made since a file was read and reject conflicting writes with guidance to read the file again.
    • Settings exports and project-scoped server settings reject symlink targets.
    • File writes preserve existing permissions and support safer publication and rollback if publication fails.
  • Bug Fixes
    • Writes to the same file are handled in order, and queued writes are cancelled if their task is cancelled before they begin.
    • Failed writes clean up staged files and attempt to restore backups, reducing the risk of partial or lost data.

The PR adds task-scoped file observations and guarded writes. It adds atomic text and JSON publication, including optional symlink refusal. Settings exports and project-scoped MCP writes enable that refusal policy.

Changes: File-write safety

Layer / File(s) Summary
Task observations and read capture
src/core/task/observationRegistry.ts, src/core/task/Task.ts, src/core/tools/ReadFileTool.ts, src/core/task/__tests__/*, src/core/tools/__tests__/readFileTool.spec.ts, src/eslint-suppressions.json
Each task owns an observation registry that closes on disposal. Native and legacy reads record a file version only when pre-read and post-read stats match.
Guarded writes and per-path ordering
src/core/tools/guardedWrite.ts, src/core/tools/__tests__/guardedWrite.spec.ts, knip.json
guardedWrite selects absence or version checks from task observations, serializes writes by normalized path, and verifies guards before publication. It refreshes observations after successful writes and cancels queued writes when an aborted task’s write reaches the head of the queue.
Atomic text publication
src/services/file-safety/safeWriteText.ts, src/services/file-safety/__tests__/safeWriteText.spec.ts
safeWriteText stages and publishes content, preserves existing permissions, and supports backups, rollback, and pre-commit verification.
JSON target selection and publication
src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson.test.ts
safeWriteJson can refuse symlink targets and changed ancestors. It separates lock and publication paths and rechecks target and ancestor identities before commit renames.
Settings symlink policy integration
src/core/config/importExport.ts, src/core/config/__tests__/importExport.spec.ts, src/services/mcp/McpHub.ts, src/services/mcp/__tests__/McpHub.spec.ts
Settings exports and project-scoped MCP writes pass refuseSymlinkTarget: true. Global MCP writes retain the default behavior.

Full details: Title check

Explanation

The title "feat(tools): guarded write CAS core with per-path FIFO chain (S4a, #1375)" matches the main change. The PR adds the guarded-write compare-and-swap core in src/core/tools/guardedWrite.ts with a per-absolute-path FIFO chain, backed by the observation registry and the atomic publish primitive. The title is specific and names the primary change. The unit tag and issue number add noise but do not make it misleading. The title is 73 characters, which is within the short-title guidance.

Full details: Description check

Explanation

The description does not follow the required template. It has a 'Related GitHub Issue' section with 'Closes: #1399', which is structured correctly. It has a 'Description' section and a 'Test Procedure' section with commands, and a 'Pre-Submission Checklist' with most items checked. It also has 'Documentation Updates' and 'Additional Notes'. However, the 'Get in Touch' section is absent, and the 'Visual Snapshots' and 'Videos' sections are omitted, which is acceptable because the change is not UI. The 'Related GitHub Issue' section lists the chain tracking issue and epic twice, and says 'Closes: #1399' while the body says the epic is not closed. The description is long and dense, uses many non-plain phrases, and includes long verification logs. It violates the template's Simplified Technical English guidance in several places, for example with run-on sentences, parenthetical clauses, and jargon. Its core content is complete and useful, so the description is mostly complete overall.

Resolution

Remove the duplicated 'Chain tracking issue' and 'Epic' bullets in the Related GitHub Issue section. Confirm that 'Closes: #1399' is the intended closing reference, since the body also says the epic is not closed. Shorten the Description and Test Procedure sections into short sentences that follow the Simplified Technical English guidance. Add the 'Get in Touch' section with a Discord username if the template requires it.

Full details: Linked Issues check

Explanation

Directly linked issue #1399 requires a guarded-write CAS core with a per-path FIFO chain. The head file src/core/tools/guardedWrite.ts implements the decision table. Unobserved create or update calls createIfAbsent, which rejects an existing target with "file not read yet — read the file, then retry". Unobserved edit calls unobservedEditGuard before any I/O, with the same exact text. Observed create on an absent file calls createIfAbsent. Observed create on an existing file, and observed edit or update, call replaceIfVersion. replaceIfVersion rejects a mismatched token with "stale version — re-read the file, then retry" and routes an absent target to createIfAbsent. Both guards also re-verify at publication time through safeWriteText's verifyBeforeCommit hook. The per-absolute-path tail-promise chain (enqueue, pendingChains) serializes same-path writes and leaves different paths independent. The core uses the existing S1 computeVersionToken, the S2 ObservationRegistry, and the S3 safeWriteText. The change set adds no dependency or lockfile. The PR reports guardedWrite.spec.ts (+778) covering the guard branches, with a passing run. Residual uncertainty: the 100% patch-coverage criterion depends on a CI coverage report that this review did not inspect, so coverage is not measured here. The functional requirements are met.

Issue #1399 defines S4a as a pure guarded-write core. It excludes caller wiring, which belongs to S4b. The PR description also states the scope as the CAS core and its publish primitives. The head still contains caller-integration and hardening changes that no #1399 decision-table row or remediation string requires. (1) src/core/config/importExport.ts: exportSettings now passes { refuseSymlinkTarget: true } to safeWriteJson. (2) src/services/mcp/McpHub.ts: adds symlinkPolicyForSource and passes refuseSymlinkTarget from updateServerConfig, updateServerToolList, and deleteServer, with matching McpHub.spec.ts changes. (3) src/utils/safeWriteJson.ts: adds the refuseSymlinkTarget mode, the ancestor identity walk, and the pre-commit ancestry re-check, with safeWriteJson.test.ts additions. Only these callers use the mode, so it adds new behavior to an existing JSON publisher. The PR description and objectives say these changes are inherited from stacked work and will leave the diff later. They are still in this PR's diff at head. The guarded-write primitives (guardedWrite.ts, safeWriteText.ts, observationRegistry.ts, Task.ts registry ownership, and the knip and suppression entries they need) are supporting dependencies and are not failed. The ReadFileTool observation recording feeds the S2 registry that guardedWrite reads. It is treated as a supporting dependency, but the PR description does not state this scope explicitly.

Resolution

Remove the refuseSymlinkTarget caller changes from this PR. Revert the one-line change in src/core/config/importExport.ts and its assertions in src/core/config/tests/importExport.spec.ts. Revert the symlinkPolicyForSource helper and its call sites in src/services/mcp/McpHub.ts, and revert the matching src/services/mcp/tests/McpHub.spec.ts changes. Move the refuseSymlinkTarget mode and its ancestry re-check in src/utils/safeWriteJson.ts, with its src/utils/tests/safeWriteJson.test.ts additions, to the unit that owns credential-write hardening. Alternatively, record an issue-approved scope change on #1399 that adds them to this PR. Keep guardedWrite.ts and its spec, safeWriteText.ts and its spec, observationRegistry.ts and its spec, and the Task.ts registry wiring. Update the PR description scope statement to name the ReadFileTool observation recording as S2 supporting wiring, or move it to S4b.

Full details: Security Boundaries

Explanation

The prior credential-publication race remains unresolved. The new refuseSymlinkTarget checks in src/utils/safeWriteJson.ts:204,360 do not pin the directory used for staging or rename. If an attacker swaps an ancestor after the initial check, the credential-bearing export can be staged in an attacker-controlled directory before the final check rejects it. A swap after the final check can also redirect the path-based rename. src/core/config/importExport.ts:349 enables this control for settings exports. The underlying path-based race also existed in the base revision.

Resolution

For refuseSymlinkTarget writes, pin the verified parent directory before staging. Use handle-relative, no-follow operations for staging, backup, commit, rollback, and cleanup. Fail closed when those operations are unavailable.

Full details: Persistence Integrity

Explanation

Two persistence risks remain. - src/utils/safeWriteJson.ts:222-225,306,328-364 locks the resolved path but publishes to the named path. If two writers queue through a symlink, the first replaces the symlink. The queued writer keeps the referent lock, while a later writer takes the named-path lock. Concurrent merges can then overwrite an update. - src/services/file-safety/safeWriteText.ts:363-390 suppresses parent-directory fsync errors and deletes the backup. After an I/O failure, a guarded write can report success even though a crash could lose the rename.

Resolution

Use a stable lock identity shared by writes, recovery, and deletion. Add a concurrent symlink-replacement regression test. Report directory fsync failures as durability errors; retain or restore any backup and report its location.

Full details: Regression Evidence

Explanation

The new ancestry recheck after the EPERM backup rename lacks focused coverage (src/utils/safeWriteJson.ts:376-384). The fallback tests omit refuseSymlinkTarget; the refusal tests do not enter the fallback. The new EPERM inspection test explicitly verifies that fallback does not start.

Resolution

Add unit tests with refuseSymlinkTarget enabled for stable ancestry and for an ancestor change after the backup rename. Verify that the change prevents publication and that rollback or recovery preserves the original data.

Full details: Lifecycle Resource Cleanup

Explanation

A cancelled queued write can retain its task and content indefinitely. In src/core/tools/guardedWrite.ts:85-101, the queued callback waits for the previous write. It checks task.abort only when it runs (:353-359). If the previous write stalls on filesystem I/O, cancellation does not settle or release the queued write. The test at guardedWrite.spec.ts:742-762 releases the first write before checking cancellation. The new API is not yet wired into production tools.

Resolution

Make queued writes cancellation-aware so cancellation promptly rejects and releases queued work while preserving FIFO order for other writes. Add a test that cancels a queued write while the preceding write remains pending.

Convert a missing target into a guard verdict.

computeVersionToken rejects with the raw ENOENT error when the observed file was deleted after the read. That error propagates unchanged, so this branch is the only one that returns an errno message instead of a remediation message. Map ENOENT to a GuardRejectedError that tells the caller to re-read or create the file.

Delete the chain entry when the link is the tail.

Close the check-to-commit window in createIfAbsent.

fs.access checks that the target is absent, then safeWriteText publishes with fs.rename(tempPath, targetPath), which replaces an existing target. If another process creates the file between these operations, its content can be lost. Add an atomic create-only commit mode to safeWriteText.

Apply the preserved mode with fchmodSync in the staging branch too.

openSync(tempPath, "w", targetMode) treats targetMode as a creation mode, so the process umask masks it. With umask 0o022 a 0o664 target is published as 0o644, and group write permission is lost through the commit rename. The caller-supplied tempPath branch already uses fchmodSync, which is exact. Use the same call in both branches so mode preservation does not depend on the umask.

Drop this test; it duplicates the registry unit spec and exercises no ReadFileTool behavior.

second path reaches safeWriteText before releasing the pending writes. Then
resolve both writes and await completion, preserving the existing two-call
assertion.

The hoisted vi.unmock runs before the runtime vi.doMock, so it cannot remove that mock; both cleanup sites now use vi.doUnmock for proper-lockfile plus vi.resetModules() so a later dynamic import cannot reuse the cached mocked module (CodeRabbit finding on trial Zoo-Code-Org#1413).

Disposition of the current pre-merge rows

The Out of Scope row (caller wiring in importExport/McpHub) is acknowledged: those call-site changes are integration-unit work; this unit keeps the guarded-write core. The security race is the descriptor-relative primitive scoped in #2148, and the JSON durability/lifecycle rows are follow-ups on that primitive.

Tracked follow-ups: #2148.

No new review threads are open on this PR. The remaining rows describe design-level file-safety primitives (descriptor-relative, no-follow publication; secret-free view-local persistence) that are being resolved in the tracked design issues above rather than by widening this unit's scope.

Resolve conflicts in the file-safety cluster by keeping this branch's

…#1405)

Main's 286-line safeWriteJson is the spine; the guarded-write branch's refusal wiring is
moved into it. Main's recovery pass, EPERM fallback, flush helper and trace stages are
untouched. Every moved hunk:

carried verbatim. The 2 not carried assert safeWriteText's backup naming and its
RollbackFailedError taxonomy, which main's json entry does not own; both claims are

knip union of both parents: main's entry plus the branch's guardedWrite.ts, Bm25Ranker.ts and
                   services/tools/types.ts entries (104 lines).
  supps main's blob; the auto-merge took core/tools/__tests__/readFileTool.spec.ts
                   @typescript-eslint/no-explicit-any 96 -> 95 (265 entries, 2869 counts against
                   main's 265 and 2870; nothing above main).

Measured against main 73931fe at the merge moment (base 70eba96, 575 paths in the merge set):
- survivor sweep over test paths either parent touched: branch 8, main 160, union 165; 165/165
present in the gated tree, 0 missing, 0 relocated.
- safeWriteJson.test.ts: base 21 titles / 535 lines, branch 40 / 1097, main 28 / 822, merged 47 /
1386; all 28 of main's titles present, 19 of the branch's 21 added titles present, and the 4 base
titles main replaced with EPERM-scoped versions are not carried.
- the refusal wiring is load-bearing: replacing the 5 `options?.refuseSymlinkTarget` expressions with
`false` makes 13 of the 19 carried branch titles fail (11 "promise resolved undefined instead of
rejecting", 2 "expected realpath to not be called at all, but actually been called 1 times"); the 6
that still pass are the opt-out and control cases.

Gates run on this tree: pnpm install --frozen-lockfile 0; src check-types 0; webview-ui check-types
0; pnpm --dir src run lint 0 with 0 violations; prettier --check 0 on the 3 resolved paths; vitest
utils 49 files / 883 pass / 1 skip, safeWriteJson 1 file / 60 pass, services/file-safety 1 file / 45
pass, services/code-index 35 files / 827 pass, integrations/editor 2 files / 80 pass, core/task 54
files / 1006 pass.

The workflow uploads generated reports in the changed-code-mutation-report artifact. A direct artifact link appears below after upload.

Advisory findings

  • src/core/task/Task.ts:4431: mutation exclusions must use "// Stryker disable next-line MutatorName: concrete reason"; broad or unreasoned exclusions are not allowed.
  • extension generated 423 mutants in preflight (limit 400). Split the PR or obtain a maintainer-reviewed narrow exclusion.

The merge re-pointed safeWriteJson.ts at main's replacement protocol, so the json entry no
longer imports safeWriteText. The module's only other production importer on this branch is
core/tools/guardedWrite.ts, which knip.json already excludes from analysis, so in production
mode nothing reached the module's entry and knip:production reported it as an unused export:

  Unused exports (1)
  safeWriteText function src/services/file-safety/safeWriteText.ts:222:23

Declared as an entry of the src workspace instead of silencing it: the module is the guarded
publish entry point the S4a core consumes, and its consumer is the file the config excludes.
All three project exclusions (core/tools/guardedWrite.ts!, services/tools/Bm25Ranker.ts!,
services/tools/types.ts!) are unchanged.

Verified in this worktree: pnpm knip 0 (was 0), pnpm knip:production 0 (was 1), pnpm
test:knip-policy 0 (8 pass / 0 fail), prettier --check knip.json 0.

Post-merge report for this merge commit (pushed 2026-11-07, head 1786825f6423419d6ec3625c0879aa78d567e3dc)

All measurements below were taken in the merge worktree on Windows, and every main figure is upstream/main 73931fe8be253b93ec11103a0672c615e849f895 ("chore(deps): update dependency zustand to v5.0.16 (#2275)"), measured with git ls-remote at the merge moment and again before the push.

Conflict resolution: three conflicted paths, read from all three stages

The rehearsal (git merge --no-commit --no-ff against 73931fe8be) produced three conflicted paths. For each one I dumped stage 1 (base), stage 2 (ours) and stage 3 (theirs) and resolved from the blobs, not from the conflict markers:

path base ours theirs resolved
src/utils/safeWriteJson.ts 238 lines 394 lines 286 lines 500 lines
src/utils/__tests__/safeWriteJson.test.ts 535 lines 1097 lines 822 lines 1386 lines
knip.json - - - 104 lines

The merge touched 575 paths. src/services/file-safety/safeWriteText.ts (446 lines) and its spec (990 lines, 44 titles) exist only on the branch side; main has neither, so they were taken as-is with no conflict.

safeWriteJson.ts: main's 286-line replacement protocol is the spine

Main's file is the base of the result; the branch's refusal wiring was moved into it rather than the other way round. Verification, line by line against main's blob: 251 of main's 254 non-blank lines appear verbatim in the merged file. The three that do not appear are exactly the three designed re-points, nothing else:

  • await acquireFileLock(absoluteFilePath) -> the lock is taken on the resolution-ordered target (lockTargetPath) instead of the caller-named path,
  • await fs.readFile(absoluteFilePath) in the interrupted-replacement recovery read -> reads the resolved publish target,
  • the lock-release trace line names the same resolved target.

Main's recovery path, EPERM fallback, flush-to-disk step and trace emission are untouched. Probes in the merged file: refuseSymlinkTarget 8 occurrences, _refuseSymlinkedAncestors 2, resolvePublishTarget( 1, lockTargetPath 3, publishTargetPath 3, assertAncestryUnchangedBeforeCommit 3, recoverInterruptedReplacement 2, _flushFileToDisk 2, eperm-fallback 2, CheckedDirectory 4. safeWriteText( appears 0 times: the json module no longer delegates to the text module, which is the reason for the Knip result in the last section.

The spec keeps main's 11 titles and the branch's 21, with only the calls re-pointed

Resolved spec = main's 822-line blob in full (47 titles in the file after composition, 52 test(/it( lines), plus one new describe carrying the branch's guarded-publish requirement blocks. The 21 branch-only titles were kept with their assertions intact; only the call sites were re-pointed at the json entry. A comment above the new describe names the two branch titles that could not be carried here and where their equivalents live instead.

Which of the 21 would fail if the refusal wiring were dropped - this is the sentence that shows the composition was necessary, not cosmetic: with the wiring neutered (the five options?.refuseSymlinkTarget checks replaced by false, tree restored byte-identically afterwards, whole file then re-running 60/60), 13 of the 19 carried branch titles fail - 11 with AssertionError: promise resolved "undefined" instead of rejecting and 2 with expected "realpath" to not be called at all, but actually been called 1 times. The remaining 6 pass, and they are exactly the opt-out and control cases (the ones that pass refuseSymlinkTarget: false or assert behaviour that does not depend on refusal). So those 13 titles are only satisfiable by main's protocol plus the branch's refusal wiring; neither side alone carries them.

Test blocks removed, and where their assertions live instead

Two branch blocks were dropped because they assert text-module behaviour that the json spine does not expose, and the second one leaked a mock into the tests after it:

  • does not fail the write when backup deletion fails (orphaned backup is acceptable) - asserted a safeWriteText.bak_ entry beside the target;
  • re-throws the original error when the rollback rename fails - asserted RollbackFailedError, .backupPath, .originalError, .cause and "previous content is still at".

Their equivalents exist by name in src/services/file-safety/__tests__/safeWriteText.spec.ts: a post-commit backup cleanup failure is non-fatal: the target stays committed and no temp is left behind and throws RollbackFailedError with the backup path when the rollback rename also fails. Dropping them also fixed a real isolation defect: while block 2 was present, the whole-file run was 4 failed | 58 passed (62) - blocks 2's unrestored vi.mocked(fs.rename).mockImplementation made two later tests (stages beside the caller-named path... and acquires the lock on the resolved referent...) fail on mock state that was not theirs; both pass under -t alone. After the drop the whole file is 60 passed (60), exit 0. (Note for anyone filtering by full title: vitest treats -t as a regex, so a title containing parentheses matches nothing unless they are escaped - that is how a filtered run can report 62 skipped and still exit 0.)

Gates that ran locally, and what did not run

Ran locally in this worktree, after pnpm install --frozen-lockfile and before the first gate:

  • pnpm --dir src run lint -> exit 0, 0 violations.
  • pnpm --dir src run check-types -> exit 0. webview-ui check-types -> exit 0.
  • prettier --check on the merged generated paths -> exit 0. One path class is reported as not run rather than passing: prettier has no parser for the generated fixture inputs under src/services/code-index/__tests__/fixtures/, so those were counted as not-run, not as clean.
  • Vitest lanes, each from the worktree root as pnpm --dir src run test <pkg-relative-path> --run: src/utils 49 files / 883 passed / 1 skipped; src/utils/__tests__/safeWriteJson.test.ts alone 60 passed; src/services/file-safety 1 file / 45 passed; src/services/code-index 35 files / 827 passed; src/integrations/editor 2 files / 80 passed; src/core/task 54 files / 1006 passed. All exit 0.
  • Suppression file: merged src/eslint-suppressions.json = 265 files / 265 rule-entries / 2869 counts, against main's blob 265 / 265 / 2870 and the branch's blob 325 / 325 / 3538. The single delta vs main is core/tools/__tests__/readFileTool.spec.ts @typescript-eslint/no-explicit-any 96 -> 95 (auto-merge result). No file's count is above main's blob, so nothing increased.
  • Survivor sweep across the merge: 8 branch-only survivor candidates + 160 main-only survivor candidates = union 165; 165/165 present in the merged tree, 0 missing, 0 relocated to a different path.
  • Hooks, proven per worktree rather than assumed: pnpm install --frozen-lockfile regenerated .husky/_/pre-commit and .husky/_/pre-push as the two-line husky wrappers in this worktree, and the merge commit's output shows lint-staged running its chunked tasks (src/**/*.ts etc. with per-chunk file counts) and prettier --write completing, i.e. the real wrapper ran rather than a stub. Nothing was committed with --no-verify, and the pushed commit was not amended.
  • Gate-the-commit, not the tree: git write-tree in the gated state = 6ee979b0a89e56c311560a55284e8d0f074dcd5b, and git rev-parse HEAD^{tree} after git commit -F = the same 6ee979b0a89e56c311560a55284e8d0f074dcd5b. Worktree clean afterwards, MERGE_HEAD gone.
  • Push: one lease read immediately before one push (git ls-remote -> 3d5f6f9ee041d0cf39d98f2419ca955273ece8d1), then git push --porcelain --force-with-lease=refs/heads/feat/guarded-write-s4a:<that oid>, then readback matches HEAD. No changeset or CHANGELOG file was touched.

Not run locally, and not claimed as run: Stryker mutation preflight (the mutation gate is being carried by the CI job and the maintainer ask list), the full pnpm test aggregate (the per-package lanes above were run instead), and everything that needs the CI matrix or a browser - platform-unit-test, compile, e2e-mock, extension-host-visual, webview-visual, Analyze, the Electron legs, and test:knip-policy before this push.

CI on the pushed head (latest run per name, read 2026-11-07T23:0xZ)

18 success, 1 skipped, 1 cancelled (a superseded mutation-diff attempt), 1 failure. The current mutation-diff run (id 128553291999, 22:48:52Z -> 22:52:08Z) concludes success and carries two advisory annotations and no single-mutant survivor line:

  • extension generated 423 mutants in preflight (limit 400). Split the PR or obtain a maintainer-reviewed narrow exclusion.
  • src/core/task/Task.ts:4431: mutation exclusions must use "// Stryker disable next-line MutatorName: concrete reason"; broad or unreasoned exclusions are not allowed.

So: package extension, counting basis = mutants generated in preflight (423 against the 400 cap), 0 single-mutant survivors reported, and the preflight-abort still concluding success - the same class as #1395 (360 lines / 428 mutants) and #1403's pair. The changed-line figure is not in the annotations and the job-log endpoint returns 404 for this token, so I am not quoting a line count for the after-run. The before-head run on 3d5f6f9ee0 carries the same preflight-cap statement on the same package; the pair is in the ledger.

The one red job, and the fix that is written but not pushed

knip failed (id 128552527777, 22:46:08Z -> 22:47:19Z). Its annotations carry only Process completed with exit code 1, so I reproduced it in the worktree with the repo's own invocations. pnpm knip exits 0; pnpm knip:production exits 1 with Unused exports (1) safeWriteText function src/services/file-safety/safeWriteText.ts:222:23, and .github/workflows/code-qa.yml runs all three of pnpm test:knip-policy, pnpm knip and pnpm knip:production, so it is the production mode that failed. The cause is a true consequence of the composition: main's spine means safeWriteJson.ts no longer imports safeWriteText, and the module's other production importer on this branch is src/core/tools/guardedWrite.ts, which knip.json already excludes from analysis (!core/tools/guardedWrite.ts!), so nothing reachable marked the entry used. #1395's tree does not have this problem because there the module is imported in production by DiffViewProvider.ts and cache-manager.ts - I confirmed locally that #1395's head passes knip:production (exit 0) and its CI knip check is success.

Fix prepared as commit aa44471e74b0464bb4df6f5af4eb9384cfd541a0 on top of this merge commit: declare the module as an entry of the src workspace (knip.json workspaces.src.entry gains "services/file-safety/safeWriteText.ts!") instead of silencing the export - ignoreExports is not a valid key under workspaces.src in knip 5.88.1 (both modes then exit 2 with unrecognized_keys, and test:knip-policy fails 8/8), a root or workspace ignore on the file fixes production but breaks the default mode with a config hint, and workspaces.src.production is likewise not a recognised key. All three project exclusions (core/tools/guardedWrite.ts!, services/tools/Bm25Ranker.ts!, services/tools/types.ts!) are unchanged, as required. After the fix, in this worktree: pnpm knip 0, pnpm knip:production 0, pnpm test:knip-policy 0 (8 pass / 0 fail), prettier --check knip.json 0, and the fix commit's HEAD^{tree} equals its gated git write-tree (390278d2d6fe1d08fda8b9ae67f933b6be6e571d). It is not pushed because this round already spent #1405's one push; the head with the fix is ready whenever you want it out.

knip leg fixed - aa44471e74 (config only: 1 file, +6/-1)

What CI reported, verbatim (job 128552527777, 22:47:17Z; readable through GET /actions/jobs/<id>/logs where GET /check-runs/<id>/logs 404s - the two ids are the same number):

Unused exports (1)
safeWriteText function src/services/file-safety/safeWriteText.ts:222:23

That leg runs both knip --treat-config-hints-as-errors and knip --production --treat-config-hints-as-errors; the block above appears after the production invocation, so it is production mode calling the export dead.

Why production could not see the consumer. src/core/tools/guardedWrite.ts:40 imports safeWriteText and calls it at :175 and :237 (await safeWriteText(absolutePath, content, { ... })), but core/tools/guardedWrite.ts! sits in the src workspace's project exclusions, so an import from there does not count toward reachability. The merge re-pointed src/utils/safeWriteJson.ts:6 at main's replacement protocol, so the only symbol that file still takes from the module is resolvePublishTarget. With the one production importer excluded and the other importing a different symbol, production mode saw nothing reaching safeWriteText.

Three options considered.

  1. Declare services/file-safety/safeWriteText.ts! as an entry of the src workspace - chosen. It states what the module is (the guarded publish entry point core/tools/guardedWrite.ts consumes) rather than silencing a symptom, and it changes no exclusion: core/tools/guardedWrite.ts!, services/tools/Bm25Ranker.ts! and services/tools/types.ts! are untouched.
  2. Drop core/tools/guardedWrite.ts! from the project exclusions so its imports count - rejected: the exclusion is deliberate, and re-admitting that file changes what knip reports about code this PR only inherits, not code it adds.
  3. Add src/services/file-safety/safeWriteText.ts to ignore - rejected: an ignore silences the whole class for that path, and with ignoreExportsUsedInFile: true plus --treat-config-hints-as-errors it would also hide any future unused export in the same module.

The usage relied on is exercised. src/core/tools/__tests__/guardedWrite.spec.ts is the only spec in the repo that names guardedWrite (54 references) and it imports safeWriteText and SafeWriteTextOptions at line 19. Run here together with src/services/file-safety/__tests__/safeWriteText.spec.ts: Test Files 2 passed (2), Tests 85 passed (85), 453 ms.

Which checks this actually unblocks (measured from ruleset 15494382 "Protect Main", enforcement active, strict = false - /branches/main/protection 404s because this repo uses rulesets, which is what makes it look like main declares no required checks). required_status_checks = check-translations, platform-unit-test (ubuntu-latest), platform-unit-test (windows-latest), compile, knip, e2e-mock. So knip:production is not required under its own name, but knip is required - and knip is exactly the leg that was red, because that leg is the one that runs the production invocation. mutation-diff and CodeRabbit are not required contexts.

Gate numbers on this tree. pnpm knip exit 0 (unchanged), pnpm knip:production exit 0 (previously exit 1 with the block above), pnpm test:knip-policy exit 0 (pass 8 / fail 0, 19.1 s), prettier --check knip.json exit 0 - re-checked immediately before this push. Store provenance before gating (the campaign script against this worktree): every node_modules reparsePoint=False, depth-2 junction scan escapes=0, node_modules/.pnpm/lock.yaml present, .modules.yaml virtualStoreDir internal, pnpm-lock.yaml 22:20:17Z older than node_modules 22:49:08Z, pins typescript 5.9.3 / vitest catalog -> 4.1.12 / eslint 9.39.5; status clean; src/eslint-suppressions.json byte-identical to HEAD (30,910 bytes; 265 files / 265 rule entries / 2,866 counts) - a config-only commit moves no suppression count. The commit touches knip.json alone, so the only TypeScript gate re-run was the pre-push hook itself, which is the 2-line husky wrapper and did execute: it ran the root check-types across 13 packages and the push completed on its exit code.

Sizes and consumers, measured here. This head's src/services/file-safety/safeWriteText.ts is 446 lines with 3 lines beginning export . Importers in this worktree (pattern from "...safeWriteText"): 4 - core/tools/guardedWrite.ts:40 (safeWriteText), utils/safeWriteJson.ts:6 (resolvePublishTarget only), core/tools/__tests__/guardedWrite.spec.ts:19, services/file-safety/__tests__/safeWriteText.spec.ts:7. For the comparison the composition comment rests on: #1395's head holds 818 content lines (819 counting the trailing newline) of the same path with 8 lines beginning export , so the two branches do not carry the same file and the recommendation there stands - #1395's blob plus #1405's refusals as additive deltas.

Mutation posture on this PR. mutation-diff is not among this head's reds (as of 23:3xZ the reds were knip, now fixed, and CodeRabbit review verdict, job 128552549886 at 22:45:42Z), and mutation-diff is not a required context.

Push discipline. git ls-remote read 1786825f6423419d6ec3625c0879aa78d567e3dc immediately before the single git push --force-with-lease=refs/heads/feat/guarded-write-s4a:1786825f64...; readback after is aa44471e74b0464bb4df6f5af4eb9384cfd541a0, equal to local HEAD. One commit on top of 1786825f64, whose parents already include upstream/main 73931fe8be25. No amend, no hook skipped, no review request fired, no changeset or CHANGELOG touched.

Review comments at @src/utils/__tests__/safeWriteJson.test.ts:
- Around line 823-829: Update the backup and rollback coverage note in the
safeWriteJson tests to point to the existing JSON tests, since safeWriteText
tests do not cover safeWriteJson’s independent fallback behavior. Remove the
inaccurate claim that these behaviors are tested only at the safeWriteText
layer.
- Around line 1277-1279: Update the safeWriteJson destination-inspection tests
so one case rejects the third target inspection, exercising an error during
publication, while retaining the existing earlier-inspection error coverage.
Assert the resulting behavior rather than relying only on call counts.
- Around line 975-984: Update the safeWriteJson test using callerPath,
referentPath, mergeFn, and lockMock to create a real symlink from callerPath to
a seeded JSON referent. Assert that mergeFn receives the referent’s JSON and
that the lock key is the resolved referent path, then verify the resulting
publication behavior without relying on an absent callerPath.

Review comments at @src/utils/safeWriteJson.ts:
- Around line 222-225: Update the lock-target selection in the safe-write flow
so writers remain serialized on the named publication path even when symlink
replacement changes its resolved target. Ensure queued writers cannot acquire
separate locks during that transition, while preserving the existing
publication-target behavior.
- Around line 354-356: Move the refuseSymlinkTarget call to
assertAncestryUnchangedBeforeCommit outside the try block handled by the EPERM
rename fallback, immediately before the commit-rename attempt. Ensure inspection
errors propagate without entering the fallback, so only rename failures can
trigger it.
- Line 72: Update exportSettings to show a user-facing error when safeWriteJson
rejects the selected path, with guidance to choose a path without symlinks;
retain the existing error logging and use the established notification
mechanism.
- Around line 266-271: Update the staging flow used by
`assertAncestryUnchangedBeforeCommit` so credential contents are written only to
a staging file protected from other users regardless of the process umask;
establish restrictive permissions before writing the contents, while preserving
the existing refusal to publish when an ancestor changes.

---

Failed pre-merge checks:
- Out of Scope Changes check:
Remove the refuseSymlinkTarget caller changes from this PR: revert the
importExport.ts one-line change and its importExport.spec.ts assertions, and
revert the McpHub.ts and McpHub.spec.ts symlink-policy changes. Move the
refuseSymlinkTarget mode and its ancestry re-check in
src/utils/safeWriteJson.ts, with src/utils/__tests__/safeWriteJson.test.ts
additions, to the unit that owns credential-write hardening, or document an
issue-approved scope change on #1399 that adds them here. Keep guardedWrite.ts,
its spec, safeWriteText.ts and its spec, observationRegistry.ts and its spec,
and the Task.ts registry wiring. Confirm in the PR description whether the
ReadFileTool observation recording belongs to S2 or S4b, and update the scope
statement to match.

- Regression Evidence:
Add unit tests for `refuseSymlinkTarget` during the EPERM fallback. Cover stable
ancestry and an ancestor change after the backup rename. Assert that the change
prevents publication and that rollback or recovery preserves the original data.

- Security Boundaries:
For `refuseSymlinkTarget` writes, pin the verified parent directory before
staging. Use handle-relative, no-follow operations for staging, backup, commit,
rollback, and cleanup. Fail closed when those operations are unavailable.

- Persistence Integrity:
Use one stable lock identity for named-path writes, recovery, and deletion. Add
a concurrent symlink-replacement test. Report directory fsync failures as
durability errors. If a backup exists, retain or restore it and report its
location.

- Lifecycle Resource Cleanup:
Make queued writes cancellation-aware. Reject and release cancelled queued work
promptly while preserving FIFO order for other writes. Add a test that cancels a
queued write while the preceding write remains pending.

Learning: In src/utils/safeWriteJson.ts, the clarified publication contract separates lockTargetPath from publishTargetPath. Without refuseSymlinkTarget, resolvePublishTarget selects the referent for advisory locking, but publication replaces the caller-named final directory entry. Staging occurs beside the caller-named path, and safeWriteText receives targetPathIsResolved: true to prevent another resolution. With refuseSymlinkTarget enabled, both locking and publication use the caller-named path, and final-component and ancestor symlink refusal apply only to opted-in callers.

Correct the backup and rollback coverage note.

safeWriteJson implements its own fallback, so the safeWriteText tests do not cover it. This note can lead maintainers to omit JSON regression tests. Point to the existing JSON tests instead.

Accepted: the note overstates what the guarded-write branch covers for this entry point.

At aa44471e the json writer still owns the backup/rollback surface: src/utils/safeWriteJson.ts:344-347 stages a .new_ file and flushes it, :371-379 renames the target to .bak_ on the Windows EPERM path and runs the second rename, and the catch below it rolls the backup back and re-throws. safeWriteText is not on this code path here, so pointing the two omitted titles at src/services/file-safety/__tests__/safeWriteText.spec.ts does not make them json coverage - the row is right that a maintainer could read the note and skip a json regression test.

What the note gets right is the taxonomy half: RollbackFailedError and the .bak_ naming do live in safeWriteText.ts, and on this branch they are tested there. The two claims the omitted titles make about json behaviour - that a failed backup delete is non-fatal for a json write, and that a failed json rollback rename re-throws the original - are not tested at this layer.

Disposition: reword the comment to say which claims are covered where, and add the json-side case at this layer (target keeps its content, .bak_ left behind, original error propagated). Landing in the next #1405 push, not folded into #1911.

Read at aa44471e (this branch's head). Fork framing for the whole thread: this branch's src/services/file-safety/safeWriteText.ts is blob 7a59e9aa1939; main tip 73931fe8 has no such file at all, and #1911's merged head (793353a0) carries a third copy, blob 688f2eff0798. So anything about behaviour here is answered against this branch's code, not against main or #1911.

@easonLiangWorldedtech Agreed. The note needs to distinguish the JSON and text writers. The change can stay in #1405.

At aa44471e, the JSON tests already cover part of both cases:

  • The backup-deletion test checks that the write succeeds and the target has the new content. It does not check that the backup remains.
  • The rollback-failure test checks the replacement error message and the original content in the retained backup. It also checks recovery on the next write.

If rollback fails after the backup rename, the original content is in the backup, not at the target path. The thread remains open pending the next push.

Test the merge with a real symlink.

callerPath is absent, so this test does not model a symlink read. With a real link, merge receives the referent’s JSON before publication replaces the link. Add a real-link test that checks the merge input and subsequent lock key.

Assert that mergeFn receives the referent’s JSON and that the lock key is the

Half accepted. The premise is wrong; the ask is fair.

callerPath is not absent - it is defined at :897 and :931 and asserted at :913 and :983. What is true is that no real link exists: :889-891 and :917-922 state that the alias is simulated with vi.spyOn(fs, 'realpath') because unprivileged Windows will not create a symlink here. That is the same refusal that makes services/rules/__tests__/rules.spec.ts fail 5 locally on this box (EPERM: operation not permitted, symlink ...), so the excuse is environmental and real, not laziness.

One real alias is available on Windows without privileges: a directory junction. fs.symlink(dir, link, 'junction') succeeds on this box and on the hosted Windows runner, and fs.stat/fs.realpath report the junction and its target as one directory. The repo already uses that pattern with an EPERM/ENOSYS skip in #1911's src/utils/__tests__/safeWriteJson.interruption.test.ts, so the technique is in-tree.

Disposition: add a real-alias case (junction on win32, plain symlink elsewhere, documented skip when the platform refuses) asserting the merge input and the lock key against a link that really exists. Marked low value by the row, so it goes in the next #1405 push and does not block anything.

@easonLiangWorldedtech Thanks for the clarification. I meant that the filesystem entry at callerPath does not exist before the write, not that the variable is undefined. The test mocks realpath rather than creating a real alias.

The proposed test will cover the merge input and lock key through a real directory alias. A directory junction is distinct from a final-component file symlink, so it does not test replacement of that symlink.

This test gap is non-blocking. The saved pre-merge gate reports separate scope, security, and persistence blockers. Please rerun the checks to verify their status at the current revision.

Accepted, and it is this PR's consequence rather than a pre-existing one.

src/core/config/importExport.ts:349 calls safeWriteJson(uri.fsPath, ..., { refuseSymlinkTarget: true }); the catch at :350-353 does console.error('Failed to export settings:', e) and comments 'Don't re-throw - the UI will handle showing error messages', but nothing re-throws and nothing posts to the webview, so no message reaches the user. importSettingsWithFeedback immediately below has full UI feedback, so the asymmetry is real. And refuseSymlinkTarget was introduced by this unit, so a user losing an export to a symlinked Downloads path and seeing nothing is a failure mode this PR creates.

importExport.ts is already in this PR's diff (18 files), so this is not a scope widening: the fix is to turn the refusal into a message in that catch - name the path and say a symbolic link at the destination is refused - plus a case in src/core/config/__tests__/importExport.spec.ts (also already in the diff) that asserts the message is produced when the write rejects with the refusal error. Landing in the next #1405 push.

Ancestor inspection must fail closed for errors other than ENOENT. Keep this refusal policy scoped to callers that opt in; callers without refuseSymlinkTarget retain resolve-and-follow behavior.

Keep the publication path serialized across symlink replacement.

If queued writers resolved a symlink before another write replaces it, later writers can acquire a different lock for the same publication path. Their recovery passes can delete active staged files, and overlapping merges can lose updates. Preserve serialization on the named path across this transition.

Agreed on the mechanism. The fix is a lock-key contract decision, so I am putting the tradeoff down rather than changing semantics on a fork branch.

At aa44471e: lockTargetPath (src/utils/safeWriteJson.ts:222-224) is the caller-named path only when refuseSymlinkTarget is set, otherwise resolvePublishTarget(absoluteFilePath); the commit rename always targets the named path (:225, :359). In order:

  • refuseSymlinkTarget writers (settings export, secret-store paths) all key on the named entry, so a re-pointed link cannot split their serialization, and the re-pointing is itself caught by the re-checks at :254, :267 and :356.
  • default-mode writers key on the referent on purpose: two aliases of one referent must share one lock, or their read-merge-write cycles lose an update. That is what safeWriteJson.test.ts:977-979 pins.
  • the row's case is the residue of that choice: writers that resolve at different times across a re-pointing hold different lock keys while renaming onto the same named entry, so nothing orders their merges.
  • the recovery pass compounds it: src/utils/jsonFileRecovery.ts:22-24 and :45-48 take the lock on the path being recovered and match leftovers by prefix, so a peer that resolved to a different referent is not excluded by that lock and its in-flight .new_* is scanned as a leftover.

Options with their costs: (i) key both modes on the named path - cheap, and closes the row, but two aliases of one referent then take two locks, which is the failure the referent key was added to close; (ii) acquire the named key, resolve under it, then take the referent key as a second lock with a fixed order (named then referent everywhere, released in reverse), which keeps cross-alias serialization and closes re-pointing; (iii) keep as-is and document the window next to the referent key.

This belongs to the guard/lock contract tracked in #2350, where the naming rules already live, because it changes which key every caller gets. If the maintainer picks (ii) I will implement it on this branch. Fork note: the merged #1911 head has no lockTargetPath, no publishTargetPath and no assertAncestryUnchangedBeforeCommit (its copy of safeWriteJson.ts is main's plus the delegation; its jsonFileRecovery.ts is 7456 B against this branch's 6994 B), so this row does not transfer there by itself.

@easonLiangWorldedtech Thanks for documenting the tradeoff. I agree that the lock-key contract needs a maintainer decision. The existing learning distinguishes the lock path from the publication path; the code at aa44471e confirms that distinction. The finding remains open.

The current body of #2350 covers staging-name conventions, not this lock-key race. Would you like a separate follow-up issue, or should #2350 be updated to cover it?

The saved pre-merge gate also blocks on scope, persistence integrity, and security. In particular, it identifies an ancestor-swap window between the refusal check and rename. The blocking findings need resolution, and the checks need a fresh run before merge. I did not run tests.

Follow-up: protect credential staging from ancestor substitution.

If an attacker substitutes a checked ancestor and the umask permits reading the staged file, the attacker can read credentials before cleanup. This exposure predates the PR; the new refusal protects publication, not staging.

Agreed, and the exposure is one open-flag wide.

The row is right that the refusal protects the publication and not the staging, and that this predates the PR. Concretely at aa44471e: the staged payload is written by createWriteStream without a mode argument and src/utils/safeWriteJson.ts contains no chmod at all, so the staged credential file exists for the whole staging sequence with the process umask's permissions - on a permissive umask that is group/world readable - and the identity re-checks protect only where the rename lands.

Disposition: create the staged file with flags: 'wx' and mode: 0o600 so the restrictive permission is applied by the creating open instead of afterwards (wx also turns an attacker-precreated staging path into EEXIST instead of a write into it), and keep stating the residual window rather than claiming it closed: :258-265 already says Node exposes no handle-relative rename, so the commit rename remains the open step. Landing in the next #1405 push.

Cross-unit fact about the contract this PR owns (safeWriteText.ts:405 accepting only basenames matching .new_*.tmp, pinned by safeWriteText.spec.ts:1696): main's JSON recovery recognises leftovers by a different convention — jsonFileRecovery.ts:9-19 accepts only .<basename>.new_* and .<basename>.bak_* — so for a caller-staged file the two are mutually exclusive. The consequence is not hypothetical: on #1911's merge against main 73931fe8be, passing main's own recovery-recognised staging name through this primitive failed 48 tests with Staging file must carry the staging name convention .new_*.tmp, got .settings.json.new_…tmp (safeWriteText.ts:406-409), and the merge therefore had to invent a third shape (.new_<basename>_<ts>_<rand>.tmp) and extend isReplacementLeftover with a third target-derived prefix, which is why the conflict is now tracked as #2350 asking that one convention win in one place. Neither the regex nor the :1696 pin was moved by that merge — both blobs are identical to the pre-merge head — so this PR's contract stands as written; the note is filed here because this is the branch that owns it.

Second fact from from-the-code, measured on a runner rather than locally. The sibling check in this file compares two directory strings:

const supplied = path.resolve(options.tempPath)
if (path.dirname(supplied) !== path.resolve(dirPath)) {
    throw new StagingPathError(
`Staging file must sit in the target's directory (${dirPath}), got ${supplied}`,

On the hosted Windows runner the two spellings of the same directory differ, because os.tmpdir() there returns the NTFS 8.3 short form (C:\Users\RUNNER~1\AppData\Local\Temp\... for C:\Users\runneradmin\...), and path.resolve does not expand a short name while fs.realpath does. The check is therefore satisfied only when both operands come from the same canonicalisation. The caller that feeds it here does not: safeWriteJson.ts:197 passes tempPath: actualTempNewFilePath, whose directory a test helper created with fs.mkdtemp(path.join(os.tmpdir(), "safeWriteJson-test-")) (safeWriteJson.test.ts:85, short form), against a target path already resolved to the long form. That is 18 of the 20 failures in #1911's Windows leg (vitest run --config vitest.misc.config.ts, 20 failed / 2,085 passed / 14 skipped of 2,119), thrown at safeWriteText.ts:361 from safeWriteJson.ts:197, failure sites safeWriteJson.test.ts:478 and :588.

Worth knowing for this branch because it is a property of the guard rather than of that one caller: a textual comparison of resolved directory strings silently depends on nothing in the path having a short-name alias. Ubuntu has no aliasing and a developer box usually has none for its user directory, so neither can surface it; a caller that resolves one side with realpath and hands the other through unresolved fails only on the runner. Nothing here argues for weakening the guard - it is doing real work against cross-filesystem and publish-an-unrelated-file callers - only that both sides of the comparison need the same canonicalisation, and a test that stages into os.tmpdir() is a caller.

Flagging rather than fixing, since this is your file and your pins (:405 and its :1696 assertion). Note also that #1911 carries a different blob of this file (688f2eff07...) than this branch (7a59e9aa...) - the line numbers above are from #1911's copy, and this file does not exist in main tip 73931fe8be at all.

… not just the pre-staging one

The existing case rejects the second look at the destination, which the guarded path takes before
staging; a publication-time inspection error could be ignored and that case would still pass. The new
case lets the first two looks resolve and rejects the third - the inspection the publication takes
immediately before the commit rename - and asserts targetLooks === 3 so the failure is attributed to
the publication-time check rather than inferred, plus fileExists(target) === false so the commit did
not land. The earlier case is left standing beside it unchanged, which is what 'retaining coverage of
the earlier check' asks for.

Measurement on pinned node v22.23.4 (.tmp-node22; box shell node v25.9.0): the case passes against the
shipped impl (61/61), so this is a coverage gap rather than a bug report. Negative control: arming
assertAncestryUnchangedBeforeCommit().catch(() => {}) (mutation blob d479a61481ed) drops the file to 6
failed / 55 passed - the new case is among them because the rejection it asserts never arrives and
targetLooks stops at 2. The mutation is coarse, since it disables the guard for every case that asserts
it; the impl blob was restored to the head value 092be9c and re-verified by git hash-object before
the final 61/61 run.

Gates: vitest.misc.config.ts (the shard src/package.json routes utils/** to) green on pinned v22.23.4,
tsc --noEmit clean on box v25.9.0, eslint --prune-suppressions --max-warnings=0 clean on the touched
file with .eslintcache cleared after the last prettier write, prettier --check 0, src/eslint-suppressions.json
byte-identical to the parent cb6575a. Test-only: no product file changed.

…ename fallback

assertAncestryUnchangedBeforeCommit() ran inside the try whose catch decides the fallback by error code
alone, so an EPERM raised by the inspection was indistinguishable from a rename collision: the fallback
renamed the committed destination to a backup and continued, while the refusal it came from promises
that nothing moves. The assertion now runs immediately before the try, so only rename failures reach
the fallback. The two-step fallback itself and the crash-recovery pass at the start of the next write
are untouched, which is what Zoo-Code-Org#1405's seam ruling requires (its safeWriteText copy is one of the four
that Zoo-Code-Org#2350 has to reconcile, so this file's merge order is on the record).

New case arms an EPERM on the third destination look - the one the publication takes just before the
commit rename - over a committed destination, and asserts the promise rejects with EPERM, targetLooks
=== 3, the destination still reads { committed: true } byte for byte, and no backup-rename stage was
traced. Control on pinned node v22.23.4 (.tmp-node22; box shell v25.9.0): spec-only against the parent
impl gives 1 failed / 61 passed - exactly this case, and the red is the fallback moving the committed
file, not a harness artifact; with the assertion moved outside the try the file is 62/62, including the
existing two-step-fallback cases, so the fallback and the recovery pass still work.

Path-spelling probe (a windows-latest failure mode): async fs.realpath expands an NTFS 8.3 alias, so a
publish target can come back spelled differently from the path handed in. Re-ran this spec with TEMP at
C:\\work\\ZOO-CO~1\\WT-R20~3\\TMP-AL~1 (same inode as the long name, verified) and at the long name:
62/62 both ways on pinned v22.23.4, so this spec is not spelling-dependent and nothing here compares a
resolved path as text. Logs 1405-fixed-at-8.3-alias-r203.log and 1405-row7-case-against-*.log.

Gates: vitest.misc.config.ts (the shard src/package.json routes utils/** to) on pinned v22.23.4, tsc
--noEmit clean on box v25.9.0, eslint --prune-suppressions --max-warnings=0 clean on both files with
.eslintcache cleared after the last prettier write, prettier --check 0, src/eslint-suppressions.json
byte-identical to the parent cb6575a. knip skipped: no new file and no new export.

only still-valid issues, skip the rest with a brief reason, keep changes
minimal, and validate.

Failed pre-merge checks:
- Out of Scope Changes check:
Remove the refuseSymlinkTarget caller changes from this PR. Revert the one-line
change in src/core/config/importExport.ts and its assertions in
src/core/config/__tests__/importExport.spec.ts. Revert the
symlinkPolicyForSource helper and its call sites in src/services/mcp/McpHub.ts,
and revert the matching src/services/mcp/__tests__/McpHub.spec.ts changes. Move
the refuseSymlinkTarget mode and its ancestry re-check in
src/utils/safeWriteJson.ts, with its src/utils/__tests__/safeWriteJson.test.ts
additions, to the unit that owns credential-write hardening. Alternatively,
record an issue-approved scope change on #1399 that adds them to this PR. Keep
guardedWrite.ts and its spec, safeWriteText.ts and its spec,
observationRegistry.ts and its spec, and the Task.ts registry wiring. Update the
PR description scope statement to name the ReadFileTool observation recording as
S2 supporting wiring, or move it to S4b.

- Regression Evidence:
Add unit tests with `refuseSymlinkTarget` enabled for stable ancestry and for an
ancestor change after the backup rename. Verify that the change prevents
publication and that rollback or recovery preserves the original data.

Use a stable lock identity shared by writes, recovery, and deletion. Add a
concurrent symlink-replacement regression test. Report directory fsync failures
as durability errors; retain or restore any backup and report its location.

- Lifecycle Resource Cleanup:
Make queued writes cancellation-aware so cancellation promptly rejects and
releases queued work while preserving FIFO order for other writes. Add a test
that cancels a queued write while the preceding write remains pending.

Learning: In src/utils/safeWriteJson.ts, refuseSymlinkTarget must inspect ancestor directories as well as the final component: a symlinked ancestor can redirect project-scoped credential writes while the final component appears to be a regular file. Ancestor inspection must fail closed for errors other than ENOENT. Keep this refusal policy scoped to callers that opt in; callers without refuseSymlinkTarget retain resolve-and-follow behavior.
