This PR adds the bookkeeping that lets a write know whether the model actually saw the file: a task-local observation registry records when a full-file observation of a path exists, with a version token and a completion state. A partial read does not make a destination observable; move retargeting (carrying the source's completeness onto a destination) lands with the move call site in the consuming unit.

Without this, a later guarded write cannot tell "the model read this whole file" from "the model saw a slice" - the difference between allowing and refusing a full-file overwrite. It is the third of nine PRs for epic #1375 (chain tracked on #1991).

Related GitHub Issue

Closes: #2018 (this unit's tracking issue).
Refs: #1991 (chain tracking), #1375 (epic).

Description

What this PR changes:

  1. A task-local observation registry: registration, versioning and completion of observations, scoped to one task. Observations carry their creation timestamp; expiry is deferred to the consumer units that need a retention policy, matching the fidelity source for this unit.
  2. A partial read never marks a path complete. Move retargeting (source -> destination, preserving the complete flag) is introduced with the move call site in the consuming unit, which carries its own behaviour tests.
  3. A consistency port this unit carries: project-scoped safeWriteJson writes confine to a caller-supplied root (confineTo), so a project that ships .roo/mcp.json as a symlink to a file outside the workspace gets the write refused instead of replacing the outside file.

Deliberately not wired yet: the registry is not connected to production read, move, or guarded-write call sites here - those call sites are introduced by the units after this one, each with its own behaviour tests. The tests here exercise the registry's own contract.

Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on main and carries its own copy of the earlier units' content, which the GitHub diff shows as context.

Review focus: src/core/task/observationRegistry.ts and its spec, plus the confineTo parameter in src/utils/safeWriteJson.ts.

Test Procedure

From the repository root, with dependencies installed:

pnpm --dir src exec vitest run --globals core/task/__tests__/observationRegistry.spec.ts # expect 11 passed
pnpm --dir src test -- utils/__tests__/safeWriteJson.test.ts services/mcp/__tests__/McpHub.spec.ts
pnpm --dir src exec tsc --noEmit

Manual check: point a project at a repo that ships .roo/mcp.json as a symlink to a file outside the workspace, edit a project MCP setting, and confirm the write is refused instead of replacing the outside file (the win32 symlink case is skipped in the unit test and verified this way).
Environment: Ubuntu 22.04 and Windows 11; unit layer only.

Post-commit durability tracking (032678d)

The Persistence Integrity row implemented at this head: safeWriteJson clears the staged path when safeWriteText reports PostCommitDurabilityError (the commit rename consumed it - it is the published target's former name), so the catch cleanup never unlinks a path this write no longer owns; the durability error rethrows and the target keeps its new content. Regression test: parent-directory fsync fails after the commit rename - target stays present with the new JSON, no unlink fires at the consumed staging path. Negative control: removing the guard reddens exactly that test. 114/119 with the file-safety suites (5 pre-existing skips). The Security Boundaries row remains the registered handle-relative family (owner-unit fix, precedent 6106959976).

CI/review round final head (6aed302)

Refreshed onto main 4f1dee18e (merge 2525fc218, no conflicts; lint-gate commit drops imports main's side orphaned and re-adds the safeWriteJson import the merged McpHub.spec mocks need). CodeRabbit's two new threads are fixed and resolved at 6aed30221: the injected fsync failure now targets a real directory descriptor with the wrapper's cause asserted, and the staging-release assertions compare the exact directory path. All six required checks green at 6aed30221.

Review round final head (41832ac)

The durability probe was reworked for portability (6953d6bcb): the injected fsync failure is triggered by watching the commit rename itself - the parent-directory fsync is the only fsync that runs after it - so the SUT opens and closes its own real directory handle and no descriptor is fabricated; the cause assertion is unchanged. 41832aca5 ports the resolveLockKey ENOENT-only fallback fix and the finally-block spy restoration from U2. All six required checks green.

Summary

Summary by CodeRabbit

  • Bug Fixes
    • Project-scoped MCP configuration changes are restricted to the workspace. Writes are rejected if no workspace location is available; global configuration writes remain unaffected.
    • JSON writes check that the destination remains within its configured scope and has not moved before publishing, reducing the risk of writing to an unintended location.
    • Improved handling of write failures, backups, cleanup, and durability errors helps preserve existing files and content that was successfully published. Writes through symbolic links resolve and use the destination consistently.

The pull request adds a task-local observation registry and updates text and JSON file publishing. JSON writes can enforce path confinement, resolve lock and publish paths, and validate target and ancestor identities. Project MCP writes pass a workspace root; global writes remain unconstrained.

Changes: File observation registry

Resolve private-file permission handling and workspace containment failures before merging. Interrupted symlink writes can also remain unrecoverable, and the reported mutation-size check still fails. Missing ancestors now correctly abort initial pin collection.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 5bfb8

The writer improves permission preservation and recovery reporting, but changes both where configuration updates land and what a rejected write means. Project symlinks can redirect updates outside the project, and a post-commit failure can leave saved MCP disable settings inconsistent with active connections. Exposure remains bounded by the extension host’s filesystem permissions; attacker control of deployment paths is not established.

Retained concerns

  • Medium · security · inferred: Project-scoped MCP updates now publish to an existing symlink’s referent without authorizing that resolved destination against the project scope. At the base, publication replaced the leaf symlink rather than modifying its referent. An actor able to control that link could redirect a user-triggered project update to another existing JSON file whose directory is writable by the extension host. Deployment attacker influence and an intended containment policy remain unproven, so this is a conditional scope-expansion concern.
  • Medium · security · inferred: A parent-directory fsync failure now rejects JSON publication after the new configuration is visible. When disabling a connected MCP server, that rejection skips the subsequent in-memory disable and disconnection. Configuration watchers suppress programmatic change/create events, and resetting the suppression flag does not itself reconcile configuration. Consequently, the saved disabled state can coexist with an active connection until another refresh, retry, or event. Actual persistence across a crash and watcher-event timing remain filesystem-dependent.

Security review details

Security Blast Radius

  • inferred — For the inspected project-MCP path, redirected publication can affect an existing JSON referent outside the workspace when its directory is writable by the extension host. No new operating-system identity or privilege grant is shown. The lifecycle concern affects live MCP connections managed by that host, not a demonstrated cross-tenant or cross-service boundary.

Security Findings and Attack Paths

  • inferred — The conditional redirection path is workspace-controlled leaf symlink, project configuration update, realpath resolution, and referent replacement. The separate revocation failure path is disable request, committed JSON rename, directory-fsync rejection, and skipped live disconnection. Neither path establishes a verified attacker exploit in the supplied deployment context.

Trust Boundaries and Controls

  • observed — Dangling links and non-ENOENT resolution failures are rejected. Supplied staging must be a regular non-symlink file beside the resolved target. These checks do not authorize the referent’s ownership scope. As counterevidence to claiming a new command-execution privilege, existing MCP initialization reads project configuration and enabled stdio configurations supply commands to a subprocess transport.

Resilience and Maintainability Implications

  • observed — Existing target mode preservation improves final-file permissions. JSON still streams into a caller-created temporary file before mode correction, so the new private self-staging directory does not protect this route. Staging validation also remains pathname-based across later open and rename operations. These residual conditions predate the delegation class of behavior; deployment exploitability is unresolved. Windows DACL preservation remains explicitly best-effort.

Hardening Proposals

  • proposed — Make resolved-target authorization a caller-owned policy: project-scoped updates could reject out-of-root referents or require explicit authorization for intentional external links. Reconcile live security state after committed-but-not-durable publication rather than treating every rejection as an uncommitted write.

Full details: Title check

Explanation

The title "feat(task): observation registry with read completeness (U3, #1375)" names the main change: a task-scoped observation registry that records file observations with a completeness flag. The changeset also adds confineTo support in safeWriteJson, safeWriteText hardening, and McpHub changes. The title does not cover those, which is expected for a single-line title. The title uses a conventional-commit prefix and an issue reference, and it stays under the usual length. It is specific enough for a teammate scanning history to understand the primary change.

Full details: Description check

Explanation

The description follows the repository template sections: Related GitHub Issue (Closes #2018), Description, Test Procedure, Pre-Submission Checklist, Visual Snapshots, Videos, Documentation Updates, Additional Notes, and Get in Touch. It explains what the PR changes, how it is tested, and why the diff looks larger than the change. It also lists review focus areas and a manual check for the symlink case. Weaknesses: the description is long and includes many process and chain details (budget counts, merge order, commit-level tracking sections) that reviewers may not need. Several sections are supplementary and do not change the assessment. The description also does not fully match the objectives: the objectives describe later ancestor-pinning, durability, and Knip work, and the mutation-diff gate reportedly failed at 589 lines against a 500-line limit. The description reports all checks green and does not mention that failure. The Test Procedure and checklist are mostly complete. The description is mostly complete, so the check passes, but the mismatch with the objectives should be reviewed.

Full details: Linked Issues check

Explanation

Direct issue #2018 is the only active linked target. Its coding requirements are met at the reviewed head.

[Registry, versioning, completion, timestamps, task scope] src/core/task/observationRegistry.ts defines FileObservation { version, observedAt, complete } and ObservationRegistry with observe, get, has, clear, and size. observe() stores Date.now() as observedAt, the supplied version token, and complete (default true). Task.ts adds a readonly observationRegistry owned by each Task instance, and dispose() clears it. The spec src/core/task/tests/observationRegistry.spec.ts covers versioning, timestamps, replacement, membership, clearing, instance independence, and completeness, including complete-to-partial downgrade. Task.spec.ts covers per-task isolation.

[Partial reads never complete] The registry records the complete flag that the caller passes. Partial observations are stored with complete = false. No production read call site exists in this unit, so no read path can mark a partial read complete. The registry does not enforce this contract itself. The contract is documented in the registry comment, and the issue does not require enforcement in this unit.

[Expiry deferred] No expiry logic is present. The issue defers expiry to consumer units, so its absence is compliant.

[Move retargeting deferred] No move retargeting is present. The issue defers it to the consuming unit.

[No production call-site wiring] The registry is not connected to read, move, or guarded-write call sites. The only wiring is Task ownership and disposal, which the issue does not prohibit.

Residual note: this PASS covers #2018 acceptance only. It does not cover the additional file-safety code in this PR, which is assessed under outOfScopeChangesAssessment.

Full details: Lifecycle Resource Cleanup

Explanation

No changed lifecycle path shows a concrete resource leak or duplicate work after disposal. Task.disposeOnce clears the new registry, and the new file-write code closes its file descriptors and releases its lock.

Full details: Out of Scope Changes check

Explanation

The reviewed diff contains file-write-safety work that #2018 does not request and that the PR description does not declare as a deliverable of this unit.

[Declared scope] The PR description lists three deliverables: the observation registry, the partial-read rule, and the project-scoped safeWriteJson confineTo port. It states that move retargeting and production call-site wiring are deferred. Issue #2018 acceptance covers only the registry, deferred move retargeting, and no production wiring.

[Demonstrated extra changes] Repository evidence:

  • At the merge base, src/services/file-safety contains no tracked files. At the reviewed head it contains src/services/file-safety/safeWriteText.ts and two spec files. The diff summary reports src/services/file-safety/safeWriteText.ts as +986 lines. It adds a staging, backup, DACL, ancestor-pinning, and post-commit durability publish primitive.
  • The commit messages state that the @internal publish primitive has its production consumer deferred to a later unit. The PR therefore adds code with no consumer in this unit.
  • src/utils/safeWriteJson.ts changes beyond confineTo: resolveLockKey locking, publish-target resolution under the lock, mode copying, post-commit directory fsync, and cleanup changes. The summary reports +319/-24 for this file.
  • The interruption test harness (src/utils/tests/safeWriteJson.interruption.test.ts) and the safeWriteJson lockKey and safeWriteText test suites cover these added behaviors.

[Connection to #2018] The issue and PR description do not require ancestor pinning, Windows DACL handling, staging-path validation, or post-commit durability policy. The issue text says only that the registry is the bookkeeping unit in epic #1375. The PR description places the Persistence Integrity and Security Boundaries rows under other owner units. Shared epic membership alone does not justify this code in this PR. The PR description also states a budget of 59 changed executable lines, while the current diff and the mutation-diff gate report about 589 changed executable lines. The diff is not one unit-sized change.

[Connected changes] The observation registry, its spec, the Task.ts hook, the Task.spec.ts additions, and the confineTo parameter with its safeWriteJson and McpHub tests are connected to #2018 or to the declared confineTo deliverable. These changes are not the cause of this finding.

Resolution

Reduce this PR to the #2018 deliverables. Remove src/services/file-safety/safeWriteText.ts and its two spec files. Revert the safeWriteJson changes that are not the confineTo parameter and its confinement checks, including resolveLockKey locking, publish-target resolution, mode copying, post-commit fsync handling, and related cleanup. Revert the matching interruption-test and safeWriteJson lockKey test changes. Keep the observation registry, its spec, the Task.ts hook and spec, and the confineTo parameter with its tests. Move the file-safety hardening to the owning unit PRs. Alternatively, rebase this PR onto main after U1 and U2 merge so the diff contains only U3 changes. Update the PR description so its budget and scope statements match the reduced diff.

Full details: Security Boundaries

Explanation

Project MCP writes can bypass the new confinement control and expose credentials. McpHub.ts:2138-2141 passes the workspace as confineTo, but safeWriteJson.ts:334-359 checks ancestor identities before opening the staged file by path. If a process replaces .roo with a symlink to an outside directory after that check, the staged MCP configuration is written outside the workspace. The prior verdict cited safeWriteText; the affected MCP path instead uses safeWriteJson directly. The race remains unresolved.

Resolution

Use a verified directory handle and no-follow, handle-relative operations for staging and publication. If those operations are unavailable, fail closed for confined writes. Add an integration test that swaps an ancestor after validation and verifies that no outside file is created or modified.

Full details: Persistence Integrity

Explanation

The changed symlink write path can leave committed state unavailable after interruption. safeWriteJson now places the EPERM fallback backup beside the resolved referent (lines 388–391), but recovery still scans the requested path (line 270). If the process exits after backing up a symlink referent, the next write misses that backup and rejects the dangling symlink in resolvePublishTarget (lines 317–330). The prior confinement concern also remains unresolved. safeWriteJson performs its last ancestor check before staging (lines 334–359), then uses path-based rename (line 375). If an ancestor is replaced by a symlink after the check but before staging, the write can replace an outside file. The prior verdict named safeWriteText as the publisher; this path actually publishes directly in safeWriteJson.

Resolution

Recover backups using the resolved referent, including when the requested symlink is dangling, under the same lock. Add an interruption test for a symlink target during EPERM fallback. Make confinement and publication atomic with a handle-relative no-follow operation, or refuse confined writes when that guarantee is unavailable. Add an ancestor-swap regression test.

Full details: Regression Evidence

Explanation

Two confinement cases still lack focused coverage. safeWriteJson.ts:128-130 rejects the scope root as a target, but the confinement tests use outside or child paths. safeWriteJson.ts:145-165 propagates ancestor stat failures, but the tests do not inject those failures during ancestor collection. The safeWriteText tests cover a separate revalidation path.

Resolution

Add safeWriteJson tests for a target equal to confineTo and for ancestor-collection stat failures, including non-ENOENT and ENOENT. Assert the expected error and that no merge, staging, or publication occurs. For the scope-root case, also assert that no lock is acquired.

…ve (U1, issue 1375)

Split unit U1 of PR 1833. Three changes, each with a test that fails without it:
- a caller-supplied staging path is checked for location and file type before anything is written, so an arbitrary path or a symlink cannot be published onto the target;
- a failed parent-directory fsync on POSIX is reported as PostCommitDurabilityError instead of being swallowed, so a successful return never claims durability the filesystem did not grant;
- the staged file and this write's own staging directory are released before RollbackFailureError is thrown.

Focused coverage for resolveLockKey added: canonical parent directory, a dangling-link chain, and termination at the bounded depth on a two-link cycle.

…ishTarget (U1, issue 1375)

The resolver may fall back to the given path only when lstat also reports the path as absent. An EACCES or EIO failure says nothing about whether the path is a link, so falling back would publish through a link we were not allowed to inspect. Focused tests added for both branches.

The three findings here are the same class as the ones on #1910 and are closed in the commit that owns safeWriteText.ts (c4120b057, which is an ancestor of this head):

  • Persistence integrity — the POSIX parent-directory fsync no longer swallows errors; a failure throws PostCommitDurabilityError, which names the target and states that the content is committed but the directory entry may not be durable.
  • Regression evidence — focused coverage added: realpath rejects with ENOENT and lstat rejects with EACCES, asserting the lstat error propagates instead of falling back to the link path; plus the ENOENT-on-both case that still falls back.
  • Lifecycle resource cleanup — the staged file and this write's own staging directory are released before RollbackFailureError is thrown, and the backup is preserved.

50 tests pass at this head; the new lstat test was verified to fail against the pre-fix file.

Re-requesting review needs a human — this token gets 404 on POST /pulls/1912/requested_reviewers for a fork PR, so the Reviews panel has to be used.

… type-sound

compile failed at the unit head on three points:
- RollbackFailureError needs a string backupPath, but the throw now happens after cleanup, so the
`string | null` narrowing was lost. The failure is now held as { error, backupPath }.
- The async lstat stand-in is built on the Stats prototype so it satisfies fsSync.Stats.
- The realpath/readlink mocks are typed to the real signatures; the readlink mock answers once
because only the link path is read.

tsc clean, 50 tests pass, ESLint --max-warnings=0 clean, no suppression change.

The any usage this entry covered is gone in the rewritten file, so the count drops 4 -> 3.
eslint --prune-suppressions --max-warnings=0 confirms it.
The read tools record the observed on-disk version through task.observationRegistry, but the field
was only declared in a later unit, so at this head the call dereferences undefined and the mocked
e2e run fails on the read_file smoke tests. The registry is introduced by this unit, so the field
belongs here.

tsc clean on this unit, 11 observationRegistry tests pass, ESLint --max-warnings=0 clean.

Review follow-ups: confineForMcpWrite treats an empty provider cwd/getWorkspacePath as no boundary and throws instead of returning undefined, so a project MCP write cannot proceed unbounded; added a regression test that the write never reaches safeWriteJson. The staging-directory cleanup test now asserts fs.rmdir on the exact directory recorded by fsSync.mkdirSync rather than a path prefix. Existing watcher-cleanup test supplies a workspace root for its project-config write. McpHub + safeWriteText specs 179 passed.
getWorkspacePath is typed to return string; the failing-case mock returns the empty string (still falsy for the fail-closed check) so check-types accepts it.

Minor · Data Integrity & Integration · Quick win

Fail the write when the publish target maps to a different lock key.

lockKey is computed at line 207, and the lock is taken on it at line 234. Under that lock, resolvedTargetPath is resolved again and then used for the merge read and the commit. If an in-scope symlink is retargeted between the two resolutions, the writer holds the lock for the old target while it merges into and commits to the new one. A writer that locks the new target directly holds a different lock, so both writers can read the same content and one update can overwrite the other.

Compare the lock key of the resolved target with the held key before the merge runs. The check fails the write instead of retrying.

Suggested fix
 		resolvedTargetPath = await resolvePublishTarget(absoluteFilePath)
+
+ // The held lock belongs to lockKey. Fail before the merge if the publish
+ // target now maps to a different lock key, so no write runs under another file's lock.
+		if ((await resolveLockKey(resolvedTargetPath)) !== lockKey) {
+ throw new Error(`Lock key ${lockKey} no longer matches publish target ${resolvedTargetPath}`)

202-202: Trivial · Maintainability & Code Quality · Quick win

Assert the staged source of the failed commit rename.

In the failed-commit test, the fs.rename mock fails the first call by position. The test never checks that call's arguments. If a regression renamed the target to a backup before the commit, that first call would fail with the same error. The target would keep its content, and no backup copy would exist, so every current assertion would still pass.

Add an assertion that the failed call's source is the staged .new_ temp file. Clear the rename mock before the failing call, so that mock.calls[0] refers to this test's first rename.

Suggested fix
 		// fs.rename is already vi.fn() — use vi.mocked to avoid double-wrapping via vi.spyOn

Add a two-pin test that replaces the non-final ancestor.

The new ancestor-pin check iterates every entry in expectedAncestorIdentities, but the authorized-path tests each pass a single entry. No inspected test replaces a non-final pinned directory. A regression that checks only the last pin would pass all of them. Add a case with two pins, replace the outer (non-final) directory, and expect AncestorReplacedError for that directory.

Outside diff Stage the JSON without following unverified ancestor components.

309-327: 🔒 Security & Privacy | 🟠 Major | 🏗️ Heavy lift

Stage the JSON without following unverified ancestor components.

Staging opens the file by path after the ancestor pins are recorded and before safeWriteText checks them. If a pinned ancestor is replaced with a link to a directory outside confineTo in that window, _streamDataToFile writes the full project MCP JSON to that outside location. The payload includes every configured server entry. safeWriteText then rejects the publish at Step 2b, and cleanup removes the staged file. The write still happens. Rejection does not prevent it.

The lexical-path case has the same cause. When the target does not exist, staging uses resolvedTargetPath, but the pins are built from the canonical parent. An in-scope link on the lexical path is not pinned, so replacing that link after the pin also redirects staging.

Checking the pins again before the open does not close this window. The open still resolves the path by name. The staged file must be created in a way that does not follow components that were not verified at open time. The check that runs at publish time does not cover this write.

Suggested fix

No code patch. The required correction is to change how the staged file is created, so the open cannot follow a replaced ancestor. A check that runs before the open does not meet this requirement.

…g the publish

_ancestorIdentities skipped a directory whose stat answered ENOENT, on the assumption that a create
into a directory the publish itself makes has no identity to pin. The publish creates the target's
parent before it takes the lock and pins the ancestors after the lock is held, so at pin time a
missing component was removed DURING the publish: the chain stays unverified, and a symlink placed at
that path before the staging write sends both the staged file and the commit rename outside confineTo.
Every stat failure now propagates (the original error is kept as the cause).

safeWriteJson.lockKey 5 + safeWriteText 87 + safeWriteText.integration 7 tests pass (the integration
suite creates files in directories that do not exist yet, so the create-into-new-directory path is
covered), tsc --noEmit 0 errors, eslint --max-warnings=0 exit 0.
Two paths conflicted: src/utils/safeWriteJson.ts and its test. Both sides had
rewritten the same module, so each was resolved by deciding which behaviour
survives rather than by picking a side wholesale.

Protocol side: upstream main. Our side had replaced this module's body with a
delegation to services/file-safety/safeWriteText, a module main does not have,
and the delegation carried no replacement protocol at all: no interrupted-write
recovery pass, no trace stage stream that persistenceTrace and taskMessages
consume, no createTempFilePath, no single-rename commit with its EPERM two-step
fallback, no rollback that keeps the backup so a later writer can recover.
Resolving to our side would have deleted behaviour main shipped and mains own
tests assert, so the protocol stays and our path-safety layer is grafted onto
it.

Grafted from our side: resolveLockKey supplies the advisory lock key,
resolvePublishTarget supplies the publish target under the same lock, and
confineTo with ConfinedPathEscapeError is checked before any filesystem side
effect, again on the lock key before the lock file is created, and again on the
resolved publish target inside the lock. Staging and all four rename sites -
commit, fallback backup, fallback commit, rollback - name the same resolved
string, so no half-graft can leave the staged file on one canonicalisation and
the commit on another. The ancestor pins are ours too: after the in-lock check
the write records the resolved target and the identity (dev, ino) of every
directory walked from the scope root down to the target's parent, and re-verifies
both immediately after the merge read and before anything is staged -
TargetMovedError when the name now resolves elsewhere, AncestorReplacedError
naming the directory that moved. Verification runs after the merge callback and
before the staged file is written, so a rejected write leaves nothing behind on
either side of the swap.

Mains recovery, trace, flush and fallback tests are kept verbatim. Our
confinement, publish-target, lock-key and ancestor-pin tests are kept alongside
them. Four of our variants were not ported, and the reason is that the mechanism
they describe is not the one this module ships:
  - "a failed commit keeps the previous content at the target and removes the
    backup copy" pinned a single-rename flow (renameCallCount === 1) and a
"safeWriteText.bak_" backup name; this protocol backs the target up with a
rename of its own, and mains
"should keep the committed file when the commit rename fails with a
non-EPERM error" already asserts the surviving behaviour against that
    mechanism.
  - the chain's re-titling of mains rename-failure tests ("... backup copy
taken", "the commit rename fails (filePath exists)") renames tests over a
different rename sequence; mains versions are kept verbatim.
  - the post-commit fsync variant keeps the regression it guards (published
content intact, consumed staging path never unlinked) but asserts the
contract this protocol has: main treats the post-commit parent-directory
fsync as best effort - report through console.warn and complete the write -
rather than wrapping it in PostCommitDurabilityError and rejecting. Making it
reject would change shipped behaviour, so the test moved, not the product.

The lock-key specs expected call order moved with the mechanism as well: the
delegation resolved twice more after the lock and ran two staging-path lstat
checks; this protocol resolves once under the lock and staging plus every rename
site name that same string, so one resolve and two lstat entries are gone rather
than reordered. The assertions that matter are untouched: the lock is taken on
the resolved referent, and the caller queues behind a peer instead of failing
before the lock.

No local Stryker preflight was run for this merge: the delta is a merge
resolution over code the mutation gate already covers plus test adaptation, and
no Stryker disable directive is added anywhere in it.
…d graft u3's additions onto it

The merge commit above (5da3231) resolved src/utils/safeWriteJson.ts and its test.
This commit resolves the third overlapping module, src/services/file-safety/safeWriteText.ts,
the way the seam ruling requires: my lane's copies are the base and u3's additions are
grafted onto them, because once Zoo-Code-Org#1911 lands main carries my copies and merge order decides it.
Both feature sets survive; nothing was dropped to make a conflict disappear.

The two files shared lineage exactly: resolvePublishTarget, canonicalDirKey, resolveLockKey,
_tempName, _stagingDir, _releaseStagingDir, _fsyncFile, _saveDaclWindows and
_restoreDaclWindows were byte-identical (compared by sha256 per region), so the graft is the
divergence only, decided hunk by hunk against the 6-hunk diff between the two copies:
  - took u3's side: the two new options (expectedResolvedPath, expectedAncestorIdentities)
with their doc comments, the exported AncestorIdentity type (dev, ino read with bigint
stats), the TargetMovedError and AncestorReplacedError classes, the entry check that the
path still resolves to the authorized target, and Step 2b - re-validating every recorded
ancestor identity, and the authorized name's realpath, immediately before the commit
rename, mapping only ENOENT to AncestorReplacedError and propagating any other error.
  - kept my side: the staging-directory identity matcher (a textual mismatch is resolved by
comparing dev and ino of the two directories, because path.resolve and node 22.23.4's
JavaScript fs.realpath both preserve an NTFS 8.3 spelling and only realpathSync.native
expands it, so neither can tell an alias of the target's own directory from a different
directory), with its two tests that drive that branch directly, and the mid-write retarget
guard that re-resolves the publish target immediately before the commit rename. u3's side
removed both: its staging check fell back to the plain textual rejection, and it dropped
the pre-rename re-resolution. Keeping mine is not a preference - the textual form rejects
a staging file that really is beside the target on the hosted Windows runner, whose temp
directory arrives as an 8.3 alias while the writer's resolved target arrives in the long
    spelling of the same directory.
Result: 973 lines, mine 859 plus u3's 114 added lines, with none of my 859 removed.

Test graft (src/services/file-safety/__tests__/safeWriteText.spec.ts, 2146 lines): my file as
base with u3's 257 added lines taken - the new imports, the _dirIdentity BigIntStats stand-in,
53 propagated-failure tests and the 193-line 'authorized-path pins' describe - while my
identity-branch, hard-link and unresolvable-identity tests are kept verbatim. u3's
safeWriteText.integration.spec.ts (177 lines) is u3-only content and stays. One scaffolding
change came with u3's graft: the fs.stat default in mockDefaults() flips from rejecting ENOENT
(my copy needed it so the identity check fell back to the textual rejection) to resolving
_dirIdentity(100n) (u3's ancestor pin needs a stable identity). My dependent tests still
assert rejection and still pass against that default; the claims are unchanged and the run
below is the evidence. src/utils/__tests__/safeWriteJson.lockKey.spec.ts stays at the merged
protocol's call order (see the merge commit message for why the entries are gone rather than
reordered); the assertions that matter - lock taken on the resolved referent, and queuing
behind a peer instead of failing before the lock - are untouched.

Product ruling recorded so the next unit does not re-litigate it: the Lead ruled on the
post-commit parent-directory fsync that best effort stays. After the commit rename the write
has succeeded; rejecting the call then tells a caller its write failed when the payload is
already at the target, and invites a retry that double-applies. Directory durability is
POSIX-only, is a logged warning, never fatal, and never a PostCommitDurabilityError out of
the JSON path. That is why safeWriteJson's _flushDirectoryToDisk returns early on win32 and
warns instead of rejecting, why the merged test asserts the durability warning in place of the
rejection while keeping the same injection point and the same regression assertions, and why
it matches what Zoo-Code-Org#1917 landed. safeWriteText itself still rejects via PostCommitDurabilityError
for a failure of the file fsync before the commit - that is a different point in the protocol
and its tests are kept.

No Stryker disable directive is added anywhere in this commit, and no local Stryker preflight
was run for it: the graft is content already mutation-covered on both sides, re-wired. Gate
numbers on this committed tree are reported to the Lead with the push decision.
… files

prettier --check flagged src/utils/__tests__/safeWriteJson.test.ts and
src/utils/__tests__/safeWriteJson.lockKey.spec.ts after the merge and graft commits.
The fix is prettier's own output only: 1 insertion and 10 deletions across the two files,
no test text, no assertion, and no ordering changed. The full gate is re-run on the
committed tree after this commit, including the misc shard that executes both files.

The knip findings seen in this worktree before the gate re-run (4 unused TaskStatus enum
members in packages/types/src/task.ts plus one vitest/globals config hint) were proven
environmental, not content: the same checkout of plain upstream main 73931fe run in a
second worktree with the same symlinked node_modules reports the identical two findings,
while knip in a worktree with a real install reports none. They come from workspace
resolution following the junction into another worktree's node_modules, so the knip number
for this PR is taken from a real install, reported to the Lead with the push decision.
…ity: the alias repro and the measurement fixes

This repairs main's own file - src/utils/__tests__/safeWriteJson.interruption.test.ts is not unit content.
It was not in this PR's delta against main 73931fe before this commit: the graft made the writer
publish at the path it resolves, which is where the divergence shows up, so repairing the injection is a
prerequisite for the graft rather than content the U3 unit owns. If a maintainer wants it standalone,
cherry-picking this one commit is cheap.

Reproduced the windows-latest condition locally with the recipe from the Lead's two red legs on Zoo-Code-Org#1917:
create a directory, take its NTFS 8.3 alias, point TEMP and TMP at the alias, and run the same commit on
the same pinned runtime (node 22.23.4). Alias used here: ZOO-CO~1\TMP-AL~2\LONG-D~1 for
C:\work\Zoo-Code-fork\.tmp-alias-vps2f\long-directory-name-under-alias; both spellings stat to one
inode 673102249:562949965815680, async fs.realpath returns the long form for it, plain realpathSync
returns the alias unchanged, and realpathSync.native returns the long form.

On that setting the misc shard went 12 failed | 2097 passed with every failure in
src/utils/__tests__/safeWriteJson.interruption.test.ts and every failure reading 'expected
'WRITE_COMPLETED' to contain 'EXIT_AT_REPLACEMENT_RENAME' - the child's fault injection never fired. With
TEMP pointed at the long spelling the same tree was 2109 passed | 13 skipped. Same commit, same runtime,
only the spelling of TEMP changed.

Cause, in the child script: it hooked fs/promises.rename and killed itself when
path.resolve(newPath) === targetPath. The parent authorized an alias-spelled path; the writer renames to
the publish target it resolved through async fs.realpath, which the alias expands, so the two strings
were different for one file and the kill point was never reached - an injection that silently stopped
injecting, the same failure mode as the deleteSemantics whole-path equality on Zoo-Code-Org#1917.

Fix, in the same shape as the Zoo-Code-Org#1917 fix and without weakening the claim: the predicate now asks what the
rename destination IS - same basename, and the same containing directory by dev+ino (stat bigint,
fail-closed to the rejection when either directory cannot be stat'd). The unique basename per storage
directory keeps it from firing on the backup rename, whose destination is a .bak_ name. The child still
performs the real write from the alias spelling, so the divergence stays under test rather than being
spelled away. Verified: 12 passed under the alias spelling and 12 passed under the long spelling.

Also corrected three comments that stated the measurement backwards. They claimed fs.realpath preserves
an NTFS 8.3 spelling and that only realpathSync.native expands it. What was measured on 22.23.4 against
a directory created for the purpose: the asynchronous fs.realpath - the call resolvePublishTarget and
canonicalDirKey both use - DOES expand the alias; path.resolve and the synchronous fs.realpathSync
preserve whatever spelling they are handed; only realpathSync.native expands in the synchronous family.
The identity comparison in the staging guard is unchanged - it is the comparison both sides agree on
whatever spelling each was handed - and the two identity tests' comment now says the stub hands each
side its own spelling instead of claiming that mirrors node.

No product code changed in this commit beyond the comments: safeWriteText.ts is comment-only,
safeWriteText.spec.ts is comment-only, and the change in safeWriteJson.interruption.test.ts is the
injection predicate and the child's own target variable. No suppression count moved (eslint
--prune-suppressions --max-warnings=0 exit 0 on the touched files, suppressions file restored), prettier
reports them unchanged, and the full gate on the committed tree is in the push report.
…mer yet, so tag it instead of blanking the module

The knip check on this PR has two steps. CI's 'Run knip checks' passes; 'Run production Knip checks'
failed on 1fb5fcc with one finding, and it reproduces locally on a real install:

    Unused exports (1)
    safeWriteText function src/services/file-safety/safeWriteText.ts:392:23

Why: in production mode Knip does not count test usage, and nothing in the shipped extension imports the
function. u3's head 9713c4a passed the same step because its src/utils/safeWriteJson.ts imported
safeWriteText at line 11 - the JSON writer delegated to the primitive. The seam ruling resolved the
safeWriteJson conflict pair to main's replacement protocol instead, so that import is gone and the
primitive's only remaining importers are its own unit tests. grep over src, packages and apps at this head
finds zero production importers outside the module: observationRegistry.ts and Task.ts perform no file
writes at all, and every McpHub write is JSON, which is safeWriteJson's job. So the export is real, the
consumer is later in the merge order (the guardedWrite migration), and the missing consumer is the price
of the seam ruling rather than a mistake in the graft.

What was measured before choosing the fix. knip 5.88.1's schema has no per-symbol ignore: ignoreExports
is not a key, and ignoreExportsUsedInFile is already true at the top of knip.json - it does not cover this
export because the function is not referenced inside its own file. The only config knob that would clear
the finding is ignore, and that blanks this whole 973-line module in both knip views, hiding any future
finding in it too. The alternative of shipping the export from the unit that adds its consumer would move
2,323 lines of the primitive's evidence out of this PR, which is the opposite of keeping both feature sets.

Chosen fix: tag the export @internal, which is what the repository already does 38 times at main - for
example src/api/providers/utils/abort-signal.ts:254, apps/cli/src/lib/task-history/index.ts:11 and
apps/cli/src/lib/storage/credentials.ts:16 - and which knip honours by default. The claim is true: the
symbol is not part of any shipped API surface, it is repo-internal, and no shipped entry reaches it yet.
stripInternal is set nowhere in this repository, so the tag does not remove the declaration from any
generated type surface. The tag is a comment: no executable line changed.

After it, on the committed tree with the pinned node 22.23.4 and a real install: knip --production exit 0
and knip --treat-config-hints-as-errors exit 0 (measured before this commit, logs S364-knip-production and
S364-knip-default), tsc 0 errors, eslint on the file with --max-warnings=0 --prune-suppressions exit 0 with
no count moved, prettier unchanged, and the services shard unchanged at 5 failed | 1705 passed | 1 skipped
(services/rules EPERM only). The full five-shard gate on the resulting head is in the push report.

If the maintainers would rather this PR not carry the tag, the alternatives are the knip.json ignore line
or holding the export until the guardedWrite migration lands; both are one decision away and neither
changes the graft.

Pushed 085fbf8 (fork fws/u3-observation-completeness) — repairs to the two things the last head got wrong

Commits on this branch now: 5da3231af merge main 73931fe into u3, ddcb15172 trio graft, 54bb628dd prettier, 22a4ba72b interruption injection + measurement corrections (tree identical to the previous 1fb5fcc47; this message carries the statement below too), 085fbf85e the Knip fix.

Whose file the interruption repair is

src/utils/__tests__/safeWriteJson.interruption.test.ts is main's file, not unit content. It was not in this PR's delta against main before 22a4ba72b: it only needed changing because the graft makes the writer publish at the path it resolves, which is where the divergence shows up. It is a prerequisite for the graft rather than content the U3 unit owns; if a maintainer wants it standalone, that one commit cherry-picks cleanly.

The windows condition, reproduced locally rather than reasoned about

Recipe from the two red legs on #1917: create a directory, take its NTFS 8.3 alias (C:\work\ZOO-CO~1\TMP-AL~2\LONG-D~1 for …\.tmp-alias-vps2f\long-directory-name-under-alias), point TEMP and TMP at it, run the same commit on the pinned runtime (node 22.23.4). Both spellings stat to one inode 673102249:562949965815680.

Before this fix the misc shard went 12 failed | 2097 passed, every failure inside that spec and every one reading expected 'WRITE_COMPLETED' to contain 'EXIT_AT_REPLACEMENT_RENAME': the child hooked fs/promises.rename and killed itself on path.resolve(newPath) === targetPath, but the parent authorized the alias spelling while the writer renames to the publish target it resolved through async fs.realpath, which expands the alias — an injection that silently stopped injecting, the same failure mode as the whole-path equality on #1917. Pointed at the long spelling the same tree was 2109 passed.

After the fix (predicate = same basename plus the same containing directory by dev+ino, stat bigint, fail-closed when either directory cannot be stat'd) the committed head runs 2109 passed | 13 skipped, exit 0 with TEMP on the alias spelling and 2109 | 13 skipped with TEMP on the long spelling. A probe under the same environment reports os.tmpdir() returning the alias spelling itself, so the alias run really ran under it. The hosted platform-unit-test (windows-latest) leg was read from CI at 1fb5fcc47: completed:success; ubuntu, compile, e2e-mock and check-translations likewise.

Three comments were corrected because they stated the measurement backwards: async fs.realpath expands the alias; path.resolve and sync fs.realpathSync preserve whatever spelling they are handed; only realpathSync.native expands in the sync family. The identity comparison in the staging guard is unchanged - it is the comparison both sides agree on whatever spelling each was handed - and the two identity tests' comments now say the stub hands each side its own spelling, which is the worst case a writer can be put in, rather than claiming that mirrors node.

The Knip failure, diagnosed and fixed

The .github step Run knip checks passes; Run production Knip checks failed at 1fb5fcc47 with exactly one finding, and it reproduces locally on a real install:

Unused exports (1)
safeWriteText function src/services/file-safety/safeWriteText.ts:392:23

u3's head 9713c4aefe83 passed the same step because its src/utils/safeWriteJson.ts:11 imported safeWriteText - the JSON writer delegated to the primitive. Resolving the safeWriteJson conflict pair to main's replacement protocol removes that import, and a grep over src, packages and apps at this head finds zero production importers outside the module itself: observationRegistry.ts and Task.ts perform no file writes at all, and every McpHub write is JSON, which is safeWriteJson's job. The export is deliberate; its consumer is later in the merge order, in the guardedWrite migration.

Measured before choosing a fix: knip 5.88.1 has no ignoreExports key, and ignoreExportsUsedInFile - already true at the top of knip.json - cannot cover this export because the function is not referenced inside its own file. The only config knob that clears it is ignore, which blanks this whole 973-line module in both Knip views and hides anything found there later. Shipping the export from the unit that adds its consumer would move 2,323 lines of the primitive's evidence out of this PR. Chosen: tag the export @internal, which is what this repository already does 38 times at main (src/api/providers/utils/abort-signal.ts:254, apps/cli/src/lib/task-history/index.ts:11, apps/cli/src/lib/storage/credentials.ts:16) and what knip honours by default; the claim is true, no shipped entry reaches the symbol yet. stripInternal is set nowhere in this repository, so the tag removes nothing from any generated type surface. It is a comment: no executable line changed. After it knip --production exits 0 and knip --treat-config-hints-as-errors exits 0.

For context on an earlier local reading: at plain main 73931fe8be25 in a junctioned-node_modules worktree knip reported 4 findings (TaskStatus.Running/Interactive/Resumable/Idle at packages/types/src/task.ts:101-104) plus a vitest/globals configuration hint and exited 1, while the same tree with a real install exits 0. My earlier local "knip red" was an artifact of junctioned dependencies; the plain-main comparison is what proved it. knip.json is byte-identical to main's in this PR.

Mutation gate

mutation-diff failed on scope, not on a mutant: Mutation gate failed: extension has 589 changed executable lines (limit 500). Split the PR or obtain a maintainer-reviewed narrow exclusion. The job also warns core Stryker preflight could not start: spawnSync /tmp/stryker-launch-…/node_modul… (the known Stryker 10 + vitest-runner runtime-activation defect) and carries one advisory survivor in packages/core/src/value.ts, which is not in this PR's delta. No local Stryker preflight was run for this push, and no Stryker disable directive is added anywhere on this branch.

Local gate on the committed tree (pinned node 22.23.4; committed blobs verified byte-equal to the worktree for all 13 delta files)

misc 118 files 2109 passed | 13 skipped · core 197 files 4247 passed | 9 skipped (the run exits 1 on an unhandled rejection - EnvironmentTeardownError: Closing rpc while "onUserConsoleLog" was pending from core/webview/__tests__/ClineProvider.spec.ts - while every test passed; reproduced twice at this head and CI's core legs are green) · api 114 files 4070 passed | 2 skipped · tree-sitter 64 files 337 passed | 12 skipped · services 5 failed | 1705 passed | 1 skipped, all five in services/rules/__tests__/rules.spec.ts on EPERM: operation not permitted, symlink (this box cannot create symlinks unprivileged). tsc 0 errors; eslint --max-warnings=0 --prune-suppressions exit 0 on the 13-file delta with no suppression count moved (its rewrite of src/eslint-suppressions.json is formatting only - the committed file is byte-identical to main's blob); prettier clean.

The services tally is a correction: an earlier report of 11 failures counted artifacts of junctioned node_modules. After a real install the 4 MarketplaceManager.spec.ts TelemetryService not initialized failures, the 2 parser.spec.ts failures and the one-off ShadowCheckpointService.spec.ts ENOTEMPTY … rmdir CheckpointService teardown race all disappeared.

Stack note

#1405's head aa44471e74 carries rows against src/utils/safeWriteJson.ts (its rows 3 and 7). This branch now carries main's replacement protocol plus our confinement layer in that same file. Flagging the overlap so #1405 and #1913 do not resolve safeWriteJson.ts against different blobs: whoever lands first should expect the other to need one re-run of that file, not a re-decision.

Addendum on one number above: the inode pair quoted for the alias directory (673102249:562949965815680) was measured on the directory created earlier in the work; the alias run on this head used a recreated directory at the same path whose inode pair is 673102249:17451448568457636, identical for both spellings there too. The claim - one inode, two spellings - is what carries; the number is a property of the directory instance.

Follow-up from the Lead's rulings at this head

  • Knip: the @internal commit is the mechanism and no knip.json ignore line is being added alongside it - one mechanism only. The ignore alternative would blind both Knip views to a 973-line module to clear one export, and holding the export out of this PR would move 2,323 lines of the primitive's evidence out of the unit that introduces it, against the seam ruling. The disclosure above stands as written: CI's failing step is the --production one, and my earlier exit 0 was the non-production invocation.
  • Mutation: advisory, cap-class. Verbatim: Mutation gate failed: extension has 589 changed executable lines (limit 500). Split the PR or obtain a maintainer-reviewed narrow exclusion. 085fbf85e is a comment, so 589 does not move with it. The core Stryker preflight could not start: spawnSync /tmp/stryker-launch-… warning is the Stryker 10 + vitest-runner runtime-activation defect, not a mutation result; the packages/core/src/value.ts:1 survivor is read as an artifact of that failed preflight - unverified, deliberately not chased. Recorded as planned-and-issued on the chain tracking issue [Tracking] guarded-write chain (epic 1375): rules, port sources, chain-level rows #1991 (comment 6716641767 is the Advisory lock key must be identity-backed, not realpath text: an NTFS 8.3 alias and its long form take different locks #2356 identity clause; the [Tracking] guarded-write chain (epic 1375): rules, port sources, chain-level rows #1991 record carries the budget).
  • Core teardown, stated as my measurement: at this head the core shard exits 1 on EnvironmentTeardownError: [vitest-worker]: Closing rpc while "onUserConsoleLog" was pending, originating in core/webview/__tests__/ClineProvider.spec.ts, with all 4247 tests passing; reproduced on two consecutive runs. I have not established whether CI's core legs absorb it. What I read was green at the previous head; at this head the legs were still running when I last read them, and if they come back red this teardown error is the first thing I will read, not the last.

Inline comments:
Review comments at @src/services/file-safety/safeWriteText.ts:
- Line 808: Update the publication flow around fs.rename so the staged file
cannot be replaced or redirected between validation and commit; establish a
mechanism that protects the staged file through the rename, while preserving the
existing validation and fsync guarantees.
- Around line 383-389: Update resolveLockKey so exhausting the eight-link
traversal rejects instead of returning the canonicalized intermediate key; keep
returning the canonical key when readlink confirms the chain has reached its
terminal referent.

Review comments at @src/utils/__tests__/safeWriteJson.test.ts:
- Line 1260: Update the safeWriteJson test setup so opening the directory
succeeds deterministically, then assert that the warning contains fsyncFailure
rather than merely matching fsync. This ensures the test verifies the injected
fsync failure path.
- Around line 1143-1145: Update the safeWriteJson regression coverage to swap
the ancestor at the stream boundary rather than in merge, and assert that the
write is rejected without publishing outside the intended location. Ensure
safeWriteJson validates the ancestor at commit time so the boundary swap is
detected.

Review comments at @src/utils/safeWriteJson.ts:
- Around line 290-292: Retain the initially resolved confinement root in the
safe-write flow and reuse it for checks after waiting for the lock. Before
recovery or publication, reject any change in the scope directory’s resolved
location; do not let a moved scope establish a new confinement boundary.
- Around line 339-340: Update the pinned-directory revalidation in the
safe-write flow to also verify that each directory’s current resolved path
remains within the intended confined path before staging. Keep the existing
device and inode identity checks, and abort if either the resolved path or
identity no longer matches.
- Around line 339-340: Update ancestor revalidation in the safe-write flow so
`EACCES` and `EIO` from `fs.stat` propagate to callers instead of being
converted into `AncestorReplacedError`. Keep missing-path failures such as
`ENOENT` and `ENOTDIR`, along with directory identity mismatches, closed as
before; reuse the existing error-code handling rather than changing unrelated
behavior.
- Around line 372-375: Update the safeWriteJson flow so staging and publication
remain within confineTo even if an ancestor is replaced after the pre-staging
check. Revalidate the authorized directory relationship at the points where the
temporary file is staged and where actualTempNewFilePath is renamed to
resolvedTargetPath, and use a path-binding approach that prevents either
operation from being redirected outside the authorized directory.
- Around line 525-529: Update safeWriteJson so the staged file is created with
restrictive permissions, and abort the write if reading an existing target’s
mode with stat or applying it to stagedPath with chmod fails. Do not continue to
replace the target after either mode operation fails.
- Around line 335-336: Update the publish-target check using
resolvePublishTarget to retain the newly resolved path when it differs from
expectedResolvedPath. Pass the authorized expected path and the current resolved
path to TargetMovedError so the error reports both accurately.
- Around line 290-294: Update the recovery flow guarded by _resolveScopeRoot and
_escapesScope so it cannot delete replacement files or restore a backup through
an ancestor swapped after lock acquisition. Establish and enforce confinement at
the point recovery mutates files, using a mechanism that prevents ancestor
changes from redirecting those operations; a pre-write confinement check alone
is insufficient.
- Around line 388-391: Update the safeWriteJson recovery flow so interrupted
EPERM-fallback backups are found through the referent’s directory before strict
target resolution, including writes through differently named symlinks. Preserve
the existing confinement checks when locating and recovering referent artifacts;
keep the replacement-rename behavior in the EPERM fallback unchanged.

---

Failed pre-merge checks:
- Out of Scope Changes check:
Reduce this PR to the #2018 deliverables. Remove
src/services/file-safety/safeWriteText.ts and its two spec files. Revert the
safeWriteJson changes that are not the confineTo parameter and its confinement
checks, including resolveLockKey locking, publish-target resolution, mode
copying, post-commit fsync handling, and related cleanup. Revert the matching
interruption-test and safeWriteJson lockKey test changes. Keep the observation
registry, its spec, the Task.ts hook and spec, and the confineTo parameter with
its tests. Move the file-safety hardening to the owning unit PRs. Alternatively,
rebase this PR onto main after U1 and U2 merge so the diff contains only U3
changes. Update the PR description so its budget and scope statements match the
reduced diff.

- Regression Evidence:
Add `safeWriteJson` tests for a target equal to `confineTo` and for
ancestor-collection `stat` failures, including non-ENOENT and ENOENT. Assert the
expected error and that no merge, staging, or publication occurs. For the
scope-root case, also assert that no lock is acquired.

- Security Boundaries:
Use a verified directory handle and no-follow, handle-relative operations for
staging and publication. If those operations are unavailable, fail closed for
confined writes. Add an integration test that swaps an ancestor after validation
and verifies that no outside file is created or modified.

- Persistence Integrity:
Recover backups using the resolved referent, including when the requested
symlink is dangling, under the same lock. Add an interruption test for a symlink
target during EPERM fallback. Make confinement and publication atomic with a
handle-relative no-follow operation, or refuse confined writes when that
guarantee is unavailable. Add an ancestor-swap regression test.

Learning: In Zoo-Code-Org/Zoo-Code's file-safety split tracked in easonLiangWorldedtech/Zoo-Code#41, U3 (#1912) owns the TypeScript in-memory ObservationRegistry primitive in src/core/task/observationRegistry.ts and its completeness contract. Production read recording belongs to U4 (#1913), move completeness to U6 (#1915), guard core to U5 (#1914), remaining write-tool integration to U7 (#1918), and interactive-save integration to U8 (#1916). Reject unresolved lock keys.

If a chain has more than eight links and its referent is absent during an EPERM fallback, resolveLockKey returns an intermediate link. Once the referent returns, concurrent merges can publish under different locks and lose updates. Detect an unfinished traversal and reject it.

Protect the staged file through publication.

If another process can modify the staging directory, it can replace tempPath after the descriptor is fsynced and closed. The rename can then publish different content or a symlink without validating or flushing it. Protect the staged file through the commit.

Update the publication flow around fs.rename so the staged file cannot be

Test an ancestor swap when staging starts.

The current fixtures swap the ancestor in merge, before the last pre-staging check. Add a swap at the stream boundary and assert rejection and no outside publication. safeWriteJson currently has no commit-time validation, so these tests miss that failure.

As per path instructions, “Require regression coverage at the lowest valid harness with behavior-focused assertions.”

Verify the injected fsync failure.

If opening the directory fails, the warning still contains fsync, so this test can pass without invoking the fsync spy. Make the directory open deterministic and assert that the warning contains fsyncFailure.

Keep the initially resolved confinement root.

If the scope directory moves while the writer waits for the lock, the later checks accept its new location and can publish JSON outside the original scope. Retain the initial root and reject scope changes before recovery or publication.

Report the current resolved target.

If the target changes after authorization, this error reports the original target as the current destination. It can also report the requested path instead of the authorized target. Preserve the new resolution and pass both paths to TargetMovedError.

Revalidate each pinned directory’s resolved path.

If an absent target’s ancestor moves outside the scope and its old name becomes a symlink to it, the inode check still passes. The write can then publish outside confineTo. Compare each pinned directory’s resolved path as well as its identity before staging.

Preserve transient ancestor stat errors.

If ancestor revalidation fails with EACCES or EIO, this code reports AncestorReplacedError and loses the original error code. Propagate those errors so callers can diagnose the failed write. Keep missing-path and identity-mismatch failures closed.

Update ancestor revalidation in the safe-write flow so `EACCES` and `EIO` from
`fs.stat` propagate to callers instead of being converted into
`AncestorReplacedError`. Keep missing-path failures such as `ENOENT` and
`ENOTDIR`, along with directory identity mismatches, closed as before; reuse the
existing error-code handling rather than changing unrelated behavior.

Prevent confined writes through replaced ancestors.

If an ancestor is replaced after the pre-staging check, staging and rename can write JSON outside confineTo. Project MCP writes can therefore expose credentials or overwrite an outside mcp.json. Keep both staging and publication bound to the authorized directory.

Update the safeWriteJson flow so staging and publication remain within confineTo

Recover backups from the referent’s directory.

If an EPERM fallback is interrupted after the backup rename, a later write through a differently named symlink scans the alias namespace and misses the backup. The dangling alias then causes the write to fail. Recover referent artifacts before strict target resolution, while preserving confinement checks.

Keep staged JSON private and fail closed on mode errors.

If stat or chmod fails, the write can succeed with a 0o644 replacement for a 0o600 target. Other users can then read the content. Create the staged file with restrictive permissions, and abort if an existing target’s mode cannot be read or applied.

Carried here at the Lead's request; measured on the vps2-f #1913 lane, pinned node v22.23.4 (box node 25.9.0).

mutation-diff on #1913 went failure -> success by merging main, and back to failure on the next commit:

  • e62f198ce, before merging main: failure, check-run 125486348227, workflow run 41506292261. Gate
line as recorded there: "Mutation gate failed: extension has 589 changed executable lines (limit
500). Split the PR or obtain a maintainer-reviewed narrow exclusion."
  • e3d632799, four commits after merging main 73931fe8: success, check-run 128619142516, workflow
    run 42444051212.
  • 8fa7d858c, ~14 new executable lines later: failure, check-run 128620716380, workflow run 42444581569.

The mechanism is visible in the PR's own delta: +5188/-161 against the old base, +5153/-39 against
73931fe8. Merging main shrinks the changed-executable-line count, so on a cap-class red it is the
cheapest lever available on this series - and the gain is not durable, as the third row above shows.

Two things I am not claiming. The failure class at 8fa7d858c is not established: that job's log 403s
for this token and its annotations carry only "Process completed with exit code 1", so line cap again
and surviving mutants in an unexercised error branch are both consistent with what I can read. And a
lane's own gate line is the only thing that tells you which class its red is - read it before
assuming the lever applies.
