feat(tools): publish apply_patch through the guard (U6, #1375) - #1915
This PR routes the apply_patch tool through the guard, so a patch hunk can no longer "write" a file the model never fully read: an unobserved overwrite and a stale version token are refused with the standard remediation. A move carries the source's completeness to its destination instead of claiming completeness for lines the model never read, and a partial-source move onto an already-observed destination is rejected before any state changes. It also fixes a resource leak: when the guard refuses a publish, saveDirectly now rolls back the parent directories it created.
It is the sixth of nine PRs for epic #1375 (chain tracked on #1991).
apply_patch publishes via guardedWrite, so an unobserved overwrite and a stale version token are rejected with the read-first / re-read-then-retry remediation instead of clobbering the file.
A move re-targets the source's observation onto the destination: the destination inherits the source's complete flag, so a slice, range, truncated, or indentation-block source never authorizes a full-file replacement at the new path.
A partial-source move onto a destination that was already observed completely is rejected before any file or registry state is touched.
saveDirectly captures the list createDirectoriesForFile returns and, if the guard rejects, removes those directories innermost-first with rmdir (which refuses a directory another writer populated, so the loop stops at the first failure) and rethrows the original write error.
Ancestor-identity pinning at publish: the confined write records the identities of the directories it created and re-checks them before the commit rename, refusing with AncestorReplacedError if an ancestor was swapped underneath it.
Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on main and carries its own copy of the earlier units' content, which the GitHub diff shows as context.
Review focus:src/core/tools/ApplyPatchTool.ts, the guard wiring in src/core/tools/guardedWrite.ts, the rollback path in src/integrations/editor/DiffViewProvider.ts, and the specs named below.
Test Procedure
From the repository root, with dependencies installed:
Environment: Node 22+, Linux/macOS/Windows CI runners (the guard's platform-specific branches are exercised through the injected platform option, not a real Windows host).
Pre-Submission Checklist
Scope is one gate scope and matches the unit.
No .changeset or CHANGELOG changes (AGENTS.md).
src/eslint-suppressions.json unchanged - no suppression count increased.
New code lints clean (--max-warnings=0) rather than relying on suppressions.
Tests added at the lowest layer that would have caught each finding.
Visual Snapshot item considered: not applicable (no rendered surface).
Visual Snapshots
Not applicable - no UI change.
Videos (interaction / animation only)
Not applicable.
Documentation Updates
None. No user-facing setting, command, or documented behavior string changes; the guard's remediation text is already introduced in earlier units. No .changeset and no CHANGELOG edit (AGENTS.md).
mutation-diff is red on the changed-line budget only: the gate counts the GitHub diff, which carries the unlanded predecessors (1205 changed executable lines against the 500 cap; this unit's own delta is 105). The row clears when the predecessors land and this branch is rebased.
The directory-rollback test is a real pin: with the DiffViewProvider.ts change stashed it fails.
Local lanes at this head: integrations + core/tools + activate - 1340 passed / 17 skipped across 60 files; tsc --noEmit clean; eslint clean on the changed files with no suppression-count increase.
CI at this head: all required checks green; mutation-diff red on the budget-cap count described above (no surviving mutants in this unit's own lines).
Reviewer and maintainer questions are welcome as comments on this PR or on #1991.
Stryker directive removal (equivalence notes)
Per AGENTS.md the two Stryker suppression comments in the guarded save
path were removed instead of kept; their equivalence claims:
writeKind default StringLiteral ("update" -> ""): an empty kind
dispatches exactly like "update" inside guardedWrite - only "edit"
and "create" branch distinctly - and the default-parameter line is
not part of this unit's changed-line selector, so the mutant is
equivalent / out of scope.
saveTask operand of the adoption-probe guard (LogicalOperator): GuardRejectedError is only thrown by guardedWrite, which is reached
only after the !saveTask check above has already thrown. Whenever
the operand is evaluated saveTask is provably non-null, so dropping
the operand cannot change which branch is taken.
mutation-diff remains advisory for stacked units; maintainers decide any
narrow gate-config exclusion.
Committed gh device ID removed (.local/state/gh/device-id, gitignored,
swept in by an add -A during a refresh). No other .local/ strays on any
unit branch or main.
Changed-block as any cleanup in DiffViewProvider.spec.ts: bracket
access for private members + documented unknown-based structural casts for
VS Code doubles; suppression 310 -> 285.
Cleanup-safety-net test asserts content, not just count: the single console.error must carry [Original Error Caught] with the commit-failure
object as its second argument, and no call may carry the cleanup-error text.
Re-review responses (error-fix re-review wave)
ApplyPatchTool move, durability-error branch (Minor Functional): first
answered by completing the source unlink and reporting unlink failures as PartialMoveError - since SUPERSEDED by the crash-availability reversal
below; see the next section for the shipped semantics.
A follow-up review (Major Data-Integrity) reversed the previous round's
guidance for the same branch, and the new semantics shipped: when the
destination's commit rename landed but its directory entry is not known to
be durable (PostCommitDurabilityError), the source is KEPT and the
durability error propagates - unlinking it could leave NEITHER path
available after a crash. The duplicate is visible to the model and
recoverable; a file lost to the crash is neither, so crash availability
outranks not leaving the content at both paths. The try/catch is gone (the
error propagates naturally) and the durability test now asserts the source
unlink was NOT called; the unlink-failure test from the superseded round was
removed with its unreachable scenario. Negative control: restoring the
unlink reddens exactly the kept-source test.
Move deletion bound to the recorded observation (440dca4)
The Persistence Integrity row demanded the source deletion prove the
source is still the file the observation authorized. The move now
deletes under the shared advisory lock (withFileLock on the canonical
lock key): the source must still be present in the observation registry
and its recorded version must equal a fresh bigint stat of the source,
otherwise the move raises PartialMoveError before any unlink. A stat
failure is likewise a PartialMoveError - an unreadable source is not a
deletable source. Tests: the never-observed case now asserts no unlink
and no destination tracking; three new tests pin the changed-source
rejection, the vanished-source rejection, and that the deletion runs
under the source's lock key. 26/26 green; negative controls (faithful
mutants of each check) each redden exactly one test.
Durability note: on the durability-error path the source is kept
outright - PostCommitDurabilityError propagates before the deletion is
reached - the strongest form of the protected-deletion contract this
unit's issue asks for.
Security Boundaries disposition (warn-only DACL). The row asks for
the DaCl restore failure to fail closed. That change belongs to the
publish-primitive owner (PR 1916, fixed there in 3624e4f), which
merges before this unit in the declared order; this unit carries the
same carrier copy and reconciles at rebase (record 6106959976 on issue
1408; tracking 1991). Implementing it twice here would fork the fix.
Walkthrough carry-over evidence (04:34 walkthrough vs head 440dca4)
The 04:34 walkthrough still carries the Persistence Integrity and
Security Boundaries ERROR rows whose claims predate this head (pushed
2026-10-16T03:55Z). The PI row's raw-unlink line numbers belong to the
superseded head 993fc6f; at this head the deletion is lock-serialized
and observation-bound exactly as the Resolution demands (missing,
changed, vanished, or unobserved sources are kept; PartialMoveError
names the committed destination and remaining source; the durability
path keeps the source outright; tests and negative controls above). The
SB row re-flags the carrier copy's warn-only DACL restore, whose
fail-closed fix lands at the publish-primitive owner (PR 1916, 3624e4fea) that merges before this unit; copies reconcile at rebase
(record 6106959976 on issue 1408). Evidence comment posted on the pull
request; no code change this round.
File writes now check that files have not changed since they were read, with safeguards for partial reads and concurrent edits.
File publishing uses staged writes, supports path confinement, and checks symbolic links and target changes.
Editor saves use the same file-change checks, and previews can be opened without writing them to disk.
Bug Fixes
Read results distinguish clipped lines from omitted lines.
Failed writes preserve their original errors and clean up temporary files where possible.
Failed moves are reported instead of being treated as successful.
Tasks now track file-version observations and read completeness. Patch, diff, and editor writes use guarded publication. Text and JSON writes use staged publication with target checks, locking, and cleanup.

Do not merge yet. Session startup and cancellation races can empty files or leave cancelled edits published, while guarded writes can act on a different target than the one authorized. Valid targeted edits can also be rejected, move completeness can be overstated, and staged replacements can change access permissions.
Security Architecture Review
Security architecture risk:🟡 Moderate · up to 5a0dd
The change improves protection against stale and partial-file overwrites. However, replacing an existing file can weaken Windows access restrictions when permission restoration fails. Some move operations also retain their existing non-atomic behavior.
Retained concerns
Medium · security · inferred: Existing-file direct tool writes now replace the file instead of updating it in place. On Windows, publication precedes DACL restoration, and failures saving or restoring the original DACL are swallowed. Where the replacement has broader permissions than the original, an approved content edit can therefore expose the file to additional readers or writers, temporarily or persistently. Successful restoration mitigates the persistent case but does not make access-control preservation a publication precondition.
Security review details
Security Blast Radius
inferred — The inspected exposure is host filesystem content published with the extension process's existing authority. The Windows concern affects individual rewritten files whose original DACL is more restrictive than the replacement's permissions; additional principals may gain read or write access without receiving elevated process privileges.
Security Findings and Attack Paths
inferred — A legitimate approved write to a Windows file with a restrictive explicit DACL reaches replacement publication. If saving or restoring that DACL fails, the write still succeeds. A principal permitted by the replacement's broader permissions can then read or modify content previously restricted by the original DACL. The failure behavior is demonstrated by mocked tests; deployment-specific permission widening was not reproduced.
Trust Boundaries and Controls
observed — Observation authority is task-scoped and separates content completeness from filesystem version identity. The guard checks cancellation after queueing and under the lock, rejects stale versions, and retains partial completeness after targeted edits.
observed — Publication deliberately follows existing symlink referents and rejects dangling links. Move containment is lexical, while ignore matching resolves referents but allows outside-directory paths or errors. Tool writes already followed symlinks at the base, so this is not established as a new tool escape. JSON writes now follow referents instead of replacing links; production-path attacker control remains unresolved.
Resilience and Maintainability Implications
observed — Rejected editor saves use discard-only recovery rather than writing the original preview back over newer disk content. Placeholder removal checks its captured version under the shared lock, and overlapping teardown operations are serialized. Successful publication clears an unchanged dirty buffer by reloading from disk rather than performing another unguarded save.
observed — Move destination publication and source deletion remain separate operations. Source deletion has no version check and its failure is logged rather than propagated; the non-focus move branch still uses raw destination writes. Comparison with the base confirms these lifecycle limitations predate the PR, so they are not retained as newly introduced concerns.
Hardening Proposals
proposed — Make preservation of an existing Windows DACL a publication precondition: prepare and verify equivalent restrictions on the replacement before making it visible, and reject the write when preservation cannot be established.
proposed — Document the resolved-target authorization policy for tool and persistence writes, then test outside-workspace referents and referent changes. Treat source-version-protected move cleanup and consistent guarding across execution modes as follow-up work for the existing lifecycle gaps.

Full details: Title check
Explanation
The title "feat(tools): publish apply_patch through the guard (U6, #1375)" matches the main change: ApplyPatchTool now publishes through guardedWrite, with move observation handling and PartialMoveError. It is specific and conveys the primary change. The "(U6, #1375)" suffix is a unit and epic reference that adds noise but does not make the title unrelated.
Full details: Description check
Explanation
The description follows the template structure: it has a Related GitHub Issue section with a Closes link to #2021, a Description section, a Test Procedure section with commands, and a Pre-Submission Checklist. It also covers the changes in detail. Gaps remain. The Get in Touch section is empty, and the Documentation Updates section is filled in but not as a checkbox choice. The description is long and includes chain and process notes that do not belong in the template. Its own Test Procedure names a spec file, applyPatchTool.guardedWrite.spec.ts, which is not in the changed file list. The raw summary lists applyPatchTool.execute.spec.ts instead. The description also says the source-deletion ordering was reversed in later rounds, and the Description item 2 does not match the shipped move semantics described later. Since the template is largely filled out and the core intent is clear, the check passes, but the author should fix the spec path and tidy the stale or conflicting notes.
Full details: Out of Scope Changes check
Explanation
The reviewed changes support the guarded apply_patch objective in #2021. These changes include the observation registry, the ReadFileTool and indentation-reader completeness signals, the guardedWrite primitive, DiffViewProvider wiring, ApplyPatchTool move handling, and the safeWriteText and safeWriteJson publish safety. The PR description states that the branch carries predecessor units (U1 to U5, U8) from the non-cumulative chain. The predecessor work supplies the guard, observation, and publish primitives that #2021 uses. The safeWriteJson confineTo change is predecessor content that does not call apply_patch directly. Its connection to #2021 is weaker, but no concrete unrelated change is shown. No files were excluded from review.
Full details: Linked Issues check
Explanation
Issue #2021 requires that apply_patch publish through the guard, and that a move carry source completeness and refuse a partial-source move onto an observed destination before state changes. The head code meets these requirements only when PREVENT_FOCUS_DISRUPTION is enabled.
Met at head (src/core/tools/ApplyPatchTool.ts):
Add hunks call saveDirectly or saveChanges with "create".
Same-file update hunks call saveDirectly or saveChanges with "edit".
The guarded move branch (isPreventFocusDisruptionEnabled true) reads the source observation, rejects a partial source onto an observed existing destination with GuardRejectedError before publish, and passes sourceComplete to saveDirectly.
Source deletion runs under withFileLock with an observation-version check. Failures raise PartialMoveError.
A PostCommitDurabilityError from the guarded publish propagates without unlinking the source.
Not met at head: the move branch for the experiment-off path (the else branch of the move publish) calls fs.writeFile(moveAbsolutePath, newContent, "utf8") directly. This bypasses guardedWrite. It also skips the partial-source-onto-observed-destination rejection, so a partial-source move can overwrite an observed destination. The PR objectives state that this path is a follow-up. The issue acceptance criteria are not qualified by experiment state, so the requirement is unmet for that path.
The saveDirectly rmdir rollback and the guardedWrite wiring were not re-read in full at this head. The read was truncated at 16000 bytes. Those points rest on the change summary and the PR description, and they do not change this verdict.
Resolution
Route the experiment-off move publish in ApplyPatchTool.ts through the guard. Replace the direct fs.writeFile call with a guarded publish (saveChanges with "create", or an equivalent guardedWrite call). Carry the source completeness to the destination first, and apply the same partial-source rejection that the experiment-on branch applies. Add a test that covers a partial-source move onto an observed destination with the experiment off. If the issue owner decides that the experiment-off move path is out of scope for #2021, update #2021 to say so. Then this check can pass for the guarded branch alone.
Full details: Security Boundaries
Explanation
The Windows DACL issue remains unresolved. src/services/file-safety/safeWriteText.ts:719-764 replaces the target before restoring its DACL, and _restoreDaclWindows silently ignores restoration errors at lines 283-295. When a secret file has a restrictive explicit DACL and its parent grants broader access, a failed restore can leave the published content accessible to other users. guardedWrite.ts:474-480 uses this path for edits. The added test at safeWriteText.spec.ts:706-728 accepts a successful write after restore failure.
Resolution
Preserve the target DACL on the staged file before publication, and fail closed if DACL capture or application fails. Ensure staging and backup copies do not expose the content. Surface any post-commit restoration failure and add regression tests for both DACL capture and restoration failures.
Full details: Persistence Integrity
Explanation
Three changed persistence paths remain unsafe: - safeWriteText.ts:439-440,809-840 creates missing parent directories but does not remove them after a pre-commit failure. The prior failure remains unresolved. At this head, fs.mkdir creates the parents before staging; _missingDirectoryTail is absent. - guardedWrite.ts:371-383 checks for an absent target, then publishes without failIfExist. If another process creates the target between the check and the rename in safeWriteText.ts:693-720, the rename overwrites its file. - ApplyPatchTool.ts:522-529 passes sourceComplete as reobserveBasis, not completeOverride (DiffViewProvider.ts:2346-2359). A move from a partially observed source to an unobserved destination records the destination as complete (guardedWrite.ts:685,781-786). A later full replacement can then discard content the model never read.
Resolution
Track and remove newly created parent directories on pre-commit failure, innermost-first, and add a real-filesystem regression test. Use atomic no-replace publication for createIfAbsent and handle an existing-target error. Pass sourceComplete as completeOverride and test a partial-source move to an unobserved destination followed by a full replacement.
Full details: Regression Evidence
Explanation
The prior coverage gap remains. Task.ts:302 adds a per-task ObservationRegistry, but Task.spec.ts has no registry assertions. The registry tests construct registries directly, and the guard tests supply mock tasks. Neither verifies that real tasks receive distinct registries.
Resolution
Add focused tests in src/core/task/__tests__/Task.spec.ts that verify a real task exposes an ObservationRegistry and two real tasks receive distinct registries. Keep the standalone registry tests.
Full details: Lifecycle Resource Cleanup
Explanation
The prior teardown race remains on the RollbackConflictError path. runTeardown() clears its claim when cleanup throws (DiffViewProvider.ts:1698-1707), then revertChanges() restores preview tabs outside the claim (:1569-1575). If a rollback conflicts and preview restoration blocks, a second cancellation or task disposal can start another rollback and restore the same tabs again. Concurrent callers already waiting on the rejected teardown can also each enter the conflict handler. The new blocked-restoration test covers a successful rollback, not this conflict path.
Resolution
Keep conflict cleanup, preview restoration, and finalization inside one shared teardown claim. Make waiting callers join that cleanup without repeating it. Add a test that blocks preview restoration after a rollback conflict and starts a second revert.

Confirmed — this is the same defect as the other three fsync threads, not four separate ones.
The rollback in the catch ran whenever backup mode had renamed target -> backup, including when the failure happened after the commit rename had already published. PostCommitDurabilityError tells the caller the content is at the target path, but the catch had already renamed the backup back over that target, so the error message and the file disagreed.
Fixed in the unit that owns the publish path, #1910, commit 58f803ddb: a committed flag is set immediately after the commit rename and the rollback is skipped once it is set, so only a pre-commit failure can restore the backup.
Regression test at the lowest layer that would have failed (commit rename succeeds, post-commit directory open fails, backup mode on): fails without the guard, passes with it. This unit carries the same copy of safeWriteText.ts, so it inherits the fix once #1910 merges first in the chain order U1 -> U2 -> U3 -> U4 -> U5 -> U8 -> U6 -> U7 -> U9.
Partly valid, but outside this unit's scope. The else branch is pre-existing main behaviour: the guarded publish is gated by the PREVENT_FOCUS_DISRUPTION experiment by design, and this unit's stated scope is the guarded path. Routing the experiment-off path through guardedWrite is a behaviour change for users with the experiment off, so it is not a fix this unit can make on its own. Recorded as a follow-up on the tracking issue so the gap is not lost: with the experiment off, an apply_patch move writes the destination with plain fs.writeFile and no completeness check.

499-500: The experiment-off move still writes the destination without the guard.
The enabled path now publishes through guardedWrite(..., "create", sourceComplete). The else branch at Line 506 still writes moveAbsolutePath with plain fs.writeFile, and it runs no partial-source check. A previous review reported this issue. The author recorded it as a follow-up outside this unit's scope.
The failure-reporting suite reads through the guarded stat-around-read observation contract; add the stat and observation-registry doubles so its failure paths do not report a spurious TypeError. 35 passed.

Learning: In src/core/tools/ApplyDiffTool.ts, ApplyDiffTool.execute() records a stat-stable internal read as partial when no prior observation exists. When a prior observation has the same version token, it preserves that observation's completeness. When the prior token differs, it leaves the prior observation unchanged so the guarded save can reject the stale version instead of silently refreshing authorization. The regression cases in src/core/tools/__tests__/applyDiffTool.guardedWrite.spec.ts cover matching complete observations and older complete observations.

…he default fs mocks
Review follow-up: record the held-lock state inside the unlink mocks and assert it after execute; afterEach restores stat/unlink defaults alongside realpath/lstat/readlink/lock. applyPatchTool.execute.spec 27 passed.
No throwing assertions inside mocks — both move tests now record whether the source/referent lock was held when unlink ran (unlinkHeldLock) and assert it after tool.execute returns, instead of throwing an expectation inside the mock (which surfaced as a handleError and obscured the real failure).
Default stat/unlink restored — the suite's afterEach now also restores the default stat (stable version token) and unlink implementations alongside realpath/lstat/readlink/withFileLock, so a test's mocks cannot leak into the guarded-write tests that follow.

Make the empty-scope premise check skip under a repo-local temp directory.
The premise check fails when os.tmpdir() resolves under process.cwd(). It then fails before the confinement assertion runs. The failure is loud, but it has nothing to do with the implementation. This only happens when a developer or runner sets TMPDIR, TMP, or TEMP to a path inside the working tree.

Update the “rejects an empty-string confineTo whose target is outside the

when `tempDir` resolves inside `process.cwd()`. Keep the confinement assertion
for cases where the temp directory is outside the working directory.
Record mention reads as observations only when they are version-matched and complete.
Record mention reads as observations only when they are version-matched and complete.
A file sent through a /path mention reaches the model, but ObservationRegistry has no entry for it. A later apply_patch update to that existing file is rejected as unobserved and asks for a re-read. The rejection is recoverable, and no content is overwritten.
Record a mention read only when all of these hold:
Matching version tokens: Capture the version token before the read. Capture it again after the read. Record the observation only when the two tokens match. A token captured only after the read can pair newer disk state with older content, so the match confirms that the content belongs to that version.
Complete content: The text must not be truncated, must not have clipped lines, must not be a transformed binary extraction, and must not be a lossy decode. extractTextFromFileWithMetadata reports wasTruncated, but its result has no lossy-decode field. Binary extraction reports wasTruncated: false. The mention path cannot prove completeness from its current result.
Registry access:getFileOrFolderContentWithMetadata does not take the registry. The registry must be passed to it through its callers.
Until the result can report these conditions, leave mention reads unrecorded. This keeps the current recoverable rejection.

Keep mention reads unrecorded in ObservationRegistry unless the read can
establish matching version tokens captured before and after, and complete,
untransformed content with no truncation, clipped lines, binary extraction, or
lossy decoding. The current getFileOrFolderContentWithMetadata result cannot
establish all these conditions, so do not record observations through its
callers until it can report them and receive the registry.
Pin the observed registry key to the resolved absolute path in both observation tests.
Pin the observed registry key to the resolved absolute path in both observation tests.
The first test in the observation registry block checks only toContain("existing.ts"), then calls reg.get(calledPath) with the same captured key. A relative key such as "existing.ts" passes both checks. Production code keys the registry by the absolute path from path.resolve(task.cwd, relPath), and guardedWrite looks up that same absolute key. A regression that observes relPath would therefore pass this test. The legacy-format test has the same substring-only check. Replacing both checks with an exact comparison closes the gap.

Update both observation registry tests to assert that the captured path exactly
matches the resolved absolute path for the corresponding file, rather than
merely containing its filename. Apply this to the existing.ts and legacy.ts
checks, and leave the registry lookup and version assertions unchanged.
Add a Task-level isolation test for the observation registry.
Add a Task-level isolation test for the observation registry.
The test "two separate Task-owned registries are independent" builds two bare ObservationRegistry objects and never constructs a Task. Task creates one registry per instance at src/core/task/Task.ts:299, but no test checks that. A change that shares one registry across Task instances would leave this test passing.
Add a test after "does not cross-drain queues between separate tasks" in src/core/task/__tests__/Task.spec.ts. That file already builds two real Task instances with createProvider(), apiConfig, and startTask: false:
it("gives each Task its own observation registry",()=>{consttaskA=newTask({provider: createProvider(),apiConfiguration: apiConfig,task: "task A",startTask: false,})consttaskB=newTask({provider: createProvider(),apiConfiguration: apiConfig,task: "task B",startTask: false,})taskA.observationRegistry.observe("/shared.ts","v1")expect(taskA.observationRegistry.get("/shared.ts")?.version).toBe("v1")expect(taskB.observationRegistry.get("/shared.ts")).toBeUndefined()})
Also rename the bare-registry test in readFileTool.spec.ts so its title matches what it checks:

Rename the bare `ObservationRegistry` test to clarify that it checks standalone
registries, not Task-owned registries. Add a separate test using two real `Task`

empty.
The no-side-effect check misses one case. Its fs.mkdir filter, startsWith(missingScopeParent + path.sep), excludes a call that creates missingScopeParent itself. fs.access(target) cannot detect that call because target sits below scope. A regression that creates only missingScopeParent before rethrowing the ELOOP error still passes this test.

The staging cleanup test replaces createWriteStream with a Writable double that has no file descriptor. The test therefore never runs cleanup while a real staging file is still open. Add a case that uses the real createWriteStream and makes the serializer fail on real input, such as a toJSON method that throws. Assert the rejection, then assert that no .file-safety-staging entries remain.
Do not wait for the file stream's close event in this test. On a serializer error, _streamDataToFile does not destroy fileWriteStream, so close may not be emitted. In the test teardown, destroy the stream and await its closure before cleanup assertions run.

…alias spelling of a confined write
Review follow-up for Zoo-Code-Org#1915 (threads 4447363355, 4447363373, 4447363393).
- safeWriteJson: after the locks are held, re-derive the lock key of the publish target the
confined write resolved under the lock. When it is not the file whose lock this call holds,
release and refuse (PublishTargetMovedError) instead of merging and committing under another
writer's lock.
- safeWriteJson: pin the lexical ancestor chain alongside the canonical one, so a confined write
to a missing target under a symlinked parent fails with AncestorReplacedError when that alias
is repointed before the commit.
- guardedWrite: name a remediation a clipped-line file can satisfy. A re-read shows the same
clipped line, so the rejection states that the file cannot be replaced in full and points at a
targeted edit of the visible text.
Two new cases in safeWriteJson.lockKey.spec.ts, each negative-controlled (pin removed -> test
fails -> pin restored), and the partial-observation rejection assertion extended.
…-over-link target
Review follow-up for Zoo-Code-Org#1915 (thread 4447363410).
The backup step copies the target with fs.copyFile, which follows a symlink. For a publish-over-link
write that copy held the REFERENT's bytes inside the LINK's directory - rights the referent's own
DACL never granted, and the rollback path could rename that copy over the link, replacing the link
with a regular file of outside content. The entry this publish replaces is the link, so no content
backup is taken for it; a regular publish-over-link target keeps its durable backup.
Tests: "publishOverLink over a symbolic link takes no content backup of the referent" and
"publishOverLink over a regular file still takes the content backup" (safeWriteText.spec.ts).
Negative control: with the guard disabled the link test fails on a copyFile call. The trailing-lstat
count in safeWriteJson.lockKey.spec.ts records this target-entry check.
… walked
The confined-ancestry pin measured the caller's path against realpath's spelling of the
scope root. When the scope root is reached through a link - which is what a Windows host's
temp or profile directory can be - path.relative against that canonical spelling climbs out
of the scope, _ancestorChainUnderScope collapses the chain to the root alone, and the alias
the write actually walked through is left unpinned: repointing it while the write holds its
locks moves the commit and every directory created on the way down while every recorded
identity still matches. That is how the confined-repoint test passed on this host and
published through the repointed alias on the Windows CI lane.
_lexicalScopeRootFor picks whichever spelling the lexical parent is genuinely under - the
caller's confineTo first, realpath's second - and the lexical chain is built from it.
The new test constructs that shape deterministically: a scope root named through a link, a
second link below it, repointed inside the merge callback. Without the fix it fails with the
same shape as CI (the write resolved instead of refusing). Both repoint tests now report a
swap that never landed before they report a missing refusal.
Verified on this host: misc group 2121 passed | 19 skipped, core group 4217 passed | 9
skipped, safeWriteJson pair 49 passed | 6 skipped, tsc --noEmit and eslint
--max-warnings=0 clean, negative control (lexicalRoot = scopeRoot) failed exactly the new
test.
…ling
The pin-shape test asserted expectedAncestorIdentities against an exact array of realpath
spellings. That encoded the host assumption the pin fix removed: where the scope root is
reached through a link, the pin legitimately carries four names for the same two directories
(realpath's spelling plus the caller's own), and the test failed on the Windows CI lane with
'expected [ ... (4) ] to deeply equal [ ... (2) ]' while passing on a host whose temp
directory is not a link.
Compare the directories the pinned names denote - realpath set equality - and require the
caller's spellings to be present, which is what _lexicalScopeRootFor guarantees.
The new test builds that host shape deterministically: a junction as the scope root with the
confined target below it. Negative control (lexicalRoot = scopeRoot) turned exactly two tests
red: this one, 'expected [ ... (2) ] to include <rootalias>', and the lockKey spec's repoint
test, 'the confined write published through the link named by the scope root instead of
refusing it'. Restored byte-identically and re-run green.
Verified here: misc group 2122 passed | 19 skipped, core group 4217 passed | 9 skipped,
safeWriteJson pair 50 passed | 6 skipped, tsc --noEmit exit 0, eslint --max-warnings=0 exit 0,
prettier --check clean.
Conflict set resolved per the stack's settled copy sources; every copy source
named per file group:
- src/utils/safeWriteJson.ts + src/utils/__tests__/safeWriteJson.test.ts +
.lockKey.spec.ts + .interruption.test.ts: copy source = settled Zoo-Code-Org#1916 head
711ada7 (which itself pins the json quartet at
74f5eb2, the Zoo-Code-Org#1914 head). Main-side taskMessages.ts consumes
SafeWriteJsonTraceStage + the trace option, which only this json generation
provides; the era json predates main's Zoo-Code-Org#2297 and fails tsc against it.
- src/services/file-safety/safeWriteText.ts + spec + integration spec: same copy
source (the same 74f5eb2 pin). json and text travel as one generation.
- src/core/tools/guardedWrite.ts + src/core/tools/__tests__/guardedWrite.spec.ts:
copy source = 711ada7 - the U8-era guardedWrite surface adapted to the pinned
generation: publish calls carry backup: true, resolveLinkPathLockKey and
canonicalDirKey are module-local helpers, and the spec's mock arguments spell
backup: true where the era spelled retainedTarget: true (31 sites; the 12
DiffViewProvider.spec sites travel with that file's copy below).
- src/integrations/editor/DiffViewProvider.ts + __tests__/DiffViewProvider.spec.ts:
copy source = 711ada7 in full. This is the era x 3ee042d x main 3-way
resolution Zoo-Code-Org#1916 carries, and it includes the teardown-tail deadlock fix:
revertChanges' tail finalizes via finalizeSessionFromTeardownTail() - the same
finalization minus the session-start join, because the reservation that join
would wait on is itself waiting on this pass (reset -> reservation -> open ->
teardownInFlight -> the tail -> reset would never settle). The fix is verified
IN THIS TREE, not inherited silently: its regression test "revertChanges' tail
does not deadlock when open() starts during the pass" runs in this branch's
misc shard gate, together with the companion ownership assertion the fix owes.
- src/eslint-suppressions.json: main's counts first (the theirs side); u6-only
file entries kept only where main carries none (25 entries); then eslint
verify with --prune-suppressions on the merged tree. No prune carried across
the merge: shared-file counts restart from main and only eslint may lower them.
Generation-pairing rule enforced by this resolution (adopted as a rule by the
stack lead; the four-generation map and the discriminator experiment are posted
on Zoo-Code-Org#2350 where the convergence review reads them): json plus its specs and text
plus its specs travel as ONE generation, always. The evidence behind it: on
Zoo-Code-Org#1916 an era-text x 74f5-json chimera passed tsc --noEmit, every ubuntu leg, and
every local misc shard, and failed only on the windows platform-unit misc shard
(15 rows in the era windows-only ACL/backup-stat and interruption kill-timing
branches, run 42459507387 job 128665547657) - the shape of a red a reviewer
would attribute to the change in the diff rather than to the pairing.
Auto-merged Task.ts accepted after review: same two-lineage splice Zoo-Code-Org#1916 merged
(era-side hunks and main's Zoo-Code-Org#2297 hunks are disjoint regions).
- src/utils/__tests__/safeWriteJson.test.ts additionally adopted from 025fb7d
(Zoo-Code-Org#2359 head, test-only hardening, keyed on inode not path spelling): the
windows platform-unit rows that motivated it are the same on any tree
carrying 74f5eb2's json test (reason lines read from runs 42461215177
128670729075 and 42448302148 128631851822). src/utils/safeWriteJson.ts is
byte-identical between the two refs, so json + its specs still travel as one
generation - this branch adopts the generation's windows-proven test file
(025fb7d windows platform-unit green with this file against the identical
implementation, run 42455872567 job 128654644966), matching Zoo-Code-Org#1914/Zoo-Code-Org#1916
byte-for-byte so the stack carries one copy of this test.
Verdict — head 42a1bbb86e281cd5d9368d40bf3c19cf3a577271 (merge resolution, gates on the committed tree)
What this head is.main@73931fe8b merged into 94f4a23f7 (U6, apply-patch wiring). The conflict cluster (applyPatch/Task/dvp/safe-write/guardedWrite) was resolved by wholesale adoption of the settled blobs: the dvp pair from settled #1916 head 711ada709 (whose json/text trio and guardedWrite are themselves pinned at the #1914 generation 74f5eb287), and safeWriteJson.test.ts additionally from 025fb7d0d — test-only, same implementation (byte-identical safeWriteJson.ts), windows-proven pair, per the generation-pairing rule recorded on #2350 (addendum comment 6719173333). Suppressions = main's blob plus the 25 u6-only entries where main has none, then a full-tree prune; the merge commit message names each conflict's copy source, as required.
The U8 teardown-deadlock fix travels here and is verified IN this tree:revertChanges()' tail finalizes through finalizeSessionFromTeardownTail() instead of reset(), breaking the reset-join ↔ open-on-teardown cycle, and this branch's own misc shard runs the regression test ("revertChanges' tail does not deadlock when open() starts during the pass") green — not inherited from #1916's shard.
CI (as of posting).platform-unit-test ubuntu success, windows success; knip success; mutation-diff failure — advisory, reason read from job 128673972349: "extension has 1821 changed executable lines (limit 500)" (vs PR merge-base; same advisory class as every head in this chain). extension-host-visual and the CodeRabbit re-review were still running when this comment posted.
Local gates (idle, windows box). tsc 0; core 4385 passed/9 skipped exit 0; misc 2243 passed/17 skipped exit 0 (deadlock test included); unit 12712 passed with only the 5 known rules.spec symlink-EPERM environment rows; services red only in the same 5 environment rows; knip 0/0 from root; prettier clean on all 13 merge-unique files. (Gate-invocation note for reviewers: prettier/knip run from <wt>/src produce false findings — 964 phantom warns / 87 phantom unused-exports — the numbers above are the repo-root invocations.)
Mutation recount (local, named base). vs 94f4a23f7 (the branch head before this merge): types 15/500, webview 401/500, extension 2016/500. CI's 1821 uses the PR merge-base — different base, same advisory class; both named.
Review state. All 51 review threads on this PR are resolved. The CodeRabbit CHANGES_REQUESTED wall (Oct-05→Oct-22) is per-push bot reviews of earlier heads; the required-approvals list (taltas, navedmerchant, edelauna, JamesRobert20) has no human approval yet.
Plainly: the merge resolution passes every mechanical gate on its committed tree and the deadlock fix is proven in this branch's own test shard. What remains is human approving review including at least one code owner.

Review comments at @src/core/tools/guardedWrite.ts:
- Line 477: Update both safeWriteText calls in the replacement and create paths
to pass expectedResolvedPath: publishTarget, ensuring each publish stays bound
to the target retained by the guard checks.
Review comments at @src/integrations/editor/__tests__/DiffViewProvider.spec.ts:
- Line 3781: Update the rollback conflict assertion and the two neighboring
conflict tests to verify that safeWriteText was never called, rather than only
checking that it was not called with "original". Preserve the existing conflict
expectations.
Review comments at @src/integrations/editor/DiffViewProvider.ts:
- Line 2358: Update the direct and preview save paths in DiffViewProvider to
pass the targeted-edit kind "edit" to EditTool and SearchReplaceTool. Preserve
the existing observation and version checks, and avoid changing full-file write
behavior.
- Around line 885-887: Add a session-validity check in the save flow around
`vscode.workspace.encode`, before `guardedWrite` publishes the encoded content.
If the session is no longer active after encoding completes, stop the save so it
cannot overwrite the restored file; preserve the existing behavior when the
session remains active.
- Line 904: Keep the preview created in the DiffViewProvider save flow isolated
from the target file until approval; do not let Auto Save or apply_patch write
preview content directly to the target. Ensure only guardedWrite publishes
accepted content, while preserving the existing rejection and cleanup behavior.
- Around line 2223-2224: Coordinate the public reset path in DiffViewProvider
with any active rollback so session state is not cleared until rollback
completes, including when rollback is waiting for the file lock. Keep
performFinalReset’s teardown tail from awaiting its own finalizationInFlight
promise; preserve this non-self-waiting behavior while ensuring resets outside
that tail wait for rollback before clearing session state.
- Around line 2207-2209: Update the reset flow in DiffViewProvider to join
sessionStartInFlight before checking sessionFinalizationClaimed, so a reset
during startup waits for the new session to finish joining before deciding
whether finalization is already complete. Preserve the existing finalization
behavior after the startup wait.
- Around line 834-840: Restrict every adoption path around the
preOpenObservation and openToken checks to rejections identified as
GuardStaleVersionError; do not adopt or record a new observation after ignore
denials or other rejection types.
- Around line 1580-1589: Update the `!ownedTeardown` path in `revertChanges()`
so that, after joining the in-progress save cleanup, it still performs the
rollback with the existing version check before returning. Preserve the cleanup
wait and avoid running a duplicate reset.
- Around line 1497-1503: Update the cancellation flow around closeAllDiffViews
and RollbackConflictError so the cancelled preview proposal cannot remain
eligible for a later save and overwrite the peer’s disk changes. Discard only
the proposal’s edits, or otherwise prevent them from being saved, while
preserving independent user edits and the peer’s content.
- Around line 2358-2359: Update the move publish call in DiffViewProvider so
`reobserveBasis` is false and `completeOverride` receives `sourceComplete`;
preserve the source completeness value when recording the destination.
- Around line 447-454: When placeholder creation fails after directories are
created, clean up the empty directories tracked by createdDirs before reset
discards that tracking. Preserve any directory containing files, including files
created by another writer; update the open failure cleanup flow around
openDiffEditor and undoPartialOpen without removing non-empty directories.
- Around line 383-391: Bind placeholderIdentity to the file created by open(),
not whichever file occupies absolutePath at the first pathname stat. Capture the
created file’s identity from the open handle and ensure failed-open cleanup
rechecks that identity before unlinking, preserving any replacement file.
- Line 1095: Update the diff cleanup paths in DiffViewProvider, including
accept/reject handling and rollback-conflict cleanup, to close only the current
task’s diff tab instead of calling global cleanup through closeAllDiffViews.
Preserve cleanup behavior for the current task while leaving other tasks’ open
previews untouched.
- Line 1506: After replaceIfVersion successfully restores originalBytes, refresh
the observation token only if the current observation still describes the
restored content; preserve its completeness and do not overwrite a newer
observation. Use the existing observation state and validation mechanisms to
establish that match before updating the token.
- Around line 264-267: Update the `open()` flow to preserve the new session’s
requested `editType` before awaiting `finalizationInFlight` and
`teardownInFlight`, then use that preserved value when determining `fileExists`.
Ensure finalization cannot clear or replace the requested type before the new
session reads it.
- Around line 249-251: Prevent overlapping open attempts in DiffViewProvider by
making the sessionStartInFlight reservation cover the full runOpenTransition
lifecycle, including pending writes and cleanup, so retries wait for or reuse
the active attempt instead of starting another; do not rely on isEditing as the
concurrency guard.
- Around line 1123-1129: Update saveChanges to preserve
teardownCancellationRequested when cancellation occurs during diagnostic
processing, then check it before returning the proposed content. Ensure the
caller treats a cancelled save as unsuccessful, and do not clear the
cancellation signal before the caller can act on it.
- Around line 2388-2391: Update the WriteToFileTool flow around saveDirectly and
guardedWrite so parent directories are not left behind when a queued write is
rejected; defer directory creation until the guarded write is accepted, or
otherwise remove only newly created empty directories on rejection. Preserve
existing directories and the current guarded-write behavior.

@src/services/file-safety/__tests__/safeWriteText.integration.spec.ts:
- Around line 25-31: Update the comment in the safeWriteText integration test to
state that DACL restoration is best-effort after commit and a restore failure
does not reject the write. Remove the claim that restoration failure throws and
the citation to the nonexistent focused test.
Review comments at @src/services/file-safety/__tests__/safeWriteText.spec.ts:
- Around line 1030-1055: Update the “fsyncs the parent directory after the
commit rename on POSIX” test to assert that the `fs.rename` call occurs before
the second `fsSync.fsyncSync` call, while preserving the existing
descriptor-order and close assertions.
- Around line 616-625: Strengthen the Windows DACL test around safeWriteText to
verify the restore destination and its order relative to fs.rename, rather than
only checking execFile call count. Establish the intended DACL restore sequence
from the implementation and make the test assert that behavior without assuming
a destination or order the implementation does not guarantee.
Review comments at @src/services/file-safety/safeWriteText.ts:
- Around line 527-533: Update the write loop in safeWriteText to detect a
zero-byte result from fsSync.writeSync and fail the write instead of looping
indefinitely. Preserve offset advancement for positive writes and ensure the
failure propagates through the existing cleanup path.
- Around line 282-295: Update _restoreDaclWindows so an icacls restore failure
is surfaced as a post-commit error instead of being swallowed, and preserve the
DACL dump when restoration fails so the caller can warn the user or use it for
repair. Ensure successful restores retain the existing best-effort flow.
- Around line 786-793: When backup cleanup fails after publication, update
guardedWrite to record the landed token under the lock for
OrphanedPublishedBackupError before propagating the cleanup error. Preserve the
existing cleanup-error propagation and ensure retries observe the published
replacement rather than the stale prior observation.
- Around line 412-418: Update the symlink traversal in safeWriteText’s lock-key
resolution to follow the complete chain instead of stopping after eight links.
Track visited keys to detect cycles, and resolve the lock key to the terminal
referent while preserving the existing canonicalization behavior.
- Around line 621-628: In the `safeWriteText` backup flow, revalidate
`expectedAncestorIdentities` after the backup copy completes and before the
rename publishes the file. Preserve the existing backup behavior and safeguards;
this recheck narrows the race window without claiming to eliminate the final
rename race.
- Around line 813-819: Backup cleanup errors are silently discarded when a
publish fails. Update the cleanup path around backupPath and
releaseBackupOnSuccess to surface the backup path and cleanup error while
rethrowing the original publish error unchanged, preserving its identity,
including for PostCommitDurabilityError.
- Around line 470-478: Update the staging-path validation around
`options.tempPath` to reject it when it resolves to the target, before entering
the cleanup block. Reuse the existing canonical path check and preserve the
regular-file and symlink validations.
- Around line 470-478: Update safeWriteText so a caller-supplied tempPath cannot
be replaced between validation and publication, ensuring the file published by
the commit is the validated regular staging file. Establish protection that
holds through the rename; a one-time check or recheck that leaves a substitution
window is insufficient, and preserve the existing staging-path safeguards.
- Around line 506-522: The staging file must retain the existing target’s group
before publication. In the target-mode handling around openSync and fchmodSync,
obtain and apply the target’s group to the staging file before rename; if the
group cannot be preserved, abort the write rather than publish with altered
permissions. Keep the fresh-target behavior unchanged.
- Around line 9-13: Update the backup option documentation in safeWriteText to
state that backup: true copies the existing target without moving or restoring
it, and that a post-commit error may leave the new content at the target.
Clarify that backup: false makes no backup copy; do not promise rollback.
- Around line 758-766: Update the Windows publication flow around daclDumpPath
and _restoreDaclWindows so the target’s restrictive DACL is applied to the
staging file before the commit rename, ensuring the file is protected as soon as
it is published. Reuse the existing saved DACL and restoration safeguards, and
do not rely on applying the DACL only after the rename.
- Around line 439-441: Update safeWriteText to persist the parent-directory
chain before reporting a durable write: sync each directory entry in its parent,
including ancestors that existed before the call, and propagate any sync failure
instead of reporting success. Use the project’s supported directory-sync
mechanism without weakening existing write safeguards.
- Around line 726-740: Update the successful fs.link cleanup path so a failed
fs.unlink does not silently abandon the staging file and its directory: retain
cleanup ownership and retry or report cleanup separately from the committed
publish status. Preserve stagingCommitted’s role in preventing post-commit
cleanup from removing published content, and do not report the write as failed
after the content has been committed.
Review comments at @src/utils/__tests__/safeWriteJson.lockKey.spec.ts:
- Around line 195-196: Update the safeWriteJson lock-key test to keep the
consoleError call-count assertion and also verify that the logged message and
error object are the original failure, checking the error by identity rather
than value. Ensure an ENOENT cleanup log cannot satisfy these assertions.
Review comments at @src/utils/__tests__/safeWriteJson.test.ts:
- Around line 667-671: Update both identity reads in the test’s
`isInjectedInode` check to request bigint stats: the
`actual.stat(unreadableBackup)` call and the `actual.stat(candidate)` call.
Compare their `dev` and `ino` values without number rounding so distinct backup
identifiers remain distinguishable.
Review comments at @src/utils/safeWriteJson.ts:
- Around line 67-70: Correct the race-condition documentation in the `confineTo`
comment: describe that an ancestor swap before staging can let the commit
replace an existing file outside the scope, and that post-commit undo may delete
the replacement without restoring the original. Remove the inaccurate
`StagingPathError` explanation and preserve the stated limitation that
`confineTo` confines the resolved target, not its ancestor tree.
- Around line 397-399: Update the staged-file flow around _copyTargetMode and
_flushFileToDisk to open the staged file before applying the target mode, then
sync its contents through that already-open handle so read-only permissions do
not prevent the sync.
- Around line 481-488: Update the confinement check around _resolveScopeRoot and
_assertWithinScope so any resolution or validation failure triggers safe
staged-file cleanup and backup rollback before the error propagates. Ensure
cleanup also works when path resolution fails and cannot leave the target
missing; preserve the existing confinement safeguards and use only paths that
can be safely established.
- Around line 294-298: Update the publish flow around resolvePublishTarget to
recover a dangling alias’s referent from its backup while holding the referent’s
lock, before strict alias resolution. Preserve confinement checks and revalidate
the target after recovery; keep rejection of genuine dangling links and ensure
failures release the advisory lock.
- Around line 585-598: Update the mode-preservation flow around fs.stat and
fs.chmod so failures that prevent reading or applying the existing target’s mode
propagate and abort publication before rename. Preserve the existing handling
for ENOENT and ENOTDIR when no target mode is available, and update the failure
tests to verify publication is aborted when mode preservation fails.
- Around line 298-307: Validate that resolvedTargetPath still corresponds to the
held lockKey before recovery and publication; the alias’s current key alone can
miss a retarget-and-return. Use the existing key-resolution mechanism to check
the retained target, and retry or reject on mismatch before proceeding.
- Around line 601-602: Update the comment for _flushFileToDisk to describe
syncing the fully written staged file to storage before the rename, not syncing
the directory after commit.
- Line 407: Update the confined commit path at the fs.rename call so it cannot
resolve the destination through an ancestor swapped after the confinement check.
Ensure the rename is anchored to the verified ancestor or otherwise protected
against replacement, and abort without modifying the destination if that
protection cannot be established.
- Around line 387-388: Update the confined-write verification guarded by
confinementRoot to avoid loading the entire staged file into stagedBytes; verify
it in bounded-memory chunks while preserving the existing escape-undo guarantees
and complete payload validation. Ensure escape undo also avoids allocating a
second full-file buffer, using an approach that preserves its current behavior.
- Around line 200-206: Redesign the escape cleanup around publishedPath so it
unlinks only when it can atomically establish that the file is still the exact
content this operation published; the current before/after stat checks cannot
detect same-size in-place rewrites. Use an ownership-safe approach that closes
the check-to-unlink race, and otherwise leave the file intact.
- Around line 192-206: Update the escape cleanup around publishedPath to track
whether the commit succeeded and establish that this operation owns the file
before unlinking it. Do not treat matching expectedBytes or stable device/inode
metadata alone as proof of ownership; preserve the existing re-stat checks and
skip deletion when ownership cannot be established.
- Around line 196-206: Update the escaped-publish cleanup path around
ConfinedPathEscapeError to report the cleanup failure and publishedPath when
both cleanup attempts fail, so the caller can arrange recovery. Preserve
ConfinedPathEscapeError as the primary error and retain the byte-content and
file-identity checks that prevent deleting peer content.
- Line 283: Update the lock coordination around acquireFileLock in safeWriteJson
so settlement and deletion of a symlinked history_item.json serialize on
compatible locks; settlement must not publish success or cache the task if
deletion removes its history path. Preserve the existing safeguards while
ensuring the lock key covers both the alias and referent as needed.

Failed pre-merge checks:
- Linked Issues check:
Route the experiment-off move publish in ApplyPatchTool.ts through the guard.
Replace the direct fs.writeFile call with a guarded publish (saveChanges with
"create", or an equivalent guardedWrite call). Carry the source completeness to
the destination first, and apply the same partial-source rejection that the
experiment-on branch applies. Add a test that covers a partial-source move onto
an observed destination with the experiment off. If the issue owner decides that
the experiment-off move path is out of scope for #2021, update #2021 to say so.
Then this check can pass for the guarded branch alone.
- Regression Evidence:
Add focused tests in `src/core/task/__tests__/Task.spec.ts` that verify a real
task exposes an `ObservationRegistry` and two real tasks receive distinct
registries. Keep the standalone registry tests.
- Security Boundaries:
Preserve the target DACL on the staged file before publication, and fail closed
if DACL capture or application fails. Ensure staging and backup copies do not
expose the content. Surface any post-commit restoration failure and add
regression tests for both DACL capture and restoration failures.
- Persistence Integrity:
Track and remove newly created parent directories on pre-commit failure,
innermost-first, and add a real-filesystem regression test. Use atomic
no-replace publication for `createIfAbsent` and handle an existing-target error.
Pass `sourceComplete` as `completeOverride` and test a partial-source move to an
unobserved destination followed by a full replacement.
- Lifecycle Resource Cleanup:
Keep conflict cleanup, preview restoration, and finalization inside one shared
teardown claim. Make waiting callers join that cleanup without repeating it. Add
a test that blocks preview restoration after a rollback conflict and starts a
second revert.

Learning: In src/core/tools/ApplyPatchTool.ts, if destination publication during a move throws PostCommitDurabilityError, preserve the source and propagate the error. The destination rename has committed, but its directory entry is not known to be durable. Unlinking the source could leave neither path available after a crash. Do not attempt source unlink or convert this failure into PartialMoveError.

Measured: adoption after a refused write already requires the stale-version pairing — the pairing check returns true only when the caller's observation still names the version the preview saw ('the caller's observation must still name the version the preview saw'). A post-preview .rooignore change with a clean matching buffer is the documented 'the guard was right to reject; the matching bytes are the clobber it warned about' case. The restriction this row asks for is the implemented rule.

Prevent overlapping diff sessions across retries.
If a stream fails while a partial write is still pending, the retry can start another write on the same provider. Both calls can enter open() before isEditing is set. One diff can then use the other file’s content or editor, and listeners can escape cleanup.

Prevent overlapping open attempts in DiffViewProvider by making the
sessionStartInFlight reservation cover the full runOpenTransition lifecycle,
including pending writes and cleanup, so retries wait for or reuse the active
attempt instead of starting another; do not rely on isEditing as the concurrency
guard.

Measured against the flagged tree: open() takes a session-start reservation (sessionStartInFlight) and runOpenTransition joins any in-flight finalization and teardown before assigning session state; reset() joins the in-flight finalization instead of starting a second teardown. The interleaving the model shows is exactly what those joins serialize — the TLA step abstracts them away. No gap found; no change.

Preserve the requested edit type across finalization.
If a modify preview starts while the previous session is finalizing, the reset can clear editType before open() reads it. Confirmed code-true: fsSync.writeSync can return 0 without advancing, and the loop spun forever with cleanup never running. Fixed at the origin lane (the shared staging loop lives in U5-era content): U5 now fails the write on a non-positive result so teardown runs, with a focused regression test (one attempt, no fsync, no publish, staged file unlinked) — commit 3dcc4b6. This branch receives it through the declared U5 merge order; not copied sideways.

Correct the race-condition documentation in the `confineTo` comment: describe
that an ancestor swap before staging can let the commit replace an existing file
outside the scope, and that post-commit undo may delete the replacement without
restoring the original. Remove the inaccurate `StagingPathError` explanation and
preserve the stated limitation that `confineTo` confines the resolved target,

When false (default) the atomic rename simply replaces- * the target -- crash-safe window is zero.+ * When true, copy an existing target before publishing without moving it.+ * The backup is not restored on failure. No change made on this branch; marking this row answered by the merge-order path.

Adopted at the origin lane: the comment lives in shared U5-era content and was corrected in 3dcc4b6 — the helper flushes the staged file to storage before the commit rename, which is what its body does (fs open 'r+' + handle.sync on the staged path). Measured against the flagged tree: the staging-directory lifecycle (creation, per-write uniqueness, best-effort removal, failed-removal tolerance, caller-owned directories) is covered by a dedicated describe in the spec with five focused tests; the suggestion restates covered behavior. Posture answer, verified against this tree: the durability contract is the rename protocol: the staged file is fsynced before the commit and the parent directory is fsynced after it (spec: 'fsyncs the parent directory after the commit rename on POSIX'); a refusing filesystem must not block the publish, which the best-effort comments state. Task deletion can interleave with settlement through a symlink
If history_item.json is a symlink and another store deletes it after safeWriteJson rechecks its lock key, deletion locks and removes the alias while settlement holds the referent lock. Settlement can then read and publish the referent, return success, and cache the task after its history path was deleted. This violates the deletion and settlement serialization contract.

This PR routes the
apply_patchtool through the guard, so a patch hunk can no longer "write" a file the model never fully read: an unobserved overwrite and a stale version token are refused with the standard remediation. A move carries the source's completeness to its destination instead of claiming completeness for lines the model never read, and a partial-source move onto an already-observed destination is rejected before any state changes. It also fixes a resource leak: when the guard refuses a publish, saveDirectlynow rolls back the parent directories it created. It is the sixth of nine PRs for epic #1375 (chain tracked on #1991).

Description
What this PR changes:
apply_patchpublishes viaguardedWrite, so an unobserved overwrite and a stale version token are rejected with the read-first / re-read-then-retry remediation instead of clobbering the file.completeflag, so a slice, range, truncated, or indentation-block source never authorizes a full-file replacement at the new path.saveDirectlycaptures the listcreateDirectoriesForFilereturns and, if the guard rejects, removes those directories innermost-first withrmdir(which refuses a directory another writer populated, so the loop stops at the first failure) and rethrows the original write error.AncestorReplacedErrorif an ancestor was swapped underneath it.Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on

Re-review responses (error-fix re-review wave)
answered by completing the source unlink and reporting unlink failures as
PartialMoveError- since SUPERSEDED by the crash-availability reversalbelow; see the next section for the shipped semantics.
Crash-availability reversal (durability-error move)
A follow-up review (Major Data-Integrity) reversed the previous round's
guidance for the same branch, and the new semantics shipped: when the
destination's commit rename landed but its directory entry is not known to
be durable (
PostCommitDurabilityError), the source is KEPT and thedurability error propagates - unlinking it could leave NEITHER path
available after a crash.
The move now
deletes under the shared advisory lock (
withFileLockon the canonicallock key): the source must still be present in the observation registry
and its recorded version must equal a fresh bigint stat of the source,
otherwise the move raises
PartialMoveErrorbefore any unlink. A statfailure is likewise a
PartialMoveError- an unreadable source is not adeletable source.
Durability note: on the durability-error path the source is kept
outright -
PostCommitDurabilityErrorpropagates before the deletion isreached - the strongest form of the protected-deletion contract this
unit's issue asks for.
The PI row's raw-unlink line numbers belong to the
superseded head
993fc6f; at this head the deletion is lock-serializedand observation-bound exactly as the Resolution demands (missing,
changed, vanished, or unobserved sources are kept;
PartialMoveErrornames the committed destination and remaining source; the durability
path keeps the source outright; tests and negative controls above). The
SB row re-flags the carrier copy's warn-only DACL restore, whose
fail-closed fix lands at the publish-primitive owner (PR 1916,
3624e4fea) that merges before this unit; copies reconcile at rebase(record 6106959976 on issue 1408). Evidence comment posted on the pull
request; no code change this round.
