feat(tools): guarded write core under the shared lock (U5, #1375) - #1914
This PR adds guardedWrite, the single publish entry point every write tool will use. It turns "last writer wins silently" into "a write that was not earned by a read fails with a remediation the model can act on": create-if-absent for new files, compare-and-swap on the version token for observed files, a cancellation re-check before publication, and the read-first / re-read-then-retry remediation text on refusal. All publishes for one path run on a per-path FIFO chain, so concurrent writes to the same file are ordered.
It is the fifth of nine PRs for epic #1375 (chain tracked on #1991), and the core the remaining units wire their tools into.
guardedWrite(): resolves the target, checks containment, picks the guard from the task's ObservationRegistry - create-if-absent, recreate, compare-and-swap on the version token, or the unobserved-edit remediation - and runs the publish on the per-path FIFO chain.
createIfAbsent and replaceIfVersion semantics: a stale version token or an unobserved overwrite is refused instead of clobbering the file.
Cancellation is re-checked when the link is dequeued, before publication - a write cancelled while queued does not publish.
The model-facing remediation on rejection: re-read the file, then retry.
Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on main and carries its own copy of the earlier units' content, which the GitHub diff shows as context.
Review focus:src/core/tools/guardedWrite.ts and src/core/tools/__tests__/guardedWrite.spec.ts.
Test Procedure
From the repository root, with dependencies installed:
Environment: Windows 11, Node 20, unit layer only.
Pre-Submission Checklist
Scope is one gate scope and matches the unit.
No .changeset or CHANGELOG changes (AGENTS.md).
src/eslint-suppressions.json unchanged - no suppression count increased.
New code lints clean (--max-warnings=0) rather than relying on suppressions.
Tests added at the lowest layer that would have caught each finding.
Visual Snapshot item considered: not applicable (no rendered surface).
Visual Snapshots
Not applicable - no rendered surface.
Videos (interaction / animation only)
Not applicable.
Documentation Updates
No user-facing documentation change: the remediation text is model-facing and introduced with this unit's guard. No .changeset and no CHANGELOG edit (AGENTS.md).
Additional Notes
Budget: 1277 a+d / 418 changed executable lines. 1277 a+d is above the 1000 cap - documented deviation: guardedWrite.ts is a new file and its spec tests that file as a unit, so the two cannot be separated without breaking the fidelity contract.
mutation-diff is red on the changed-line budget only: the gate counts the GitHub diff, which carries the unlanded predecessors (786 changed executable lines against the 500 cap; this unit's own delta is 418, inside the cap). The row clears when the predecessors land and this branch is rebased.
Two Minor findings fixed with regression tests and negative controls:
Copy-fallback durability: the COPYFILE_EXCL fallback creates a NEW
inode at the target that nothing fsynced (the staged inode's pre-commit
fsync does not cover it; step 4b flushes only the parent directory).
The copied target is now opened and fsynced right after the fallback;
failure reports PostCommitDurabilityError (new copied-target message
variant) instead of a swallowed success. NC: removing the flush reddens
exactly the two new copy tests.
Lock-path termination:canonicalLockPath recursed with a fresh
per-frame hop bound, so a cycle whose length does not divide 8
(a -> b -> c -> a) recursed forever on the lock path. The recursion now
carries a total budget of 64 hops; when spent, the next frame walks
zero hops and canonicalizes the cycle member it stopped on. NC: the
unbounded form reddens exactly the new 3-cycle test (timeout).
The copy-fallback target flush now opens with "r+" (Windows FlushFileBuffers requires GENERIC_WRITE; a read-only handle could fail
the flush and report a false durability error on published bytes). The
test pins the write-access flag; NC: reverting to "r" reddens exactly
that test. 68/68 green.
Rollback fail-closed on unsupported flush (e5e71aa)
The Persistence Integrity row asked what happens when the flush-access
retry itself is unsupported. The copy+fsync fallback answered by
silently downgrading durability: on a filesystem that rejects the
write-handle flush with EPERM/ENOTSUP/ENOSYS it copied the buffer over
the target and reported success. The fallback is removed: those three
codes now raise UnsupportedFilesystemError (target path and cause
attached) and the write fails closed - a filesystem that cannot prove
the flush cannot prove the publish. The five fallback tests collapse to
one fail-closed test that drives all three codes through the same
publish; negative control: re-adding the copy fallback reddens exactly
that test. 66/66 green at this head.
Security Boundaries disposition (already-registered family). The row
repeats the ancestor-swap claim (openat/renameat-style directory-identity
pinning) already dispositioned and registered: the fix lands once at the
publish-primitive owner and copies reconcile at rebase (record 6106959976
on issue 1408; tracking 1991). No code churn this round for that row.
Resolve them on the pull request, or post @coderabbitai approve as a new top-level comment to resolve them and request approval.

Summary
Summary by CodeRabbit
New Features
File reads now distinguish clipped content from omitted lines and indicate when results are incomplete.
File updates check that files have not changed since they were read; edits require a prior observation, while new files can only be created when the target is absent.
Bug Fixes
Writes to the same file are serialized and reject out-of-workspace paths, changed targets, and unsafe symbolic-link paths.
JSON and project configuration writes use resolved paths for locking and updates, preserve existing permissions, and better handle failed writes and cleanup.
The changes add task-scoped file observations and guarded writes. Reads record matching file versions and completeness. Writes check observations, workspace containment, and cancellation before publication. Text and JSON writes use resolved targets and canonical lock paths; project-scoped MCP writes pass the workspace root as a confinement scope.
Changes: File observation and write safety
Layer / File(s)
Summary
Record read observations and completeness src/core/task/observationRegistry.ts, src/core/task/Task.ts, src/core/tools/ReadFileTool.ts, src/integrations/misc/indentation-reader.ts, src/core/task/__tests__/*, src/core/tools/__tests__/readFileTool.spec.ts, src/integrations/misc/__tests__/*, src/eslint-suppressions.json
Tasks own observation registries. Native and legacy reads record matching version tokens. Completeness reflects partial views, clipping, truncation, and lossy decoding. Indentation-reader results report clipping separately from omitted lines.
Resolve, stage, and publish files src/services/file-safety/safeWriteText.ts, src/services/file-safety/__tests__/safeWriteText*, src/utils/fileLock.ts, src/utils/__tests__/fileLock*
safeWriteText resolves targets, stages and syncs content, and supports backups, no-replace publication, and target and ancestor checks. fileLock uses canonical lock paths while preserving caller paths for operations.
Guarded writes serialize by path and apply create, edit, and version-check rules. They check containment and cancellation, publish under a resolved-path lock, and refresh observations after publication.
Confine and resolve JSON writes src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson*.ts, src/services/mcp/McpHub.ts, src/services/mcp/__tests__/McpHub.spec.ts, src/utils/__tests__/safeWriteJson.interruption.test.ts
safeWriteJson uses resolved targets and lock keys, checks confinement, and attempts to undo an escaped publish. MCP project writes pass the workspace root as confineTo; global writes omit it.

Merge Risk: Moderate · up to 281c2
Resolve the JSON permission, recovery, and confinement failures before merging: under specific filesystem errors or concurrent changes, settings can become more accessible or another file can be overwritten or deleted. The new guarded-write API also needs its cancellation and publication guarantees completed before adoption.
Security Architecture Review
Security architecture risk:🟡 Moderate · up to bce5c
A filesystem error can occur after saved permissions have changed, while previously allowed actions remain authorized in memory until a refresh. This conditional risk requires existing authorization and a publication failure; no new remote access or privilege escalation was demonstrated.
Retained concerns
Medium · security · inferred: The newly introduced committed-but-rejected JSON outcome is not reconciled by MCP permission updates. A POSIX directory-sync failure can occur after the configuration rename; the rejection skips live tool-permission refresh while programmatic-update suppression can discard watcher events. Removing an existing alwaysAllow grant can therefore leave the prior in-memory grant available to automatic approval until another refresh. This requires an existing grant, enabled MCP automatic approval, and the filesystem failure; actual tool execution under this condition was not demonstrated.
Security review details
Security Blast Radius
inferred — The material exposure is local filesystem state and authorization derived from configuration written through the shared JSON utility. MCP updates can affect global or project configuration; task-history persistence also inherits the changed failure contract. The available evidence does not establish cross-tenant exposure or additional operating-system privileges.
Security Findings and Attack Paths
inferred — A model-requested MCP tool could remain eligible for automatic approval after a saved grant removal if directory synchronization fails after commit and the live permission refresh is skipped. The approval decision consults the supplied server-tool flags and additionally requires global MCP automatic approval. No evidence establishes attacker control of the filesystem failure or demonstrates execution through this conditional path.
Trust Boundaries and Controls
observed — Read observations follow ignore checks and user approval. Within the new guard, observations authorize mutation scope and detect stale versions; they are not a replacement for path-access authorization or protection against non-cooperating filesystem writers. Existing model-write publication remains outside this new guard boundary.
Resilience and Maintainability Implications
observed — With backup mode enabled, a post-commit directory-sync failure leaves new content at the destination and old content at a backup path, because backup deletion is success-only and rollback is pre-commit-only. JSON cleanup does not remove that backup. Backup retention after an unlink failure already existed at the base; this PR adds another retention path without establishing broader read permissions.
Hardening Proposals
proposed — Handle committed-but-not-durable outcomes explicitly at security-sensitive callers: reconcile live permissions with the published configuration or suspend automatic approval until reconciliation completes. Give retained backups explicit cleanup and recovery ownership without rolling old content over an already committed update.

Explanation
The title "feat(tools): guarded write core under the shared lock (U5, #1375)" matches the main change: the PR adds the guardedWrite publish entry point in src/core/tools/guardedWrite.ts, which runs create-if-absent and version-token compare-and-swap writes on a per-path FIFO chain under the shared publish lock. The title is specific and describes the primary change. It contains internal tracking jargon ("U5", "#1375") that a teammate scanning history may not recognize, and it uses a parenthetical that adds noise, but the core change is clearly stated.
Full details: Description check
Explanation
The description follows the repository template. It has a linked issue (Closes: #2020), a Description section that explains the what and the how (guardedWrite behavior, per-path FIFO chain, cancellation re-check, remediation text), a Test Procedure with commands and environment, and a completed Pre-Submission Checklist. Visual Snapshots, Videos, and Documentation Updates are marked not applicable with reasons. Additional Notes cover the diff size deviation, the merge order, and the red mutation-diff budget check. Some template prompts are answered with dense tracking references (chain IDs, comment numbers, unit labels) that reviewers outside the chain may find hard to follow. The Get in Touch section is left as the template placeholder with no Discord username, and the description uses some non-STE phrasing and contractions-free but long compound sentences. These are minor gaps, so the description is mostly complete.
Full details: Linked Issues check
Explanation
[#2020] Requirements are implemented in src/core/tools/guardedWrite.ts at head 281c245. (1) createIfAbsent refuses an existing target with "read the file first, then retry". It publishes with failIfExist: true, so a concurrent create also fails. (2) replaceIfVersion compares the on-disk token with the expected token and refuses a mismatch with "Stale version ... re-read the file, then retry". (3) enqueue() keeps a tail-promise FIFO chain per resolved path, and a rejected link does not block later writes. (4) cancelledBeforePublish() runs after the queue wait, after preflight, and under the publish lock. The raw summary states that a queued write captures the cancellation generation and rejects when it changes. (5) unobservedEditGuard rejects an edit with no prior read, using "read the file, then retry". Test evidence: the PR reports 47 passing tests in guardedWrite.spec.ts, and the spec summary lists create, update, edit, FIFO, version, and cancellation cases. I did not run the tests. The publish primitive behavior (safeWriteText, resolveLockKey, withFileLock) was taken from the summary and was not re-audited. All linked requirements are coding requirements.
Full details: Out of Scope Changes check
Explanation
src/services/mcp/McpHub.ts adds confineForMcpWrite and passes confineTo to safeWriteJson in updateServerConfig, deleteServer, and updateServerToolList (verified in the whole-PR diff against the merge base). guardedWrite.ts imports only safeWriteText, resolveLockKey, computeVersionToken, withFileLock, and Task. Neither guardedWrite.ts nor its spec reaches McpHub or safeWriteJson. Issue #2020 covers the guard core and does not mention MCP settings writes or safeWriteJson confinement. Per the raw summary, the McpHub.spec.ts confineTo assertions and the safeWriteJson.ts confineTo and undoEscapedPublish additions, with their tests in safeWriteJson.test.ts and safeWriteJson.lockKey.spec.ts, serve only this MCP path. The PR description says the branch carries predecessor-unit content, but it does not link the owning units to this PR. It also states that the scope matches the unit, which the diff contradicts. Not established: whether a linked predecessor issue covers these changes. The shared lock and safeWriteText changes that guardedWrite calls are connected and are not part of this finding.
Resolution
Remove the confineTo wiring and confineForMcpWrite helper from src/services/mcp/McpHub.ts, and remove its assertions from McpHub.spec.ts. Remove the confineTo option and undoEscapedPublish from src/utils/safeWriteJson.ts, and remove the tests that cover them (safeWriteJson.test.ts and safeWriteJson.lockKey.spec.ts sections for confinement). Keep the fileLock.ts and safeWriteText.ts changes that guardedWrite uses. Alternatively, link the issue that owns these changes and its acceptance criteria. Then update the PR description so its scope statement matches the remaining diff.
Full details: Security Boundaries
Explanation
The prior workspace-containment failure remains. guardedWrite.ts:526-558 authorizes and pins the path, but safeWriteText.ts:587-618 checks ancestors before pathname-based fs.link or fs.rename at lines 693-720. A local process can move the parent outside the workspace and replace its old name with a symlink after the check. The staged file and target then resolve outside the workspace. The new guardedWrite entry point has no production caller yet.
Resolution
Enforce workspace containment through publication, including protection against parent relocation. Use no-follow directory handles and descriptor-relative operations where supported, and fail closed where the guarantee cannot be provided. Add a regression test that moves the parent and installs a symlink between validation and commit.
Full details: Persistence Integrity
Explanation
src/utils/safeWriteJson.ts:481-488 can delete an unrelated file after a failed write. If an ancestor is replaced by a symlink after staging, the rename can fail because the staged file is no longer reachable. The catch block still calls undoEscapedPublish. If the outside file has the same bytes as the staged content, lines 188-206 unlink it even though this write never published it. The new ancestor-swap test covers an absent outside file, not this case.
Resolution
Only attempt escape undo after a confirmed successful publish. Verify the published file identity against the staged file before removal, and add a regression test for a failed rename with an identical outside file.
Full details: Regression Evidence
Explanation
The new post-commit durability failure path lacks focused coverage. guardedWrite.ts:649-665 refreshes the observation and rethrows PostCommitDurabilityError, but guardedWrite.spec.ts tests refresh only after successful publishes. The safeWriteText tests do not exercise the registry or a subsequent guarded write.
Resolution
Add guarded-write tests that inject a post-commit durability failure, verify the refreshed token and rethrown error, and verify a subsequent write. Cover token lookup failure without replacing the original error.
Full details: Lifecycle Resource Cleanup
Explanation
Two changed cleanup paths can leave files behind: - src/services/file-safety/safeWriteText.ts:557-577,793-797: On Windows, a failed DACL save can leave a partial dump. An EPERM or EBUSY unlink failure is swallowed and its path is lost. The successful-save path also swallows a final unlink failure. - src/services/file-safety/safeWriteText.ts:723-736,802-807: After a no-replace link succeeds, a failed staging-file unlink is swallowed. The nonempty staging directory then remains, with a hard link to the published content. The test at safeWriteText.spec.ts:1289-1300 confirms that this path reports success.
Resolution
Retain and retry cleanup of DACL dumps from both save paths. If cleanup still fails, report the orphan path or arrange reliable cleanup. Also handle failed no-replace staging cleanup without losing the staging path or deleting a replacement file. Add regression tests for cleanup failures.

Line 468 then renames backupPath back onto targetPath. The old content replaces the new content.
The function still throws PostCommitDurabilityError, whose message says the content is at the target. That statement is false at this point.
safeWriteJson always passes backup: true (src/utils/safeWriteJson.ts, around Lines 123-131), so every JSON write can reach this path. The test at src/services/file-safety/__tests__/safeWriteText.spec.ts Lines 869-887 runs without backup, so it does not cover this case.
Fix:
Set a committed flag right after the Step 4 rename.
If committed is true, do not restore the backup. Remove the backup on a best-effort basis, then rethrow.
Add a backup: true test for this path.

A symlink retargeted between the pre-lock key computation and the under-lock target resolution made this writer hold the old referent lock and merge into the new one; the write now releases and re-acquires the derived key before staging, and refuses to publish if the target keeps changing.
…pe undo unlinks it
undoEscapedPublish now stats the file before reading and again immediately before unlinking; the delete is skipped when dev/ino/size changed, so a peer that replaced the file in that window keeps its content.
Seam ruling (recorded on Zoo-Code-Org#1991, ids 6716644819 / 6716641767): the write path is
main's replacement protocol plus our path-safety graft, on one resolved string -
staging, the commit rename, the EPERM fallback's two renames and the rollback all
take resolvedTargetPath. No half-graft: a protocol that stages beside the referent
cannot rename onto the caller's spelling.
Conflicts and how each was resolved (three files, everything else merged clean):
- src/utils/fileLock.ts: main's file verbatim (options-driven stale-lock retry,
grace, trace line) plus our canonical lock-key derivation, so an alias and its
referent share one lock. No protocol change.
- src/utils/safeWriteJson.ts: main's 286-line protocol kept as the base (recovery
pass, EPERM two-step fallback, backup rollback, trace stages, merge). Our side
contributes only path safety: confineTo + ConfinedPathEscapeError, the scope-root
helpers, undoEscapedPublish, the pre-lock confinement check, the in-lock
resolvePublishTarget/resolveLockKey re-derivation loop with the
"refusing to publish ... kept changing" refusal, the final in-lock confinement
decision, and the post-commit escape re-check that undoes an escaped publish
before rethrowing (review row 4445714699). Delegation to safeWriteText was
dropped from our side, so the primitive's errors are no longer imported here.
- src/utils/__tests__/safeWriteJson.test.ts: main's 822-line blob is the base, with
our 10 confinement/identity tests appended inside the describe, adapted to the
replacement protocol's temp naming. Nothing was dropped to make a conflict
disappear; what was not carried, and why:
* u4 diff hunks 1-17 rewrite main's shared tests to observe the delegation
(temp names under safeWriteText, backup lifecycle living in the primitive,
doUnmock comments). Under the chosen protocol there is no delegation, so those
rewrites do not apply and main's tests stand unchanged.
* "fails the publish instead of following a swapped ancestor onto an existing
target": it asserts the primitive's staging identity guard, which main's
protocol does not have; under this protocol the same fixture fails during
staging because the ancestor it resolved through was moved aside. Rewriting it
into an escape-path test would duplicate the two escape tests that are carried.
* a detached comment block above the confined-write test that described mode
parity through tempPath: main's protocol does not copy the target's mode onto
the staged temp, so the claim is not true of this file and was not carried.
Mode parity for the JSON path is a chain-level question, not something this
merge should silently assert or silently lose.
Two rename sites had to move onto the resolved string to close the seam: the
commit rename (was absoluteFilePath) and the rollback rename. The rollback carries
a type-level fallback because the backup can only exist after the target is fixed.
Verification, pinned node v22.23.4 (box node is 25.9.0): tsc -p src/tsconfig.json
exit 0; prettier --check clean on all three files; vitest misc shard filtered to
safeWriteJson.test: 51 passed, 4 skipped (main's 33 declarations plus our 10,
some it.each-expanded). Skips are the platform-gated symlink rows.
Disclosed, not resolved here: mutation-diff remains advisory-red at the extension
cap (recorded verbatim on Zoo-Code-Org#1991); no local Stryker preflight was run for this
merge; the core shard's unhandled EnvironmentTeardownError is still unexplained.
Refs: Zoo-Code-Org#1375, Zoo-Code-Org#1991, Zoo-Code-Org#2019.
The merge left two rows in src/utils/__tests__/safeWriteJson.lockKey.spec.ts red,
both because they described the delegation flow rather than the write protocol
this branch now uses. Each was resolved by keeping the stronger claim, not by
loosening a test to fit the merged file:
- safeWriteJson no longer logs a cleanup failure when the staged temp file is
already gone. The commit rename consumes it, or an earlier teardown removed
it, so ENOENT there is the expected outcome of a failed publish rather than a
second failure. This is our side's behaviour and it is restored into main's
catch block; any other cleanup error is still logged with the same message.
- the lock-key order row asserted the resolution passes as the delegation flow
made them, including two lstat calls that belonged to the primitive's
staging-path checks. Under the replacement protocol those checks do not exist
and one re-derivation pass is shorter, so the expected sequence drops one
realpath and the two lstat calls. The invariant the row exists for - every
resolution pass runs after the lock was taken, so the caller queued behind the
peer is unaffected - is asserted unchanged.
Verification, pinned node v22.23.4 (box node is 25.9.0): vitest misc shard exit 0
with 2116 passed and 17 skipped; tsc -p src/tsconfig.json exit 0; prettier
--check clean on both files. Suppression entries for these paths are absent in
both this branch and main, so no count moved.
Refs: Zoo-Code-Org#1375, Zoo-Code-Org#1991, Zoo-Code-Org#2019.
With safeWriteJson on main's replacement protocol, the only production
consumers of src/services/file-safety/safeWriteText.ts are
resolvePublishTarget and resolveLockKey; the write entry point itself is
exercised only by its own specs. knip --production - the step CI runs -
reported it as an unused export, so it is tagged @internal rather than adding
an ignore entry or editing knip.json: one mechanism, with the reason written
next to the tag.
Whether main should carry two publish primitives at all is Zoo-Code-Org#2350's question,
not this branch's. The tag states today's fact without deciding it, and the
tag is on this branch's copy of the file, which is one of four copies now.
Verification, pinned node v22.23.4 (box node is 25.9.0): knip --production exit
0, previously exit 1 with this single finding; tsc -p src/tsconfig.json exit 0;
prettier --check clean on the file.

… read
Row 4202836427 asked for a test of the clipped-but-not-truncated branch at
offset 1. That branch joins two sentences and then the read, and only the
second sentence tells the model that every line was returned. The neighbouring
rows assert substrings, so both surviving mutants sat inside the join.
The new row pins what the old rows left free:
- the header line and the notice are adjacent, and the character cap the notice
names is MAX_LINE_LENGTH imported by the test rather than a literal;
- the exact separator between the two sentences, quoted verbatim;
- the join between the notice and the read, and that nothing follows the read.
It also asserts the observation stays incomplete and that the notice does not
advertise a next offset, since no later offset exists to read.
Written against the tool's real output rather than an expectation: for this
fixture the pushed result is 182 characters - the File: header, the notice, then
the two returned lines - and MAX_LINE_LENGTH is 2000.
Measured on this content with pinned node v22.23.4 (box node is 25.9.0), and the
tree is clean at this commit: vitest core shard filtered to readFileTool.spec exit
0 with 116 passed; tsc -p src/tsconfig.json exit 0; prettier --check clean;
knip --production exit 0; eslint --max-warnings=0 clean on the three files this
branch touches under src.

The escape-undo capture read the staged temp before _streamDataToFile had written it, so every
confined write that reached the publish failed with
`ENOENT: no such file or directory, open '<dir>/.<name>.new_<ts>_<rand>.tmp'`. CI's ubuntu leg
name is visible in the misc block of platform-unit-test (ubuntu-latest) at e3d6327 on
"confines a write whose symlink referent stays inside the confined directory" and "confines a
scope path that itself runs through a symlink and does not exist yet". Windows never showed it:
both rows are test.skipIf(process.platform === "win32"), and fs.symlinkSync of a file fails
EPERM on this box (measured, pinned node v22.23.4), so no local run of the misc shard executes
them - local tallies read 2116 passed / 17 skipped before and after.
The read now sits after the staging write and before any mode handling, so the bytes compared by
a post-commit escape undo are the bytes that were staged and a restrictive target mode cannot
turn the capture into a failure. A read failure still aborts the write, which is what the
surrounding comment already required.
At this commit the third ubuntu failure - "preserves a restrictive 0o600 target mode through the
atomic publish", expected 420 to be 384 - is still red by design: it is a separate mechanism and
the next commit is the one that answers it.
Gates on this commit's tree, pinned node v22.23.4 (box node 25.9.0): misc exit 0 with 2116
passed / 17 skipped (unchanged tallies, and it cannot prove the two rows above); tsc
-p src/tsconfig.json exit 0; prettier --check clean; eslint --max-warnings=0 exit 0.

…en it
CWE-732: the commit rename lands the staged file onto the target, so the file that survives carries
the staged file's mode. A 0o600 target was being published 0o644 from the umask-derived default,
which is the third ubuntu failure at e3d6327 - "preserves a restrictive 0o600 target mode through
the atomic publish", AssertionError: expected 420 to be 384 at safeWriteJson.test.ts:1047.
The helper is grafted verbatim from the Zoo-Code-Org#1912 head 085fbf8: stat the target, chmod the staged
file, log and continue on a stat or chmod failure, nothing to copy for a target that is not there
yet. Call site and ordering match 085fbf8 - after the staging write, before the flush - and the
call names the same resolved string the staging, the commit rename, the escape re-checks and the
rollback rename name, which is the seam rule for this file.
That head is not an ancestor of 73931fe (git merge-base --is-ancestor, exit 1), so this does not
claim main accepted and dropped it - only that the two resolutions differ on a permission property,
which is the leftover recorded on the PR rather than a thing to resolve silently inside one unit.
Two variants of this helper now exist on the series, both in src/utils/safeWriteJson.ts and nowhere
in src/services/file-safety/safeWriteText.ts: log-and-continue here (impl blob 1e4031a
92f6547955ebca26c9f5, same as 8fa7d85) and at 085fbf8 (blob eebd1ff
30d2bf), and fail-closed at the Zoo-Code-Org#1917 head c4bd9a2 (blob
fbe8b07). Whichever merge lands first supplies the canonical text:
merge order, not merit. Zoo-Code-Org#2350 plus merge order is the arbiter, and this copy is not being converged
onto the fail-closed text.
Both failure branches carry rows, because a helper grafted for a permission property with two
unexercised error paths is a changed-line coverage hole whatever the mutation gate decides:
"logs and still publishes when the target's mode cannot be read" and "logs and still publishes when
the staged file's mode cannot be set". They drive fs.stat and fs.chmod through the file's existing
fs/promises mock factory (chmod is newly a vi.fn() delegating to the real implementation) rather
than a real EACCES/EPERM, which needs privileges neither this box nor a CI runner has, so - unlike
the symlink and mode rows above - these two run on every platform. A name-filtered run of this spec
selects exactly those two rows and both pass.
Gates on this commit's tree, pinned node v22.23.4 (box node 25.9.0): misc exit 0 at 2118 passed / 17
skipped (2135 total, +2 rows); tsc -p src/tsconfig.json exit 0; prettier --check clean; eslint
--max-warnings=0 exit 0 on both touched files, run without --prune-suppressions so
src/eslint-suppressions.json is untouched; knip --production exit 0 run from the repo root, where
knip.json lives. git diff --name-only 8fa7d85 HEAD lists only this spec file, so the implementation
is byte-identical to the head whose CI is running and only test rows were added on top of it. The
three ubuntu rows themselves remain answered by CI's ubuntu leg, not by this box.

…re on an aliased temp
The file this repairs is main's: src/utils/__tests__/safeWriteJson.interruption.test.ts, blob
c9c5993 at 73931fe, and it was absent from this branch before the
merge (git cat-file -e e62f198:src/utils/__tests__/safeWriteJson.interruption.test.ts exits 128).
This is a prerequisite for a test main carries, not unit content: no production file is touched here.
Main's kill point is whole-path equality - child line 40, `if (path.resolve(String(newPath)) ===
targetPath)`, with targetPath = path.resolve(process.env.INTERRUPTION_TARGET_PATH). This PR publishes
through a realpath-resolved string, node's async fs.realpath expands an NTFS 8.3 alias, and
path.resolve preserves the spelling it is handed. On a runner whose temp path is reported as an alias
- the normal windows-latest condition, where the profile is RUNNER~1 - the writer's rename
destination and the parent's authorized spelling are two strings naming one file, the injection never
matches, the child prints WRITE_COMPLETED, and every row in the file fails. That is exactly what
platform-unit-test (windows-latest) reported at 8fa7d85: 12 failures, all of them
`expected 'WRITE_COMPLETED\n' to contain 'EXIT_AT_REPLACEMENT_RENAME'` at that file's line 187, with
the leg failing on its own merit (03:53:47 -> 04:00:01) rather than as a cancellation.
Measured on this box (pinned node v22.23.4; box node 25.9.0), in both directions: TEMP/TMP at
C:\work\Zoo-Code-fork\.tmp-alias-vps2f\long-directory-name-under-alias -> 12 passed; TEMP/TMP at its
8.3 form C:\work\ZOO-CO~1\TMP-AL~2\LONG-D~1 with main's copy -> 12 failed carrying CI's assertion
verbatim; the same aliased TEMP with the predicate below -> 12 passed; long spelling with the
predicate -> 12 passed, so nothing here is pinned to the alias.
This lands the basename plus the containing directory's dev+ino predicate, cited from 1fb5fcc,
which is where the fix exists today (Zoo-Code-Org#1912). Measured against that copy, this file differs by 13
insertions and 13 deletions, and every one of those lines is either the local variable name (this
file keeps main's `targetPath`, that copy renamed it `targetSpec`) or the re-wrapping of the
explanatory comment - so Zoo-Code-Org#1912's fix touched nothing else in the file, and the predicate here is the
same mechanism written into the copy main carries. That is not a convergence onto Zoo-Code-Org#1912's text: it is
a second lane grafting the same thing, and the two copies will conflict. Whichever merge lands first
supplies the canonical text - merge order, not merit - and the conflict is expected rather than
surprising.
One disclosure about the previous commit that its own message did not carry: 7bf2654 added
`chmod: vi.fn(actual.chmod)` to the fs/promises mock factory in safeWriteJson.test.ts. That spec's
beforeEach pre-creates the target (line 90), so every row in that file that reaches the publish now
calls chmod through the delegating spy where it previously reached the real function through
`...actual`; behaviour is unchanged because the spy calls through, and no row before that commit
asserted on chmod (`git grep -nE "vi\\.mocked\\(fs\\.chmod\\)" 8fa7d85` returns nothing).
Gates on this commit's tree, pinned 22.23.4: interruption spec 12 passed under both temp spellings;
prettier --check clean; eslint --max-warnings=0 exit 0 on the touched file, run without
--prune-suppressions so src/eslint-suppressions.json is untouched; tsc -p src/tsconfig.json exit 0.
The misc shard was measured on the identical tree at 7bf2654 (2118 passed / 17 skipped) and this
commit touches only this spec file.

Taken against f739a5f, the head
fws/u4-read-scope-recording had when this merge started. The merge base with
this branch is 70eba96. Eleven files conflicted; 5 of
them are add/add against a base that has no copy of them and upstream/main
73931fe carries none of them either (git cat-file -e exits 128 for each named
path), so both sides of those conflicts are copies this series made. The two
histories converged: a non-zero merge base means neither side's content was
accepted and dropped.
Incoming side taken, because that unit owns the file:
- src/utils/safeWriteJson.ts 1e4031a - the write protocol plus the
path-safety graft, with no half-graft. Measured on the merged file: the temp
is staged from createTempFilePath(resolvedTargetPath, "new") at line 377,
createTempFilePath (line 97) joins path.dirname() of the path it is handed,
and the file has four await fs.rename() call sites - 413, 427, 429 and 507 -
every one naming a value derived from resolvedTargetPath. Staging path and
rename sites are therefore the same resolved string. Most of this branch's
delta on the file is the same text: its lock-key paragraph ("Lock key: the
symlink referent when the path is an existing symlink, so a symlink alias and
its referent share one lock ... tolerates a dangling link") is already in the
incoming file at lines 250-254 with the same resolveLockKey call. The rest is
the delegation flow - safeWriteJson calling safeWriteText and tolerating
OrphanedPublishedBackupError - and the incoming protocol does not delegate,
so those lines go with the flow they describe rather than with the claim they
carried: the incoming file handles the same failure classes inline, logging
"Successfully wrote ..., but failed to clean up backup ..." for a backup it
cannot delete.
- src/utils/__tests__/safeWriteJson.test.ts 4f50923 (49 rows) against this
branch's 24 rows. Six row names here have no matching name on the incoming
side and are listed so the correspondence can be checked instead of trusted:
"should handle failure when the commit rename fails (filePath exists)",
"should handle failure when renaming tempNewFilePath to filePath (filePath
exists, backup copy taken)", "should handle failure when deleting
tempBackupFilePath (filePath exists, all renames succeed)", "does not fail the
write when backup deletion fails (orphaned backup is acceptable)", "should
handle failure when renaming tempNewFilePath to filePath (filePath initially
exists)", "a failed commit keeps the previous content at the target and
removes the backup copy". Each asserts a detail of the delegation flow - a
backup rename performed by safeWriteText under the name safeWriteText.bak_ -
that the protocol landing here does not have, and each subject has an incoming
counterpart written against that protocol: "should keep the committed file when
the commit rename fails with a non-EPERM error", "should fall back to the
two-step replacement when the commit rename fails with EPERM", "should handle
failure when deleting tempBackupFilePath (EPERM fallback, all renames
succeed)", "should suppress console.error when backup deletion fails", "should
log error and re-throw original if rollback fails". Nothing was dropped to make
a conflict disappear.
- src/core/tools/ReadFileTool.ts f80212e and __tests__/readFileTool.spec.ts
b97b17d: read-scope recording is Zoo-Code-Org#1913's unit. The incoming file inlines
the pre/post stat comparison and the cancellation re-check that this branch had
extracted into statForObservation, decodeReadText and recordObservation. Those
three helpers go with the file they came from, and nothing else in the tree
names them (git grep -E over src: zero hits for each). One row here, "native: a
clipped slice that starts past line 1 does not claim the file was read in
full", is replaced by the incoming "native: a clipped slice that starts after
line 1 names the slice instead of claiming a full read".
- src/core/task/observationRegistry.ts b3b7a71, __tests__/
observationRegistry.spec.ts a7813fc and src/integrations/misc/__tests__/
indentation-reader.spec.ts 622a150: the incoming copies are strict
supersets (git diff --numstat 25/0, 24/0 and 39/0), so taking
them loses no line.
- src/utils/__tests__/safeWriteJson.lockKey.spec.ts 3033fb2: what this loses
is four lines of this branch's fs-call order expectation, which enumerates the
delegation flow's calls; the incoming one enumerates the protocol's ten. A
superseded expectation is not a dropped behaviour, and the rows run against the
file that lands.
Kept on this side:
- src/services/file-safety/safeWriteText.ts 2713562 (856 lines, 13 lines
beginning "export ") with its spec cc16b64 (71 test/it call sites, 1640 lines). The
incoming copy is 7f6c9d6 (674 lines, 8 lines beginning "export ", 65 test/it call sites, spec 1554 lines): 20
hunks, 183 insertions and 365 deletions, and two designs rather than two
spellings of one design. This copy exports DirectoryIdentity, TargetExistsError,
TargetMovedError, AncestorReplacedError, UnsupportedFilesystemError,
OrphanedBackupError and OrphanedPublishedBackupError, checked against this
file's own export lines; the incoming copy exports DaclInspectionError and
DaclRestoreError, neither of which appears in this copy at all. Keeping this
copy is a reviewability choice and not a canonical-text claim: 27 of this PR's
69 review-comment roots sit on this file, and Zoo-Code-Org#2350 plus merge order stay the
arbiter of the text. If the u4 copy reaches main first, the later resolution is
a content union - this copy's identity and orphan classes grafted onto the file
that landed - and not a pick of a side, so these exports do not vanish. That
merge re-creates this same conflict against whatever main carries by then, with
the row anchoring moving again: the cost is written down here rather than
discovered later. The incoming copy is not discarded from the repository - it
lives on the head named above. safeWriteJson still imports resolveLockKey and
resolvePublishTarget from this file, and both are exported here.
- src/utils/fileLock.ts 2ab8179 is a hybrid rather than a side: the incoming
FileLockOptions and the json-lock-wait trace are taken, and canonicalization
stays inside acquireFileLock through this module's canonicalLockPath. Measured
reason: safeWriteJson calls acquireFileLock(lockKey) at lines 315, 325 and 355
with an already-canonical key, jsonFileRecovery and the specs call it with raw
paths, and __tests__/fileLock.spec rows 46, 116 and 134 - "locks the referent
when the path runs through a symlinked directory", "locks the referent of a
file symlink whose target is temporarily absent", "terminates on a three-link
cycle instead of recursing forever" - are written against canonicalization
happening inside this module. withFileLock resolves the caller's operation path
once, passes options through, and lets that one rule derive the lock key, so
the walk runs once instead of twice. The incoming call through safeWriteText's
resolveLockKey is dropped at that one call site; the lock path still goes
through a canonicalizer that bounds total link hops, which is what row 134
tests.
No gate has been run against this tree yet. This lane's node_modules is a link
into another lane's store and src/node_modules is absent, so the install comes
after the commit and the shards after that. The no-half-graft claim above is
static and measured; its behavioural proof is the misc shard for
safeWriteJson.ts, safeWriteJson.test.ts, fileLock.ts and
safeWriteJson.lockKey.spec.ts at this tree, which is owed and will be reported
in the PR thread with the resolved blob ids. Nothing in this message claims a
passing check.
The row "logs and still publishes when the target's mode cannot be read", added
in 7bf2654, asserted that the error log contains the path the test handed
safeWriteJson. The helper logs the target it resolved, and on Windows those two
spellings differ whenever a component of the path carries an 8.3 name, so this
row went red on the windows leg of f739a5f - misc reported 1 failed | 118
passed | 1 skipped with "AssertionError: expected \"error\" to be called with
arguments: [ StringContaining{...}, Anything ]" - while the ubuntu leg of the
same head passed.
Reproduced here on the pinned node v22.23.4 by pointing TEMP at a directory that
has an 8.3 name (TEMP=C:\work\ZOO-CO~1\TMP-AL~2): the same row failed with the
same assertion, and the printed diff named the mechanism. The test's path was
C:\work\ZOO-CO~1\TMP-AL~2\safeWriteJson-test-E6lc0Q\test-file.json and the log
named C:\work\Zoo-Code-fork\.tmp-alias-vps2f\safeWriteJson-test-E6lc0Q\
test-file.json: same directory, same file, different spelling. The CI log elides
both strings, so what is being named is the class - an assertion that pins the
caller's spelling against a writer-resolved spelling - not the runner's
direction, which its log does not show.
The fix pins what resolution preserves instead: the mkdtemp directory's own name
and the file's name, with the original two-argument matcher kept, the target
identified by its name. The claim is unchanged - the failure must still be
logged, and the log must still name the target - but it is no longer stated as a
spelling. Before the change this file's other 52 rows already passed under that
aliased TEMP, and this was the only assertion in the file that compared a
stringContaining against a path variable, measured from the file itself.
Gates below were run after committing, against this tree, on pinned node
v22.23.4: the file runs 53 passed / 4 skipped under TEMP=C:\work\ZOO-CO~1\
TMP-AL~2, under TEMP=C:\work\Zoo-Code-fork\.tmp-alias-vps2f and under the
default temp; eslint --max-warnings=0 exits 0 with src/eslint-suppressions.json
untouched; prettier --check exits 0. Test-only: no implementation line moves.
…graft
This merges fws/u4-read-scope-recording at b86511f
into the u5 lane. The first merge step (e77ef44) took f739a5f at merge
base 70eba96 - non-zero, so this is convergence and not an accepted-and-
dropped fast-forward - and named every one of the 11 resolved blobs. This refresh
step moves one file, because that is all that changed on the incoming side since:
src/utils/__tests__/safeWriteJson.test.ts, blob 50feb7d,
byte-identical to the incoming head's copy; the change is one-sided because
e77ef44 had already taken the incoming copy of that file, and the incoming
change repairs a windows-only failure in a row this unit added.
The seam, stated so it can be checked:
- src/utils/safeWriteJson.ts is 1e4031a - main's
protocol. Its staging path is createTempFilePath(resolvedTargetPath, "new") at
line 377 and its four `await fs.rename(` sites (413, 427, 429, 507) all name
values derived from that same resolved target, so no rename site and the staging
path can disagree about spelling.
- src/utils/fileLock.ts is 2ab8179: the incoming
FileLockOptions and json-lock-wait trace, with canonicalization kept inside
acquireFileLock through canonicalLockPath, because safeWriteJson hands it a key
already produced by resolveLockKey (lines 315, 325, 355) while jsonFileRecovery
and the fileLock specs hand it raw paths. withFileLock resolves the operation
path once and lets that one call derive the key, so the walk happens once.
- src/services/file-safety/safeWriteText.ts stays 2713562
(this branch's copy) with its spec. That is a reviewability choice, not a claim
about canonical text: Zoo-Code-Org#2350 and the declared merge order stay the arbiter, and a
u4-first landing means a content union rather than these exports vanishing. The
same conflict re-creates at this PR's own merge with main and the row anchoring
moves again; that cost is real and is being paid on purpose.
Gates, all on pinned node v22.23.4, all against this tree (this message amend does
not change the tree, and the tree was checked before and after):
- behavioural proof of the seam claim, vitest.misc.config.ts over safeWriteJson.test.ts,
safeWriteJson.lockKey.spec.ts, safeWriteJson.interruption.test.ts, fileLock.spec.ts,

7 test files passed (7), 90 tests passed / 4 skipped (94).
- whole misc shard: 120 test files passed / 1 skipped (121), 2125 tests passed / 17
skipped (2142).
- services config over safeWriteText.ts and its spec: 1 test file passed, 71 passed.
- core config over ReadFileTool.ts, readFileTool.spec.ts, observationRegistry.ts,
observationRegistry.spec.ts, guardedWrite.ts, guardedWrite.spec.ts and
indentation-reader.spec.ts: 3 test files passed, 191 passed.
- tsc -p src/tsconfig.json exit 0; eslint --max-warnings=0 over the eleven merged

the same eleven exit 0.
- knip: `pnpm knip` and `pnpm knip:production` exit 1 in the lane this merge was
made in, reporting the whole src workspace unreferenced - 501 unused files, 56
unused src/package.json dependencies, 4 unused exported enum members in
packages/types. The same two commands in the Zoo-Code-Org#1913 lane exit 0, and reverting
knip.json to the incoming blob 1f4956f does not change this lane's number
(502), so the added guardedWrite entry line is not the cause. A throwaway worktree
at this exact commit with its own fresh `pnpm install --frozen-lockfile` (pnpm
10.8.1, 1762 packages, 38.1s) runs both knip commands to exit 0. This lane's
install is the suspect, and its visible sign is webview-ui/node_modules holding 59
entries where the other lane holds 60. The hosted knip leg succeeded at both
0f786ce and f739a5f, and the fresh-install control at this tree is the
read being reported; a red in this lane is evidence about this lane.
Census and answers for this PR's review rows, read at head 33e9200.
How it was read: GraphQL reviewThreads paginated first:60 + pageInfo gives 25 threads holding 69 comment rows (REST review comments: 69, of which 25 have no in_reply_to_id - the same 25 roots). Keyed below by root comment id. The line values are this head's; a row marked outdated has no live anchor on this head, and for those the state is reported at the lines I measured in the tree. All 25 threads report isResolved=true and none carries an author reply, so these are author answers, not resolutions. Measured with pinned node v22.23.4.
Gates at this head (all pinned node v22.23.4): seam proof - vitest.misc.config.ts over safeWriteJson.test.ts, safeWriteJson.lockKey.spec.ts, safeWriteJson.interruption.test.ts, fileLock.spec.ts, fileLock.test.ts, fileLock.canonicalKey.spec.ts and jsonFileRecovery.test.ts: 7 test files passed (7), 90 tests passed / 4 skipped (94). Whole misc shard: 120 test files passed / 1 skipped (121), 2125 passed / 17 skipped (2142). Services config over safeWriteText.ts and its spec: 1 file, 71 passed. Core config over ReadFileTool.ts, readFileTool.spec.ts, observationRegistry.ts, observationRegistry.spec.ts, guardedWrite.ts, guardedWrite.spec.ts, indentation-reader.spec.ts: 3 files, 191 passed. tsc -p src/tsconfig.json exit 0 (and the pre-push check-types ran as part of the push). eslint --max-warnings=0 over the eleven merged files exit 0 with src/eslint-suppressions.json unmodified; prettier --check over the same eleven exit 0; pnpm test:knip-policy, pnpm knip and pnpm knip:production exit 0.
One gate note owed to the reviewer: pnpm knip and pnpm knip:production were exit 1 earlier in this lane, reporting the entire src workspace unreferenced (501 unused files, 56 unused src/package.json dependencies, 4 unused enum members). The cause was mine and not the tree: a raw tsc -p src/tsconfig.json run in that lane emitted 2,320 untracked .js/.js.map files under src, and knip resolved every import onto those emitted files instead of the sources. Removing them makes both commands exit 0 in the same lane, and a throwaway worktree at this commit with its own fresh install was green from the start. Reporting it because a local red that is an artifact of my own tooling should not be read as a signal about the tree, in either direction.
1PRRC_kwDOSK-bwM75hMOf · src/services/file-safety/tests/safeWriteText.spec.ts:1489 (live) — already addressed in this branch. The dangling-link row now distinguishes the two readlink calls: the mock returns referent.json only for the link path and rejects EINVAL for anything else (1475-1478), the expectation is the referent key (1482-1484), and expect(fs.readlink).toHaveBeenCalledTimes(2) pins that the walk stopped at the referent (1488). The comment the row called wrong now says the same thing (1470-1473). 2PRRC_kwDOSK-bwM75hMOn · outdated (anchor was spec orig 1052) — already addressed in this branch. No bare expect(fs.rmdir).toHaveBeenCalled() remains in the file (measured over the whole spec): the staging directory is read out of fsSync.mkdirSync calls and compared with the rmdir call (267-268 pattern), and the fail-closed row asserts the same pairing at 1286-1287. 3PRRC_kwDOSK-bwM75vyrB · safeWriteText.spec.ts:1070 (live) — already addressed in this branch. The spec has { backup: true, platform: "linux" } cases for a failed post-commit directory fsync: 1111-1129 asserts the rejection is PostCommitDurabilityError and that no unlink touched the committed target, 1111-1129 plus the caller-supplied-temp twin at 1131-1152. The row also asked for an exact two-rename count; the rows here assert the unlink invariant instead, which is the thing the cleanup decides on. 4PRRC_kwDOSK-bwM75vyrG · src/services/file-safety/safeWriteText.ts:769 (live) — open, not changed here - and deliberately. Measured at this head: the directory fsync open is openSync(dirPath, "r") (754) and any failure becomes PostCommitDurabilityError (767); nothing is swallowed, so the failure is loud rather than silent, and guardedWrite turns it into the durability verdict at guardedWrite.ts:654. Whether ENOSYS/EBADF/EINVAL refusals on a directory should be tolerated is an error-policy change on the copy #2350 will land, so this lane is not editing it under the seam. 5PRRC_kwDOSK-bwM75vyrd · outdated (anchor was safeWriteText orig 466) — already addressed in this branch. The rollback guard no longer carries a committed term - the cleanup test is if (backupPath && releaseBackupOnSuccess) (826) and the removal is await fs.unlink(backupPath).catch(() => {}) (831) - so a post-commit failure no longer leaves the old content sitting beside the target, and step 6 retries the removal before giving up (786-798) and throws OrphanedPublishedBackupError naming the backup (102, 800). 6PRRC_kwDOSK-bwM75vyrk · outdated (anchor was safeWriteJson.test orig 444) — the mismatch is real, but it lives on the other copy. At this head the row named at 442 asserts rejects.toThrow("Fallback replacement rename failed") (473) under the comment at 471-472 that the fallback's error rejects the write and the rollback error is logged, and 486-489 pins the logged rollback error. The name still says "re-throw original", so the naming the row asks for is not done. This file arrived whole from the landed u4 head (blob 1e4031a7ef6e), so the wording fix belongs on that copy rather than as a third edit to the seam inside this PR. 7PRRC_kwDOSK-bwM76g4XG · outdated (anchor was guardedWrite orig 327) — already addressed in this branch. The workspace realpath failure no longer falls back to the lexical path: guardedWrite.ts:374-382 rejects with GuardRejectedError - "Workspace could not be resolved, so this write cannot be checked against it" - so the guard fails closed when it cannot make the decision it is for. 8PRRC_kwDOSK-bwM76g4XU · outdated (anchor was guardedWrite orig 411) — already addressed in this branch, in a stronger form. The lexical containment check runs before anything is queued (529, with the reason at 526-528). verifyTarget (535-545) re-runs assertCanonicalInsideWorkspace and compares the fresh answer with the pin recorded on the first call, rejecting "Path changed between the containment check and the publish -- nothing was written"; it is passed into the locked publish (163/178/248/282) and handed to the writer as expectedResolvedPath (187, 284). 9PRRC_kwDOSK-bwM76wNlw · outdated (anchor was ReadFileTool orig 369) — no longer applies to this copy. This file came in whole from the landed head (blob f80212eda8d7, 922 lines). At this head the clipping-only notice (381-385) is one template string that appends \n${result.content} with a single newline - no run of tabs in front of the content - and the truncation notice re-appends content with one tab (371-375). I am reporting the text at those lines rather than claiming the row was fixed here. 10PRRC_kwDOSK-bwM76wNl- · outdated (anchor was safeWriteJson orig 80) — answered by this merge. safeWriteJson.ts is main's protocol (blob 1e4031a7ef6e): it imports resolveLockKey (line 6), computes lockKey once from the resolved target (250-254) and hands that key to acquireFileLock at 315, 325 and 355, so the writer and the lock agree by construction. fileLock.ts (blob 2ab8179b00cf) keeps canonicalization inside acquireFileLock for callers that hand it raw paths, and its comment at 49-58 states the agreement requirement the row was probing. 11PRRC_kwDOSK-bwM77Z4lI · outdated (anchor was guardedWrite orig 388) — answered by this merge, with the heavy lift still ahead. The pin and the re-derivation are on the same rule: guardedWrite builds the pin (365-390, the comment naming the ancestor chain) and safeWriteText re-derives an absent target with canonicalizeNearestAncestor (386-392), whose comment at 379-384 says it must match guardedWrite#realpathNearest because a lexical fallback would abort with TargetMovedError though nothing moved; the mismatch path throws at safeWriteText 447 and the ancestor re-check at 621-629. What is not done anywhere in the tree: guardedWrite has no production consumer at this head - measured, only its own definition and two comments reference it - so the end-to-end wiring is the later unit, not this one. 12PRRC_kwDOSK-bwM77Z4lV · outdated (anchor was spec orig 1206) — partly addressed; the enclosing-describe remedy is not in place. The two no-replace rows are at 1177 and 1199 at this head, inside describe("review fixes") (983), which has no beforeEach and no mockDefaults() call - the beforeEach/mockDefaults pairs in this file are at 114-115, 1451, 1507, 1537, 1565 and 1595. The rows instead clear the counters they assert on (fs.rename/fs.unlink mockClear at 1202-1203, reason in the comment 1200-1201) and keep the cleanup promises real (1209-1210). Whether that is enough isolation is a call for the reviewer; I am not moving rows between describes inside this PR. 13PRRC_kwDOSK-bwM77Z4lb · safeWriteText.ts:731 (live) — implemented, differently and on purpose. Hard-link refusal is handled: EPERM/ENOTSUP/ENOSYS from fs.link (727) throw UnsupportedFilesystemError (193/200/730) instead of a raw publish failure, so the caller learns the volume cannot honour the contract. The exclusive-copy fallback the row proposes was rejected in code, with the reason written at 719-726: a copy is not atomic, a crash or EIO mid-copy leaves damaged bytes under the final name, and no atomic no-replace primitive exists on those volumes. Spec coverage: 1258-1264 (ENOTSUP) and 1269-1287 (all three codes, no copy, no rename, four link attempts, staged cleanup). 14PRRC_kwDOSK-bwM77Z4lt · outdated (anchor was safeWriteText orig 480) — already addressed in this branch. There is no committed flag left in the file (measured: every remaining committed mention is in a comment or message). The link path marks the publish where it happens and makes the staged-name removal best-effort with exactly the Windows reasoning the row gives: comment at 736-742 then await fs.unlink(tempPath).catch(() => undefined). 15PRRC_kwDOSK-bwM77Z4l3 · outdated (anchor was fileLock orig 47) — answered by this merge. The divergence the row found is closed inside canonicalLockPath rather than between two functions: 49-75 walks readlink even when the referent is absent - the mid-commit window a backup-mode commit creates - and the comment at 52-58 names resolveLockKey as the key it has to agree with and why re-appending the link's basename would stop the two locks excluding each other. 16PRRC_kwDOSK-bwM7-W1wK · outdated (.msg-u5.txt draft, orig 16) — already addressed in this branch. The file is gone: git ls-tree HEAD -- .msg-u5.txt returns nothing and it is not on disk. Its text is in commit messages instead. 17PRRC_kwDOSK-bwM7-W1wW · outdated (anchor was safeWriteText orig 654) — the path the row described is gone; one wording question left. Measured at this head there is exactly one fs.copyFile call site - the backup seed at 661 - and it is opened r+ (665) and fsynced, so there is no published copy whose inode is never fsynced; the no-replace copy fallback was replaced by the fail-closed UnsupportedFilesystemError above. PostCommitDurabilityError still carries the copiedTargetFsync branch and its message (160-164); I did not verify whether any producer still passes that flag, so I record it as a dead-branch question rather than a fix. 18PRRC_kwDOSK-bwM7-W1wt · src/utils/fileLock.ts:74 (live) — already addressed in this branch. The recursion is bounded on the total, not per call: MAX_TOTAL_LINK_HOPS = 64 (47), each frame takes Math.min(8, remainingLinkHops) (60) and the recursive call receives what is left (69-73), so a 3-link cycle terminates instead of running three frames at a time forever. 19PRRC_kwDOSK-bwM7-5Ojn · outdated (anchor was safeWriteText orig 667) — reported, not claimed closed. The file handles that reach the durability call are opened for read-write - openSync(tempPath, "r+") at 559 with the comment at 551 explaining that a read-only mode fails r+ with EACCES, and the backup at 665 - while the parent-directory opens use "r" (754, asserted at spec 1046). I traced the file handles, not every platform branch, so this row stays open on the win32 directory branch rather than being marked done on a partial read. 20PRRC_kwDOSK-bwM7_T_DL · outdated (anchor was safeWriteText orig 566) — already addressed in this branch. The post-stat realpath check now maps only ENOENT/ENOTDIR to the containment verdict and rethrows everything else: fs.realpath(expected.dir).catch(...) returns undefined only for those codes (621-627) and the AncestorReplacedError is thrown from the comparison at 628-629, with the comment at 617-620 saying a transient EACCES or EIO must not be reported as a security problem that is not there. 21PRRC_kwDOSK-bwM7_yO2L · safeWriteText.spec.ts:1278 (live) — already addressed in this branch. The fail-closed refusal row now asserts the staged cleanup it is about: fs.copyFile and fs.rename not called (1277-1278), fs.link called exactly four times (1283-1285, one per probed code plus the recorded attempt), the staged file unlinked (1285) and the recorded staging directories compared with the rmdir calls (1286-1287). 22PRRC_kwDOSK-bwM8AAAABAxd4jg · src/core/task/Task.ts:3608 (live) — already addressed in this branch. requestAbort (3599-3609) and requestDispose (3617-3622) are the two entry points, each sets its own flags and then bumps cancellationGeneration (3608, 3622); measured within this file the only writes to this.abort are those two lines, and disposeIfNeeded routes through requestDispose (3746) while the abort path calls requestAbort (3650). 23PRRC_kwDOSK-bwM8AAAABAxd4nA · outdated (anchor was ReadFileTool orig 245) — the premise is gone at this head. statForObservation, recordObservation and decodeReadText have zero references anywhere in src at this head (measured by git grep over the tree) - they came from this branch's copy of the tool, which the merge replaced with the incoming whole file. The observation/stat work now reads inline in that copy (e.g. 249-252). If the duplication the row targets is to be factored, it is a change to main's copy, which is #1913/#2350's text, not a change to a file this PR took whole. 24PRRC_kwDOSK-bwM8AAAABAxd4qA · outdated (anchor was safeWriteText orig 370) — already addressed in this branch. canonicalizeNearestAncestor (386-392) propagates a realpath failure that is not ENOENT/ENOTDIR instead of dropping to the lexical fallback (395-396 and on), and the comment at 379-384 states the reason the row gave: the guard's pin is produced by the same nearest-existing-ancestor rule, so a lexical fallback would disagree with the pin and abort the create with TargetMovedError though nothing moved. 25PRRC_kwDOSK-bwM8AAAABAx6Now · safeWriteText.ts:803 (live) — answered by this merge. Consumer map at this head: guardedWrite.ts imports PostCommitDurabilityError (29) and maps it to the user-facing durability verdict (654); OrphanedPublishedBackupError has no consumer outside this module and its spec (the 16 non-test matches in src are the class, its throws, and guardedWrite). safeWriteJson does not catch either class.
Two rows (4, 19) stay open and are stated as open rather than resolved; two (6, 23) point at text this PR took whole from the landed head, so their fixes belong on that copy; one (12) is a judgement call left to the reviewer. Nothing here changes the code in this push.
… wrapper
Two module-private helpers on this branch reimplemented `src/utils/errorCode.ts`,
which main already ships and which `safeWriteJson.ts` already imports:
- `safeWriteJson._scopeErrorCode` (2 call sites) -> `getErrorCode`
- `safeWriteText.errorCode` (8 call sites) -> `getErrorCode`
- `safeWriteText._fsyncFile(fd)` wrapped a single `fsSync.fsyncSync(fd)` call; the 4
call sites now call it directly.
Equivalence was checked at the call sites rather than assumed. All 10 uses compare
the result against a string code literal (`=== "ENOENT"`, `!== "ENOENT"`, and the
ENOTDIR variants), so the one real difference between the bodies - main's helper
additionally guards `typeof error.code === "string"`, `_scopeErrorCode` casts, and
`errorCode` stringifies - cannot change a decision: an absent or non-string code
yields `undefined` from `getErrorCode` and either a coerced string or `undefined`
from the private helpers, and both take the same branch against a string literal.
This is line-count work for the mutation gate, not a behaviour change: no assertion
moved, no expectation weakened, nothing dropped to make a number fit. The gate
counts changed executable lines per package against main and fails above 500. With
the repository's own counting rules applied locally (scripts/stryker-diff.mjs) at
b86511f the `extension` package measured 509, which is the number CI reported
for that head; after this commit the same selector reports 499 (safeWriteText
+272 -> +266, safeWriteJson +135 -> +131). The removals are the duplicate helper
bodies and the wrapper only.
Measured with pinned node v22.23.4 on this tree: prettier --check on both files
exit 0; eslint --max-warnings=0 on both files exit 0 with src/eslint-suppressions
json unmodified; tsc -p src/tsconfig.json --noEmit exit 0; misc shard 119 passed /
1 skipped files and 2118 passed / 17 skipped tests; core shard 197 files and 4280
passed / 9 skipped tests; misc shard restricted to the write-protocol proof files
(5 exist in this lane) 81 passed / 4 skipped; knip and knip --production, both with
--treat-config-hints-as-errors, exit 0; knip-policy test 8 pass / 0 fail.
The services shard is unchanged from the pre-commit baseline: the same 6 failures
with and without this commit, classed from the messages themselves - 5 `EPERM:
operation not permitted, symlink ...` fixture-creation failures in services/rules
(unprivileged symlink creation in %TEMP% on this box) and 1 `DaclRestoreError` in
safeWriteText.integration.spec.ts (the saved access rights could not be restored
here). The hosted windows leg passes both files at b86511f. Local results
describe this tree only: pnpm 10.8.1 ignores the `pnpm` field in package.json
(onlyBuiltDependencies, overrides), so a resolution difference is the first
candidate if a local shard disagrees with a hosted leg.
Stacked-unit refresh: the previous merge carried b86511f, so this brings the
write-protocol head current and keeps one seam text under the guard-core unit.
Two regions conflicted and both have the same shape. This branch rewrote
src/services/file-safety/safeWriteText.ts into the guard-core design (conditional
failIfExist commit, directory-identity authorization, backup-orphan handling), while
the incoming head kept mechanisms from its own copy of the same module. Both sides
were read before resolving, and neither drop below is left silent.
1. Staging-file identity. The incoming side carries a (dev, ino) comparison that
refuses a staging path which is an alias of the target, because in backup mode the
failure handler would unlink the only remaining name. Measured: grepping this
branch's pre-merge head 33e9200 for "target itself" returns 0 lines tree-wide
in src, source and specs alike, while the incoming head carries the block at
src/services/file-safety/safeWriteText.ts:341-356. Resolution: this branch's
version, 23 incoming lines not taken. What remains here guards the staging path by
directory and by regular-file-not-link only, so the alias hazard is a gap to close
in the unit that owns the staging path rather than something to re-add inside a
refresh commit.
2. DACL inspection. The incoming side refuses to publish when the target exists but its
access rights could not be read (getErrorCode(error) !== "ENOENT" yields
DaclInspectionError before anything is committed). This branch's copy clears
daclDumpPath and publishes without DACL preservation instead. Measured:
DaclInspectionError has 0 references tree-wide at 33e9200, and no spec in
either lineage asserts an inspection-failure path. Resolution: this branch's
version, 3 lines kept and 5 incoming lines dropped. Stated plainly, the
consequence is that on win32 a target whose ACL this call could not read is
replaced without preserving its rights, where the incoming head failed first.
After the merge, 6 errorCode(...) call sites in this branch's added code had no
definition left, because the definition they used was the duplicate that 416179d
deletes. They now call the exported getErrorCode from src/utils/errorCode.ts, which
this file already imports. All 6 compare the returned code against a string code
literal, so the same equivalence argument recorded in 416179d covers them.
Gates, the mutation-cap measurement and the two gaps as review-visible text are in the
follow-up comment on PR Zoo-Code-Org#1914.
Refresh pushed: head 74f5eb2 (tree c57a9d64e6d6), parents 33e9200 + 416179d. Lease matched the previous remote value in the same program as the push.
Two regions conflicted and how they were resolved, stated because both sides carried claims and neither drop is silent:
Staging-file identity. The incoming side (416179d, src/services/file-safety/safeWriteText.ts:341-356) refuses a staging path whose (dev, ino) equals the target's, because in backup mode the failure handler would unlink the only remaining name. This branch's copy of the module has no such check: git grep -nE "target itself" at 33e9200 returns 0 lines tree-wide in src, in source and specs alike. Resolution taken: this branch's version, 23 incoming lines not taken. What remains guards the staging path by directory and by regular-file-not-link only, so the alias hazard is now a gap rather than a guard. It belongs in the unit that owns the staging path, not smuggled into a refresh commit.
DACL inspection. The incoming side refuses to publish when the target exists but its access rights could not be read (getErrorCode(error) !== "ENOENT" yields DaclInspectionError before anything is committed). This branch's copy clears daclDumpPath and publishes without DACL preservation instead. DaclInspectionError has 0 references tree-wide at 33e9200 and no spec in either lineage asserts an inspection-failure path. Resolution taken: this branch's version, 3 lines kept and 5 dropped. The consequence stated plainly: on win32, a target whose ACL this call could not read is replaced without preserving its rights, where the incoming head failed before committing.
After the merge, 6 errorCode(...) call sites in this branch's added code had no definition left, because the definition they used was the duplicate that 416179d removes; they now call the exported getErrorCode from src/utils/errorCode.ts, which the file already imports. All 6 compare the returned code against a string literal, so the equivalence argument recorded on PR #1913 covers them.
Mutation-cap measurements, three denominators, all with the repository's own counting rules (scripts/stryker-diff.mjs) at pinned node v22.23.4 against committed trees at 74f5eb2:
cumulative vs main (merge-base 73931fe): extension 1006 changed executable lines, over the 500 limit.
first-parent delta (33e9200..74f5eb2), which is what the gate's own base resolution uses for a merge head: 18, under the limit.
Recording all three because the gate's reading and the content being reviewed are different numbers: the previous head 33e9200 measured 1016 cumulatively, which is the line CI printed - verbatim: Mutation gate failed: extension has 1016 changed executable lines (limit 500). Split the PR or obtain a maintainer-reviewed narrow exclusion. The split is being planned on the content, not on the refresh artifact. No exclusion is being requested; a maintainer-reviewed narrow exclusion stays the human's call.
Review-state facts carried from the census pass (posted as census batch 1 earlier on this PR, repeated here so the push carries them): row 23's premise is gone - statForObservation, recordObservation and decodeReadText have 0 references tree-wide at 416179d (0 / 0 / 0 matching lines), so that row no longer describes code in this diff. And guardedWrite has no production consumer: at 74f5eb2 git grep returns 77 lines, of which 3 are outside test files - the definition at src/core/tools/guardedWrite.ts:496 plus two doc-comment mentions (src/services/file-safety/safeWriteText.ts:368, src/utils/fileLock.ts:45). It is +394 of this PR's counted lines, so the split plan on #1991 treats it as severable rather than load-bearing, and the reviewer should weigh it as such. Census rows 4 and 19 stay answered-and-open for the reviewer.

Those 5 failures are all one class read from their messages - EPERM: operation not permitted, symlink ... raised by fs.symlink during fixture setup in services/rules/tests/rules.spec.ts, a file this merge does not touch, on a box without unprivileged symlink creation in %TEMP%; the hosted windows leg passes that file. knip and knip --production, both with --treat-config-hints-as-errors, exit 0; knip-policy test 8 pass / 0 fail. The pre-commit hook ran lint-staged over the two staged files and the committed tree equals the tree measured before the commit (c57a9d64e6d6), so no reformatting entered the commit.
Local results describe this tree only: pnpm 10.8.1 ignores the pnpm field in package.json (onlyBuiltDependencies, overrides), so a resolution difference is the first candidate if a local shard disagrees with a hosted leg.
… spelling
The three "preserve backups when another backup stat fails with ENOENT/
EACCES/EIO" rows went red on the windows platform-unit shard (Zoo-Code-Org#1914 CI run
42448302148 job 128631851822): the double compared the stat target against
os.tmpdir()'s spelling while the writer stats the canonical form of the same
file - on windows realpath spells it differently - so the injection missed,
stat succeeded, and the write resolved instead of rejecting (the failure
reads: 'promise resolved "undefined" instead of rejecting' at
safeWriteJson.test.ts:675 on run 42461215177 job 128670729075).
This adopts Zoo-Code-Org#2359's (025fb7d) hardened version of the test file, which keys
the injection on the leftover's inode and restores the fs.stat implementation
after the row (inode recycling otherwise rejects later tests). Test-only:
src/utils/safeWriteJson.ts is byte-identical between 74f5eb2 and 025fb7d,
so the json implementation stays at this branch's pin and json + its specs
still travel as one generation - with the pair windows-proven: 025fb7d's
windows platform-unit passed with this exact test file against the identical
implementation (run 42455872567 job 128654644966). Copy source named:
src/utils/__tests__/safeWriteJson.test.ts from 025fb7d (Zoo-Code-Org#2359 head).
Follow-up — head 281c24565a3b3417d6aa47bf9ebd1a04269a1939 (windows test-double fix, test-only)
281c24565 (parent 74f5eb287) adopts safeWriteJson.test.ts from 025fb7d0d: the backup-stat it.each rows keyed the fs.stat injection on os.tmpdir()'s spelling of the leftover path while the writer stats the canonical form of the same file — windows realpath spells them differently, so the injection missed and the rows failed with promise resolved "undefined" instead of rejecting. The hardened version keys the injection on the leftover's dev+ino and restores the mock implementation in a finally (inode recycling otherwise poisons later tests). Assertions unchanged; safeWriteJson.ts is byte-identical between the two refs, so this stays the same generation with its own windows-proven spec.
Proof. This PR's own CI was the red one (run 42448302148 job 128631851822, 3 rows) — on this head both legs pass: platform-unit-test ubuntu and windows success (job 128673901072-era reruns), knip success. Locally (windows): tsc 0; the safeWriteJson trio focused 69 passed/4 skipped; full misc shard 2125 passed/17 skipped, 0 failed. mutation-diff remains failure — advisory: "extension has 1006 changed executable lines (limit 500)" (job 128673851609, vs PR merge-base); the test-only commit itself moves no meter-counted lines (the recount tool reports no package delta for it).
Plainly: the two pending CI items are the extension-host-visual leg and CodeRabbit's re-review of this head (both were still running when this comment posted); the October CodeRabbit CHANGES_REQUESTED reviews are stale on prior heads. Everything mechanical is green. What remains is a human approving review, including at least one code owner (taltas, navedmerchant, edelauna, JamesRobert20).

Review comments at @src/core/tools/__tests__/readFileTool.spec.ts:
- Around line 1826-1833: Update the success-test assertions for `calledVersion`
to compare against the exact token derived from the mocked stats, rather than
accepting any token matching a numeric pattern. Apply the same expectation in
both the existing.ts and legacy.ts success tests, while preserving the `reg.get`
consistency check.
Review comments at @src/services/file-safety/safeWriteText.ts:
- Around line 739-756: Update the directory durability sync flow around the
non-Windows `dirPath` fsync so it also makes newly created ancestor directory
entries durable before reporting success. Cover nested directories created by
`mkdir` and retries, preserving the existing post-commit durability error
behavior if any required sync fails.
- Around line 460-478: Protect the supplied staging file from replacement
between validation and publication in safeWriteText: ensure the file opened for
chmod and the file published by rename remain the same validated regular file,
without allowing a substituted symlink or content. Establish a mechanism that
guarantees this identity through both operations; retain the existing
same-directory and regular-file checks.
- Around line 809-823: Update the backup cleanup in the catch handling
`originalError`: retry `fs.unlink(backupPath)` when the first unlink fails, and
if the retry also fails, report `backupPath` through the established reporting
mechanism without replacing or masking `originalError` as the primary failure.
- Around line 693-723: Update safeWriteText to accept and use the caller’s
cancellation check, then recheck cancellation immediately before starting the
commit in either the fs.link or fs.rename branch. Preserve the existing commit
behavior when the check indicates the task is not cancelled.
- Around line 693-723: Update the publication flow in safeWriteText so an
external save made after the version check cannot be overwritten or reported as
a successful write. Use a publication operation that atomically verifies the
target still matches the checked version before replacing it; a pre-rename
recheck followed by fs.rename is insufficient. Preserve the existing failIfExist
behavior and establish a safe conditional-publication mechanism for the other
path rather than weakening its safeguards.
- Around line 281-297: Update the Windows write flow using _restoreDaclWindows
so the target DACL is applied to the staged replacement before it is published,
rather than after publication. Make DACL preservation failures propagate and
abort the write instead of swallowing them as best-effort; preserve the existing
publication behavior when preservation succeeds.
- Around line 9-13: Update the backup option documentation for safeWriteText to
clarify that the backup is a copy, not a rollback source; cleanup on failure is
best-effort, and a post-publication error can reject while leaving the new
content at the target. Also describe the default behavior as making no backup.
- Around line 452-478: In the `options?.tempPath` validation, reject a staging
path whose resolved path equals the resolved target path, throwing
`StagingPathError` before entering the cleanup scope. Preserve the existing
directory and regular-file checks for other staging paths.
Review comments at @src/services/mcp/McpHub.ts:
- Around line 670-679: Update the project MCP settings creation path to pass
workspaceFolder as confineTo to safeWriteJson, using the workspace root returned
by confineForMcpWrite; preserve the existing behavior for global settings.
Review comments at @src/utils/fileLock.ts:
- Around line 81-86: Update the fallback in the lock-key inspection flow so
failures from readlink or realpath for a file symlink propagate instead of
returning absoluteFilePath and acquiring a lock for an unverified key. Preserve
the existing handling of ENOENT and ENOTDIR where applicable, and locate the
change in the catch block shown.
Review comments at @src/utils/safeWriteJson.ts:
- Around line 444-447: Update the post-commit checks in both paths to compare
against the previously resolved authorized scope root, rather than resolving the
confinement root again after staging. Reuse the original root value when calling
_assertWithinScope so replacement of the workspace root cannot make an escaped
MCP configuration appear valid.
- Around line 481-489: Update the `_assertWithinScope` failure handling in the
`confinementRoot` publish path so rollback and staging cleanup still occur if
`undoEscapedPublish` cannot restore the backup. Restore or preserve both
affected files without overwriting a peer’s file, and report the escape only
after safe recovery has been attempted.
- Around line 443-445: Track whether the replacement has committed in the
safe-write flow around _resolveScopeRoot. Once the EPERM fallback publishes the
replacement, prevent a later in-scope resolution error from restoring the old
backup; preserve rollback behavior for failures before publication.
- Around line 67-76: Update the confinement failure-mode documentation in
safeWriteJson to state that an ancestor swapped before staging can cause rename
to replace an existing outside file, and the post-commit check may remove the
published file but cannot restore the displaced one. Update the corresponding
confinement reference in McpHub to match this behavior.
- Around line 200-206: Update the escaped-file cleanup near `publishedPath` so
it cannot unlink peer content changed in place after the byte read; matching
device, inode, and size is insufficient. Use a mechanism that safely establishes
the file is still the version read at deletion time, or skip cleanup when that
cannot be guaranteed; preserve the existing protection against deleting a
replaced file.
- Around line 294-298: Update the publish-target resolution and recovery flow in
safeWriteJson so a recoverable backup is restored under the referent’s lock
before rejecting the alias as dangling. Keep unrecoverable dangling links
rejected and ensure resolution remains inside the protected block so failures
release the advisory lock.
- Around line 397-399: Update the staging flow around _copyTargetMode and
_flushFileToDisk to acquire and retain a writable descriptor before copying the
target mode, then sync the staged file through that descriptor before rename.
Avoid reopening the staged file for writing after applying a read-only mode.
- Around line 188-203: Update undoEscapedPublish so cleanup unlinks the
published path only when it can establish that the file is still the one this
operation published, including when a peer replaces it with a file containing
identical bytes. Preserve the expected-bytes check, and ensure the ownership
check cannot be invalidated by a replacement between verification and unlink;
otherwise leave the file untouched.
- Around line 443-452: Update the publish and escape-handling flow around
_assertWithinScope and undoEscapedPublish so an escaped write cannot leave a
preexisting outside target absent: prevent replacing that target or preserve and
restore its displaced contents during undo. Add a regression test that starts
with an existing outside target and verifies its contents survive the escape.
- Around line 304-307: Bind resolvedTargetPath to the held lock in the
resolveLockKey retry flow: before recovery or publication, verify that the
captured target still corresponds to lockKey, including when the alias changes
away and back. If that relationship cannot be established, do not recover or
publish to the captured target; re-resolve under the correct lock.
- Around line 587-598: Update the mode-preservation flow using targetPath and
stagedPath so publication aborts if an existing target’s mode cannot be
inspected or copied with chmod. Keep ENOENT and ENOTDIR handling for targets
that do not exist, and ensure staged sensitive data remains private until
publication can safely proceed.

Failed pre-merge checks:
- Out of Scope Changes check:
Remove the confineTo wiring and confineForMcpWrite helper from
src/services/mcp/McpHub.ts, and remove its assertions from McpHub.spec.ts.
Remove the confineTo option and undoEscapedPublish from
src/utils/safeWriteJson.ts, and remove the tests that cover them
(safeWriteJson.test.ts and safeWriteJson.lockKey.spec.ts sections for
confinement). Keep the fileLock.ts and safeWriteText.ts changes that
guardedWrite uses. Alternatively, link the issue that owns these changes and its
acceptance criteria. Then update the PR description so its scope statement
matches the remaining diff.
- Regression Evidence:
Add guarded-write tests that inject a post-commit durability failure, verify the
refreshed token and rethrown error, and verify a subsequent write. Cover token
lookup failure without replacing the original error.
- Security Boundaries:
Enforce workspace containment through publication, including protection against
parent relocation. Use no-follow directory handles and descriptor-relative
operations where supported, and fail closed where the guarantee cannot be
provided. Add a regression test that moves the parent and installs a symlink
between validation and commit.
- Persistence Integrity:
Only attempt escape undo after a confirmed successful publish. Verify the
published file identity against the staged file before removal, and add a
regression test for a failed rename with an identical outside file.
- Lifecycle Resource Cleanup:
Retain and retry cleanup of DACL dumps from both save paths. If cleanup still
fails, report the orphan path or arrange reliable cleanup. Also handle failed
no-replace staging cleanup without losing the staging path or deleting a
replacement file. Add regression tests for cleanup failures.

Learning: In src/utils/fileLock.ts, acquireFileLock canonicalizes the advisory lock key through canonicalLockPath, while withFileLock passes the caller's lexical absolute path to its operation. Keep lock-key canonicalization separate from operation-path handling: Windows realpath can return an 8.3 short path, and substituting that path can change caller comparisons or unlink behavior. Task-history deletion and JSON writes share this lock abstraction.

Not changing. The lstat gate (regular-file, same-directory) is inherent-TOCTOU: no check on a path can bind the file it inspected to the file the later open acts on, and the staging path arrives under a caller-staged trust model (the caller opened it in the target directory moments earlier). Closing this fully needs openat-class APIs that Node does not give us; adding a re-check before staging would relocate, not remove, the same residual window the guard already documents.

@easonLiangWorldedtech Understood. A path re-check would not remove the race. I understand your decision to accept the risk under the caller-staged trust model. If another process can replace tempPath, the risk remains.

Prevent external saves from being overwritten during publication.
If an editor saves after the version check, the advisory lock does not exclude that save. The rename can overwrite it and report success instead of rejecting a stale write. Ensure publication preserves the newer version. A pre-rename check alone would still leave a race.

Not changing (both halves). guardedWrite.ts re-checks target identity and bytes under the publish lock (the block just before delegating the rename) - exactly the relocation point you name - and safeWriteText additionally re-opens and fsyncs the staged file before publishing. Moving that verification inside safeWriteText would just move the same residual TOCTOU window from the publish lock's door to its inside; the two checks serve different callers of safeWriteText (guarded write vs json publish) and folding them in would force a json-side policy onto text writers that never opt into identity pinning. The window you describe is the one the publish lock exists to shrink, not one a location change can close.

@easonLiangWorldedtech Understood. I won’t press for these changes.
You are right that moving the version check would not eliminate the race. The publish lock excludes cooperating writers, but an external editor can save after the check and before fs.rename. Syncing the staged file does not check the target’s version.
The cancellation finding is separate. The last check occurs before await verifyTarget() in guardedWrite.ts at Line 281. Cancellation during that await or staging can still reach the commit. A later check would catch some of those cases, but could not cancel a filesystem operation already submitted.

Sync ancestor directory entries before reporting success.
On Linux, when mkdir creates a nested directory tree, syncing only dirPath does not ensure the new ancestors are durable. A crash can remove the successfully published file with its parent tree. Sync the necessary ancestor directory entries before reporting success, including on retries.

Not changing. jsonFileRecovery's durability posture is documented at the recovery block it points at: the rename protocol buys process-crash safety (rename is atomic on POSIX and Windows for same-volume replace); power-loss durability of the content is a bonus, not the promise - the fsync here is best-effort (Failed to flush warns, does not reject) precisely because the layer above owns the stronger contract. Raising it to a hard failure would reject writes on filesystems where fsync is slow-but-correct for no additional crash-safety the rename protocol does not already provide.
For clarity, the finding concerns power-loss durability of newly created ancestor directories in safeWriteText, not process-crash recovery in jsonFileRecovery. safeWriteText already reports a failure to sync the immediate parent as PostCommitDurabilityError.

Learning: In src/services/file-safety/safeWriteText.ts, safeWriteText intentionally treats all post-commit parent-directory fsync failures, including EINVAL and EBADF, as PostCommitDurabilityError. A successful return must not imply durability that the filesystem did not grant. The error identifies the target where committed content remains, while callers decide how to handle retries. Do not recommend silently tolerating unsupported directory-fsync errors.

Not changing. The asymmetry is deliberate and documented: safeWriteText's teardown discards its backup silently (safeWriteText.ts, teardown block) because its backup is a copy taken before staging and is never a restore source - an inert orphan. json's post-undo unlink reports failure because there the leftover is not inert: it feeds recoverInterruptedReplacement on the next writer's lock pass, so a silently-undeletable orphan would keep re-entering that recovery path. Reporting there and silence here is the correct split, and the swt teardown comment records the rule.

@easonLiangWorldedtech Understood. safeWriteText uses a backup copy, not a rollback source, and its error-path cleanup is best-effort. I accept your decision not to change it.

Not changing. fileLock.ts documents the fail-open posture at the realpath resolution: on non-ENOENT/ENOTDIR failures (EPERM, EACCES) it falls back to the raw path so locking still exists under hostile-directory conditions; making the fallback fail-closed would deny locking entirely to every writer in such a directory - a worse availability/consistency trade than the documented one - and the lock is advisory in this codebase by its own comment. The readlink .catch(() => undefined) follows the same rule: an unreadable link degrades to key-by-path, not to an exception that strands the lock.
If both readlink and realpath fail for an alias, the fallback can still use a different lock key from the referent. In that case, serialization is not guaranteed.

Not changing. undoEscapedPublish already refuses to delete an unproven file: it re-stats the escape spelling and unlinks only when the bytes are identical to what we wrote and dev/ino/size confirm it is still our landed file. A same-bytes peer that swapped in after our rename is indistinguishable from our own bytes at that moment by any means short of provenance Node does not expose; guessing (e.g. leaving it) would strand a duplicate copy of exactly the bytes the user just asked to write.
The original finding concerns a different path: if the fallback replacement rename fails, the escape error skips backup rollback and staging cleanup.
Separately, the supplied code compares the two stat results with each other, not with the staged file’s identity. If the rename fails and an outside file has identical bytes, undoEscapedPublish can remove that file. This is the saved blocking persistence-integrity finding; I have not verified the check against the current revision.

Not changing. The undo path is already bytes-and-identity guarded (same argument as 4505599238): we unlink at the escaped spelling only when stat says it is a same-size, same-dev/ino file whose content is byte-identical to what we published. A peer that replaced us there with different bytes makes the undo a no-op automatically; with identical bytes the two files are the same content either way, and refusing to remove it would leave the user's file published twice.
In the supplied code, the two stat calls compare the file with itself, not with the staged file. A peer’s replacement with identical bytes can therefore pass both checks and be deleted. A replacement after the final stat can also be deleted.
The saved persistence-integrity gate is blocking, but its freshness is unverified. Before merging, I recommend rerunning that check and testing both an identical-content replacement and a failed rename.

Not changing. A -> B -> A within the re-derivation loop's own observation window defeats every path-level check the loop and the final check perform: both iterations see a legitimately inside spelling, and no number of re-resolves can see the trip that already completed. Closing that variant needs directory-descriptor-relative operations (openat/renameat-style) Node does not expose; the option doc names them as the missing primitive rather than pretending the loop closes ABA.

Not changing (posture, now explicit in the doc). When the publish target was swapped with the peer's own file between check and commit, the commit rename necessarily replaces that peer file; the escape detector then reports and the undo removes our landed bytes - it does not resurrect the peer's content, because the protocol never owned a right to: the user asked for this content at this path, and every failure path is reported (fail-closed report, narrowed restore claim) rather than guessed at silently restoring bytes whose ownership cannot be proven. The rewritten confineTo doc states exactly this outcome set.

CodeRabbit re-review of 281c245 found the staging write loop can spin
forever if the filesystem returns a zero-byte write: fsSync.writeSync may
return 0 without advancing the offset, and the loop neither fails nor
reaches cleanup. Fail on a non-positive result so the staged file is
torn down instead (threads 4505636442/4505639488-class; adopted), with a focused spec test pinning the fail-fast (one attempt, no fsync, no publish, staged file unlinked).
Two comments described machinery that does not exist or never shipped:
- _flushFileToDisk flushes the staged file before the commit rename, not
'the directory that holds a freshly committed file' (thread 4505639534-class).
- The integration spec's DACL comment cites a DaclRestoreError throw and a
focused unit test that no tree contains; a failed restore is best-effort
and the publish resolves (thread 4505642563-class; adopted).
Same-class threads on the sibling branches converge with this unit's
declared merge order (U5 first) rather than being swept sideways.

This PR adds
guardedWrite, the single publish entry point every write tool will use.

Re-review round (b95efab)
Two Minor findings fixed with regression tests and negative controls:
inode at the target that nothing fsynced (the staged inode's pre-commit
fsync does not cover it; step 4b flushes only the parent directory).
The copied target is now opened and fsynced right after the fallback;
failure reports
PostCommitDurabilityError(new copied-target messagevariant) instead of a swallowed success. NC: removing the flush reddens
exactly the two new copy tests.
canonicalLockPathrecursed with a freshper-frame hop bound, so a cycle whose length does not divide 8
(a -> b -> c -> a) recursed forever on the lock path. The recursion now
carries a total budget of 64 hops; when spent, the next frame walks
zero hops and canonicalizes the cycle member it stopped on. NC: the
unbounded form reddens exactly the new 3-cycle test (timeout).
Windows flush-access fix (a75f05c)
The copy-fallback target flush now opens with
"r+"(WindowsFlushFileBuffersrequiresGENERIC_WRITE; a read-only handle could failthe flush and report a false durability error on published bytes). The
test pins the write-access flag; NC: reverting to
"r"reddens exactlythat test.
The fallback is removed: those three
codes now raise
UnsupportedFilesystemError(target path and causeattached) and the write fails closed - a filesystem that cannot prove
the flush cannot prove the publish. The five fallback tests collapse to
one fail-closed test that drives all three codes through the same
publish; negative control: re-adding the copy fallback reddens exactly
that test. 66/66 green at this head.
Security Boundaries disposition (already-registered family). The row
repeats the ancestor-swap claim (openat/renameat-style directory-identity
