Description
Next batch of the command migration. db:create, db:seed, db:table, migrate, migrate:refresh, migrate:rollback and migrate:status now write through $this->getOutput(), and their remaining prompts ask through $this->getPrompt(), so a Prompt override reaches them.

• Tables are written through Table in place of CLI::table().
• The private methods of db:table take the output from execute().

No behaviour changes. The existing tests pass unchanged.
