Make OpenRouter the default provider and refresh documentation - #16

Settings.provider and Settings.from_env now default to "openrouter" when LLM_PROVIDER is unset. Breaking: callers relying on the anthropic default must set LLM_PROVIDER=anthropic or Settings(provider="anthropic"). Co-Authored-By: Claude Sonnet 5.5 <noreply@anthropic.com>
