feat(task): generate a session title from the first message - #2349
This PR adds automatic session title generation for new root tasks with minimal file changes (8 files changed, no provider adapters modified).
src/core/task/session-title.ts: Generates a concise 5–10 word title in the user's language via single-shot completion with a 15-second hard timeout. Strips <system-reminder> blocks using linear search (indexOf/slice) to prevent polynomial ReDoS. Falls back to the first 42 characters of the sanitized user message if the model fails or times out.
src/core/task/Task.ts: Dispatches title generation as a fire-and-forget task in Task.startTask after the first user message is persisted. Subtasks and resumed tasks skip title generation.
src/core/task-persistence/TaskHistoryStore.ts: Adds setTitleIfEmpty with an atomic read-modify-write file lock and disk reconciliation. A title is only written if none exists on disk, protecting manual titles and clears.
src/core/webview/ClineProvider.ts: Adds applyGeneratedSessionTitle to update history and broadcast updates to the webview.

When a new root task starts, one background single-shot completion turns
the first user message into a short title in the language of the user.
The request has a 15-second limit and falls back to the first 42
characters of the cleaned message on failure. The write lands inside the
history store lock and only when the item has no title, so a title the
user set by hand is never replaced. Subtasks and resumed tasks keep
their existing names.

New conversations can now receive a title generated from the first message, without delaying startup.
If a title can’t be generated, a short fallback based on the message is used.
Existing titles, including manually renamed titles, are preserved. Updated titles appear in task history when the view is open.
New root tasks generate a session title in the background after storing the first message. The generated title is stored only if the history item has no title. The generation function removes reminder blocks, sanitizes provider output, and uses a short fallback when generation fails or returns no usable title.

Description
This PR adds automatic session title generation for new root tasks with minimal file changes (8 files changed, no provider adapters modified).
src/core/task/session-title.ts: Generates a concise 5–10 word title in the user's language via single-shot completion with a 15-second hard timeout. Strips<system-reminder>blocks using linear search (indexOf/slice) to prevent polynomial ReDoS. Falls back to the first 42 characters of the sanitized user message if the model fails or times out.src/core/task/Task.ts: Dispatches title generation as a fire-and-forget task inTask.startTaskafter the first user message is persisted. Subtasks and resumed tasks skip title generation.src/core/task-persistence/TaskHistoryStore.ts: AddssetTitleIfEmptywith an atomic read-modify-write file lock and disk reconciliation. A title is only written if none exists on disk, protecting manual titles and clears.src/core/webview/ClineProvider.ts: AddsapplyGeneratedSessionTitleto update history and broadcast updates to the webview.Test Procedure
